Clinical Presentation and Management of Malignant Psoas Syndrome: A Scoping Review of Case Reports and Case Series.

Malignant psoas syndrome (MPS) is a rare and underreported clinical syndrome that significantly impacts the quality of life of cancer patients through metastatic infiltration of the iliopsoas muscle. Patients suffering from MPS often present with painful hip flexion, loss of mobility, and immense pain in their legs and back. The current literature describing the clinical presentation, management, and prognosis of MPS is limited primarily to case reports and outdated literature reviews. There remains a gap in the current knowledge of MPS and in the management of this complex cancer-related pain

Assessing Clinician Prescribing Practices for Prophylaxis and Treatment of Acute Phase Responses Following IV Bisphosphate Infusions for Osteoporosis: A Pilot Survey-Based Study.

OBJECTIVE: There appear to be no definitive guidelines on optimal prevention and management of acute phase responses (APRs) following intravenous bisphosphonates (IVBP) for osteoporosis. Our objective was to identify physician prescribing practices for supplementary therapy aimed at managing APRs and whether these were thought beneficial. METHODS: An online survey was distributed to members of key Australian societies for geriatrics, rheumatology and endocrinology (membership n ≈ 2900) with questions assessing if respondents prescribed IVBP, their experience managing APRs including medication

A Literature Review of Renal Artery Stenosis: Presentation, Diagnosis, and Management Options in Secondary Hypertension.

Renal artery stenosis (RAS) remains a critical and often underdiagnosed cause of secondary hypertension due to its insidious progression. This traditional review examines the epidemiology, various presentations of RAS, pathophysiology, and diagnostic modalities, with a focus on treatment options based on selected clinical trials. The prevalence of RAS is notably higher in individuals with systemic atherosclerotic conditions, making atherosclerosis the most common cause of RAS. Less frequently, fibromuscular dysplasia may be involved. Although RAS presents as hypertension in most cases, some pe

Surgical Site Infection Owing to Mycobacterium mageritense After Immediate Breast Reconstruction Using a Deep Inferior Epigastric Perforator Flap.

Mycobacterium mageritense is a rare, rapidly growing, nontuberculosis mycobacterium that belongs to type IV of the rapidly growing mycobacteria. These bacteria are found in soil and water, and cause localized skin and soft tissue infections; however, they are challenging to culture, leading to diagnostic delays. To our knowledge, there have been 12 reported cases of surgical site infections (SSIs) caused by M. mageritense, with only 2 cases following breast reconstruction. A 51-year-old woman underwent nipple-sparing mastectomy and immediate breast reconstruction using a deep inferior epigastr

The epidemiology and management of spontaneous spinal epidural abscesses in children: a single-center experience.

OBJECTIVE: Spinal epidural abscesses (SEAs) are rare infections in children but can lead to significant neurological sequelae. The authors sought to describe the presentation, management, and outcomes of children with these infections at their institution. METHODS: This was a retrospective study of pediatric patients admitted with SEA to a quaternary children's hospital between 2013 and 2023. Clinical characteristics including laboratory, surgical, and antimicrobial data were collected and analyzed. RESULTS: Fourteen patients (median age 3.7 years) had SEAs. Thirteen (93%) patients developed

Role of the radiologist in the diagnosis and management of the two forms of hepatic echinococcosis.

Echinococcosis is a parasitic disease caused by two zoonotic tapeworms (cestodes) of the Echinocococcus genus. It can be classified as either alveolar or cystic echinococcosis. Although the two forms differ significantly in terms of imaging findings, they share similarities in terms of management and treatment. In parallel to medical treatment with albendazole (ABZ), and surgery, historically used in these diseases, various imaging-guided interventional procedures have recently emerged (drainage, stenting, or Puncture, aspiration, injection, and reaspiration (PAIR)). These options open up a ne

The urgent need to improve hypertension care.

Hypertension is the most prevalent health problem among adult primary care patients, but its recognition and treatment are suboptimal. Although there is ample evidence from several large-scale randomized, controlled studies that treatment of hypertension reduces morbidity and mortality, current management of hypertension is characterized by underdiagnosis, misdiagnosis, undertreatment, overtreatment, and misuse of medications. As a result, roughly 75% of the estimated 50 million adults with hypertension in the United States are at increased risk for vascular complications. Optimal therapy requ

Beyond Swelling: Clinical Insights into the Diagnosis and Management of Hereditary Angioedema.

