[Acute-Phase Medical Treatment:Treatment to "Reduce" the Risk of Symptom Worsening].

Acute medical management after acute ischemic stroke (AIS) is often perceived as a supportive or conservative treatment, following decisions regarding intravenous thrombolysis and mechanical thrombectomy. However, it should be regarded as an active strategy to reduce the risk of neurological deterioration and support subsequent recovery. Acute medical treatment can be divided into two components: prevention of recurrent stroke or infarct expansion, and prevention of systemic complications that may delay recovery. This approach mainly aims at preventing early neurological deterioration (END), a

Combined mutations of NKX2-1 and surfactant protein C genes for refractory low oxyhemoglobin saturation and interstitial pneumonia: A case report.

RATIONALE: Mutations of the NKX2-1 gene are associated with brain-lung-thyroid syndrome, which is characterized by benign hereditary chorea, hypothyroidism, and pulmonary disease with variable presentation. Surfactant protein C (SFTPC) gene mutations result in chronic interstitial lung disease in adults or severe neonatal respiratory distress syndrome. PATIENT CONCERNS: Recurrent hypoxemia was observed shortly after birth in a baby at a gestational age of 40 weeks and birth weight of 3150 g. The need for respiratory support gradually increased. He had hypothyroidism and experienced feeding di

Prenatal and Postnatal Ultrasound Findings of Congenital Diaphragmatic Hernia Associated With Myelomeningocele, Chiari II Malformation, and Renal Dysplasia.

Congenital diaphragmatic hernia (CDH) associated with neural tube defects is exceedingly rare. We report a female neonate (2550 g) born at 35 weeks (Apgar 2/4/5) with severe left-sided Bochdalek CDH, lumbosacral myelomeningocele, Chiari II malformation, and unilateral renal dysplasia. Maternal history revealed no folic acid supplementation. Despite intensive care and inhaled nitric oxide for persistent pulmonary hypertension, the patient died at 72 h. This case highlights the value of prenatal ultrasound in detecting multisystem anomalies and supports the concept of early embryologic mesoderma

Prehabilitation in Cardiac Surgery: Part 1: From Phenotype-driven Risk Stratification to Individualized Multimodal Preoperative Optimization.

Cardiac surgery patients increasingly present with frailty, sarcopenia, malnutrition, anemia, and psychological distress, contributing to high perioperative risk and impaired recovery. Prehabilitation has emerged within Enhanced Recovery after Surgery cardiac frameworks as a proactive strategy to enhance physiologic and psychological resilience before surgery. This article summarizes current evidence on risk stratification and the core components of multimodal prehabilitation, including nutrition, exercise, patient blood management, and psychological support. Emphasis is placed on phenotype-dr

Medical intestinal rehabilitation.

PURPOSE OF REVIEW: Medical intestinal rehabilitation is a part of the very complex management of patients with intestinal failure. The goal is to achieve enteral autonomy and minimize need for parenteral nutrition and hydration. In this manuscript, we will review the strategies to achieve this goal with dietary optimization and pharmacologic interventions. RECENT FINDINGS: We will review the most updated recommendations on medical management of patients with intestinal failure. SUMMARY: Medical intestinal rehabilitation is just a portion of a multistep strategy that aims to minimize need of

Upper digestive bleeding secondary to duodenal infiltration due to pancreatic cancer: a therapeutic challenge.

A 68-year-old woman with stage IV pancreatic adenocarcinoma (liver and lymph node metastases) on first-line treatment with gemcitabine. As a non-oncological comorbidity, the patient was anticoagulated with enoxaparin 8000 IU/24 hours because she had a mitral valve prosthesis. The patient made a medical consultation for presenting vomits which looked like coffee grounds and melaena. In the complete blood count, a hemoglobin of 7.5 g/dL was detected. Transfusion support, pantoprazole infusion (80 mg in 500 cc of 0.9% SSF every 12 hours) and parenteral nutrition were prescribed. Tranexamic was no

An Accidental Nutritionist.

My career as an accidental nutritionist began with my immersion in cholera control, a cyclone disaster, a smallpox epidemic, and formal training in ophthalmology and epidemiology. Interest in blindness prevention inexplicably led me to (re)pioneer the effects, treatment, and prevention of vitamin A deficiency, while faced with intense criticism by many leading scientists in the nutrition community. The resulting efforts by the World Health Organization and UNICEF in support of programs for the global control of vitamin A deficiency still face vocal opposition by some senior scientists, despite

Intractable hyperemesis gravidarum in a patient with type 1 diabetes.

Hyperemesis gravidarum is not uncommon. Its pathogenies is multifactorial but not fully understood. We present a case of a middle class, Caucasian pregnant woman aged 24 years with coexisting type 1 diabetes, who had severe hyperemesis gravidarum from the sixth week of pregnancy and was resistant to all standard and off-the-label treatments raising questions about the pathogenesis of hyperemesis gravidarum. She was managed with a multidisciplinary approach and was supported with total parenteral nutrition till she had an emergency caesarean section in the 29th week of pregnancy. Her vomiting s

Syngnathia Between the Palate and Mouth Floor, Cleft Palate, and Funnel Chest.

Syngnathia is a rare malformation involving soft tissue and/or bony adhesions between the maxilla and mandible. Less than 40 patients have been reported in the literature. Here the authors report a 6-month-old infant diagnosed as syngnathia of the palate and mouth floor combined with cleft palate and funnel chest in the Department of Oral and Maxillofacial Surgery at Henan Provincial People's Hospital in January 2015. The authors discussed and evaluated the diagnostic and treatment difficulties on surgical and anesthetic procedure. There is no standard treatment protocol, but early treatment i

Feline idiopathic cystitis.

