Spinal neuronal inhibition and EEG synchrony by electrical stimulation in subcortical forebrain regions of the cat.

In cats anaesthetized with sodium pentobarbital and 70% N2O, single lumbar dorsal horn neurons were excited by controlled noxious radiant heating of glabrous hindpaw skin. The EEG was recorded from the pericruciate cortex and posterior lateral gyrus. Subcortical forebrain sites where electrical stimulation inhibited dorsal horn neuronal heat-evoked responses contralaterally were identified by mapping the caudate nucleus, internal capsule, septum, nucleus accumbens and basal forebrain regions. Inhibitory sites were mainly located in the ventral forebrain (ventral septum, diagonal band, basal fo

The effect of routine intraoperative neuromonitoring on thyroidectomy complications: A propensity score-matched retrospective cohort study.

Intraoperative neuromonitoring (IONM) is commonly used during thyroid surgery to facilitate recurrent laryngeal nerve (RLN) identification and potentially reduce related postoperative complications. However, the efficacy of routine IONM in decreasing complication rates remains debated. In this study, we investigated the impact of IONM on postoperative complications after thyroidectomy. This retrospective single-center study included 214 thyroidectomy operations performed between 2021 and 2024. Patients were divided into 2 groups: Group 1 (n = 107) underwent surgery with routine IONM, and Group

Unilateral lower limb suspension affects motor unit action potential train characteristics in a size-dependent manner during high force isometric knee extension.

Unilateral lower limb suspension (ULLS) mimics spaceflight-induced decreases in voluntary muscle strength and muscle recruitment. The purpose of this study is to examine the effects of short-term ULLS on quadriceps motor unit action potential train (MUAPT) characteristics during high-intensity isometric contractions. Thirteen healthy adults (7 women and 6 men; age: 18-44 yr) completed bilateral knee extension assessments, including maximal voluntary isometric contraction (MVIC), interpolated twitch, and trapezoidal ramp contractions, before and after 13-day ULLS. Surface electromyography (sEMG

Diagnostic value of D-wave and motor evoked potentials in intramedullary spinal cord tumor surgery: a temporal analysis of predictive accuracy.

OBJECTIVE: The surgical resection of intramedullary spinal cord tumors (IMSCTs) is associated with significant morbidity. Intraoperative neuromonitoring (IONM) with transcranial myogenic motor evoked potentials (m-MEP) and D-wave aims to reduce new postoperative motor deficits. Still, their comparative ability to predict short- and long-term outcomes is incompletely defined. We evaluated the diagnostic performance of m-MEP and D-wave monitoring at multiple postoperative time points and examined their influence on intraoperative decision-making. METHODS: In this single-institution observationa

The role of the blink reflex in predicting facial nerve outcomes following vestibular schwannoma surgery: a 4-year retrospective review.

OBJECTIVE: The objective of this study was to evaluate the prognostic value of intraoperative blink reflex (BR) monitoring in predicting postoperative facial nerve outcomes following vestibular schwannoma (VS) resection. Given the limitations of existing intraoperative neuromonitoring (IONM) techniques, this study aimed to determine whether BR loss correlates with worsened facial nerve function, and to assess its potential as an adjunctive tool for surgical decision-making. METHODS: The authors conducted a retrospective review of adult patients who underwent VS resection between January 2021

Re-organization of motor functions after early brain damage: Direct evidence from intraoperative neuromonitoring in four cases.

BACKGROUND: The re-organization of motor functions is a sign of adaptive brain plasticity to early brain damage and understanding its mechanisms may play a key role in the future treatment modalities. This study provides direct evidence of neural plasticity in four patients with early brain damage, who underwent epilepsy surgery due to drug-resistant epilepsy. METHODS: In this retrospective analysis, four patients with early brain damage who underwent resective or disconnective epilepsy surgery were examined. Intraoperative neuromonitoring was employed to preserve motor function during surger

Robotic Assisted Transcranial Doppler Monitoring in Acute Neurovascular Care: A Feasibility and Safety Study.

BACKGROUND: Transcranial color Doppler (TCD) is currently the only noninvasive bedside tool capable of providing real-time information on cerebral hemodynamics. However, being operator dependent, TCD monitoring is not feasible in many institutions. Robotic assisted TCD (ra-TCD) was recently developed to overcome these constraints. The aim of this study was to evaluate the safety and feasibility of cerebral monitoring with a novel ra-TCD in acute neurovascular care. METHODS: This is a two-center prospective study conducted between August 2021 and February 2022 at Padua University Hospital (Pad

Probabilistic stimulation mapping from intra-operative thalamic deep brain stimulation data in essential tremor.

Deep brain stimulation (DBS) is a therapy for Parkinson's disease (PD) and essential tremor (ET). The mechanism of action of DBS is still incompletely understood. Retrospective group analysis of intra-operative data recorded from ET patients implanted in the ventral intermediate nucleus of the thalamus (Vim) is rare. Intra-operative stimulation tests generate rich data and their use in group analysis has not yet been explored.Objective.To implement, evaluate, and apply a group analysis workflow to generate probabilistic stimulation maps (PSMs) using intra-operative stimulation data from ET pat

Does an improvement in cord-level intraoperative neuromonitoring data lead to a reduced risk for postoperative neurologic deficit in spine deformity surgery?

PURPOSE: To determine if an improvement in cord-level intraoperative neuromonitoring (IONM) data following data loss results in a reduced risk for new postoperative motor deficit in pediatric and adult spinal deformity surgery. METHODS: A consecutive series of 1106 patients underwent spine surgery from 2015 to 2023 by a single surgeon. Cord alerts were defined by Somatosensory-Evoked Potentials (SSEP; warning criteria: 10% increase in latency or > 50% loss in amplitude) and Motor-Evoked Potentials (MEP; warning criteria: 75% loss in amplitude without return to acceptable limits after stimulat

Recording cortico-cortical evoked potentials of the human arcuate fasciculus under general anaesthesia.

OBJECTIVE: We examined the feasibility of using cortico-cortical evoked potentials (CCEPs) to monitor the major cortical white matter tract involved in language, the arcuate fasciculus (AF), during surgery under general anaesthesia. METHODS: We prospectively recruited nine patients undergoing surgery for lesions in the left peri-sylvian cortex, for whom awake surgery was not indicated. High angular resolution diffusion imaging (HARDI) tractography was used to localise frontal and temporal AF terminations, which guided intraoperative cortical strip placement. RESULTS: CCEPs were successfully

Usefulness of external anal sphincter EMG recording for intraoperative neuromonitoring of the sacral roots-a prospective study in dorsal rhizotomy.

BACKGROUND: In conus medullaris and cauda equina surgery, identification of the sacral nerve roots may be uncertain in spite of their anatomical/radiological landmarks. Mapping the sacral roots by recording the muscular responses to their stimulation may benefit from EMG recording of the External Anal sphincter (EAS) in addition to the main muscular groups of the lower limbs. METHOD: In a consecutive series of 27 lumbosacral dorsal rhizotomy (DRh), authors carried out a prospective study on the reliability of the EMG recording of the EAS for identification of the S1 and S2 sacral roots.