Hereditary angioedema (HAE) is a rare genetic disorder characterized by recurrent episodes of severe, non-pitting edema, most commonly caused by a deficiency or dysfunction of C1 esterase inhibitor (C1-INH), resulting in excessive bradykinin production. HAE subtypes differ according to C1-INH levels, functional activity, underlying genetic variants, and increasingly recognized molecular mechanisms. Awareness of HAE outside specialized medical fields remains limited, and recurrent angioedema may arise through different pathophysiological mechanisms, which together contribute to frequent misdiag

[A Wolf in Sheep's Clothing - Manifestation and Diversity of Immune-Related Adverse Events in Cancer Immunotherapy].

Immunotherapy, particularly immune checkpoint inhibitors, has significantly impacted cancer treatment by enhancing the immune system's ability to identify and attack cancer cells. However, this treatment introduces unique immune-related adverse events (irAEs), which require careful management due to their potential to affect multiple organ systems. IrAEs can manifest during or after treatment, affecting organs such as the gastrointestinal tract, endocrine glands, lungs, and liver, with symptoms ranging from mild to life threatening. While irAEs are distinct from those seen in traditional cance

Current treatment of hereditary nephrogenic diabetes insipidus in children.

Nephrogenic diabetes insipidus (NDI) is a rare disorder characterized by the inability of the kidneys to respond to arginine vasopressin, leading to polyuria, polydipsia, and chronic dehydration. This condition often results in severe complications, including growth failure, behavioral problems, and irreversible brain damage, particularly in young children with hereditary forms. Early diagnosis and effective management are critical for minimizing these complications and improving patients' quality of life. Current treatments focus primarily on symptom management through dietary interventions,

Anticoagulation approach in morbid obesity: a comprehensive review on venous thromboembolism management.

Obesity is a recognized risk factor for venous thromboembolism (VTE), associated with distinct challenges in managing anticoagulation therapy. There is still limited evidence regarding the impact of extreme body weight on the pharmacokinetics, pharmacodynamics, efficacy, and safety of various anticoagulant medications. To our knowledge, this is the first comprehensive review to address both prophylactic and therapeutic anticoagulant dosages specifically for managing VTE in patients with a body mass index (BMI) ≥40 kg/m2 or weight ≥120 kg. Our aim was to synthesize the findings of relevant stud

Strategical Ways for Dental Anxiety Management Prior to Third Molar Extraction.

A patient experiencing moderate to severe anxiety often skips regular check-ups, choosing to visit only when necessary. As a result, their oral health worsens. Various variables can affect anxiety such as age, gender, culture, and previous experiences. One of the most frequent oral surgical procedures is extracting the third molar, which is accompanied by severe pain, tension, and anxiety, the use of invasive equipment, and local anesthetic. A combined effect was found of four factors, including the patient, dental employees, setting, and dental procedure, which can trigger and affect dental a

Methylene Blue, a Unique Topical Analgesic: A Case Report.

Background: Rectal prolapse is a circumferential, full-thickness protrusion of the rectum through the anus, which, if not properly managed, may become incarcerated and pose a risk of strangulation. This pathology is rarely a medical emergency unless a complication is encountered. Such complications include infection, necrosis, perforation, incarceration, and uncontrolled pain. Case Presentation: We report a case of an elderly patient with pain associated with chronic rectal prolapse. Surgical intervention had been ruled out, and there had been no pain relief after using systemic analgesics. Ca

Experimental drugs for supraventricular tachycardia: an analysis of early phase clinical trials.

INTRODUCTION: Supraventricular tachycardias (SVT) are a diverse group of commonly encountered arrhythmias arising at or above the atrioventricular (AV) node. Conventional anti-arrhythmic medications are restricted by extensive side-effect profiles and limited efficacy. Catheter ablation has emerged as a first-line therapy for many arrhythmias but is not a suitable option for all patients. This has prompted the exploration of novel pharmacological approaches targeting specific molecular mechanisms of SVT. AREAS COVERED: This review article aims to summarize recent advancements in pharmacologic

Management of Spinal Muscular Atrophy in the Adult Population.

Spinal muscular atrophy (SMA) is a group of neurodegenerative disorders resulting from the loss of spinal motor neurons. 95% of patients share a pathogenic mechanism of loss of survival motor neuron (SMN) 1 protein expression due to homozygous deletions or other mutations of the SMN1 gene, with the different phenotypes influenced by variable copy numbers of the SMN2 gene. Advances in supportive care, disease modifying treatment and novel gene therapies have led to an increase in the prevalence of SMA, with a third of SMA patients now represented by adults. Despite the growing number of adult p