While FIC remains a diagnosis of exclusion, studies over the last two decades suggest that it results from complex interactions between the urinary bladder, neuroendocrine system and environmental factors, and stress appears to play a role in the pathogenesis. The self-limiting nature of FIC and importance of environmental factors on recurrence of clinical signs emphasize the need for controlled, prospective, double-blinded clinical studies to determine the best management options. Current best evidence for initial management of acute, non-obstructive FIC supports a specific nutritional recomm

[Diagnosis and therapy are still challenging, despite the rapid growth of knowledge].

Psychiatric diagnoses are not reflections of the aetiology of the disorder, but rather lists of symptoms with considerable overlaps, which hamper research and may cause confusion. The diagnoses of autism spectrum disorder, attention deficit hyperactivity disorder and tic disorder are often comorbid along with a number of other symptomatic syndromes. Individual immune responsivity is possibly involved in pathophysiological mechanisms. Multiple environmental factors may contribute to the clinical phenotypes. Recent research supports to some extent the involvement of dietary and nutritional facto

Healing of a pyoderma gangrenosum at the site of a percutaneous endoscopically sited gastrostomy tube without tube removal.

Pyoderma gangrenosum (PG) is a rare but important cause of pain and morbidity, and is associated with malignancy and shows pathergy. Occurrence at a percutaneous endoscopic gastrostomy (PEG) site is rare and may prompt PEG tube removal. This case describes management of PG at a PEG site in a man with squamous cell carcinoma of the orophayrnx. Successful healing of the lesion was achieved despite the PEG tube remaining in situ. Care was delivered within the hospice setting with support from the local dermatology service. Infection of the wound was treated with a recently licensed antimicrobial

Biliary atresia: how medical complications and therapies impact outcome.

Biliary atresia (BA) is a progressive fibro-obliterative disease of the extrahepatic biliary tree that presents with biliary obstruction in the neonatal period. Untreated, BA is a uniformly fatal disease and, yet, even with our existing therapies, at least 50% of children with BA will undergo liver transplantation by the age of 2 years. Current treatment strategies are, at best, palliative; they focus on prompt diagnosis, supportive nutritional care and interventions for sequelae. The purpose of this article is to discuss the current treatment paradigm for BA and to assess the impact these str

Combined rTPA and aspirin therapy for intracardiac thrombosis in neonates.

We describe a case of a very low birth weight premature female twin with intracardiac thrombosis. Her condition was consistent with neonatal sepsis, and she was treated with advanced life support, antibiotic therapy, parenteral nutrition, blood transfusion, and central venous catheterization. Infective endocarditis was suspected, and a large intracavitary thrombus was detected by two-dimensional Doppler echocardiography. Surgical procedure was not only technically difficult but also highly lethal, being contraindicated in some cases. Consequently, the use of the thrombolytic recombinant tissue

Bedside postpyloric placement of weighted feeding tubes.

A pilot study to evaluate a technique for the bedside placement of small-bore, weighted feeding tubes past the pylorus in critically ill patients is described. The technique involves the use of a #8 or #10 French small-bore, tungsten-weighted feeding tube, specific patient positioning, and defined insertion techniques, including gastric distention with air. The tubes were placed at the bedside in the intensive care unit by the nutrition support team nurse. Successful postpyloric placement was achieved in 85 (83%) of 103 patients. Approximate time for successful insertion was 30 minutes. A foll

The adult respiratory distress syndrome.

The adult respiratory distress syndrome (ARDS) is an extreme form of noncardiogenic pulmonary edema associated with alveolar-capillary damage. Clinical features include acute respiratory distress, dyspnea and tachypnea, severe hypoxemia refractory to oxygen therapy, and diffuse bilateral pulmonary infiltrates. Any number of serious disorders can cause ARDS, but the processes leading to the alveolar permeability defect are not understood. Therefore, therapy remains nonspecific and supportive. Treatment includes positive end-expiratory pressure, careful fluid management, steroid therapy, and ade

[Severe protracted diarrhea in infancy ("intractable diarrhea")].

In most cases, protracted diarrhea in infants is due to lack of human breast milk, and iatrogenic malnutrition. Damage to the mucosa of the small intestine maintained by cow's milk proteins and malnutrition constitute main parts of a vicious circle interrupted by completely parenteral nutrition for some time, and slow reintroduction of human breast milk. This kind of therapy reduced mortality significantly. The best prophylaxis is the full support of breast feeding. As differential diagnosis the familial lethal diarrhea of the breast fed infant has to be considered. Furthermore, in case of dia

Esophageal atresia and tracheoesophageal fistula: preoperative assessment and reduced mortality.

One hundred twenty-nine infants with esophageal atresia and tracheoesophageal fistula were reviewed from 1955 to 1978. The overall mortality was 38%. Factors associated with the increased mortality include prematurity, pneumonia, and congenital defects other than this anomaly. A classification based on these factors in introduced, which provides the clinician with a prognostic survival rate greater than 90% with only physical examination, chest and abdominal roentgenography, and intravenous pyelography. Postoperative mortality was reduced to 11% in the last five years of the study; this is att

Frailty across Hip arthroplasty pathways: vulnerability domains, outcomes, and implications for care-a narrative review.

BACKGROUND: Frailty helps explain why older patients with similar chronological age and comorbidity burdens experience markedly different outcomes after hip arthroplasty. Interpretation is complicated because instruments capture different vulnerabilities and elective primary, revision, and fracture-related arthroplasty impose distinct stresses and intervention windows. REVIEW APPROACH: PubMed and Web of Science Core Collection were searched from inception through 27 July 2026, followed by a targeted PubMed search on 29 July 2026.
