Whipple's disease is a multisystem bacterial disease usually characterized by malabsorption, diarrhea and polyarthritis. A 45-year-old Sudanese man from western Sudan presented with oronasal leishmaniasis of three years duration. He had no history of previous kala-azar or cutaneous leishmaniasis. OBJECTIVE: To determine the frequency of detection of human herpesvirus-8 (HHV-8) in HIV-related oral ulcers.
DESIGN: Analysis of archived biopsy material.
METHODS: Nested polymerase chain reaction of DNA extracts.
RESULTS: HHV-8 DNA was detected in six of 10 oral ulcers of HIV-positive patients without oral Kaposi's sarcoma (KS) lesions and five of 11 oral KS lesions. The positive non-KS samples were derived from various oral sites.
CONCLUSIONS: In HIV-positive people, HHV-8 can infect oral tissues that are not affected by KS.

In order to improve the diagnosis of Listeria monocytogenes infection, we have developed a polymerase chain reaction (PCR)-based assay combined with microplate capture hybridization technique. The system is based on selective amplification of L. monocytogenes with two specific primers based on the iap gene. Human brucellosis is an imported zoonosis extremely uncommon in Japan. Brucellosis was found in a 38-year-old surgeon who had never been abroad; he developed intrapulmonary infiltrates and pleural thickening indistinguishable from pulmonary tuberculosis. We report a 68 years old male with a polymyositis associated to HTLV-I. Diagnosis was based on clinical picture, an increased creatin-phosphokinase levels, electromyography and muscle biopsy. Faecal samples collected from 114 fully vaccinated pre-school children and 32 unvaccinated infants in Ibadan, Nigeria, were assayed for poliovirus in Hep-2 and RD cell cultures. 8 strains of poliovirus type 1 were isolated from 146 samples--3 from 32 unvaccinated children aged less than 40 d and 5 from 114 fully vaccinated children aged between 9 and 60 months. Studies using Sabin and wild monoclonal antibodies and the polymerase chain reaction confirmed 7 of the 8 isolates to be of the wild type, a possible source of infection among vaccinated children.

We describe a 47-year-old patient with intracerebral tuberculoma of the falx cerebri and fronto-parietal cerebral cortex. The patient presented after a single episode of epileptic seizures. Computed tomography and MRI scans were suggestive of meningioma. At surgery a granulomatous tumor was removed that adhered to the cerebral cortex. A study of beta-thalassaemia alleles in 29 beta-thalassaemia homozygotes and HbS/beta-thalassaemia patients from western Saudi Arabia using the allele-specific priming technique of the polymerase chain reaction revealed a total of five mutations. The spectrum of mutations found in the western region is significantly different to that previously reported in the eastern region, and brings the total number of mutations observed in Saudi Arabian nationals to ten. A comparison of the types and frequencies of the Saudi Arabian mutations with those found in neighboring Arab countries is made.

We describe a 73-year-old forest owner with widespread erythema, myalgia, and proximal muscle weakness. The clinical signs and the results of electromyography, magnetic resonance imaging, and a muscle biopsy were consistent with dermatomyositis. However, serology was positive for Borrelia burgdorferi. Viral particles of human papillomavirus (HPV) types 11 and 16 were isolated from flat condylomas and biopsies of dysplastic lesions and used to infect human keratinocytes. Two cases of benign neurogenic amyotrophy associated with Klinefelter's syndrome are reported. Both presented with slowly progressive, diffuse neurogenic muscle atrophy of juvenile onset. The clinical and histological findings in a 54-year-old patient with enteropathy-associated T-cell lymphoma (EATL) occurring 18 years after renal transplantation are presented. Ten years after adult-onset coeliac disease the patient developed medium to large T-cell non-Hodgkin's lymphoma of the small intestine. Epstein-Barr virus (EBV) genome was detected by polymerase chain reaction in the lymphoma tissue and localized via Epstein-Barr virus RNAs in situ hybridization to some of the tumour cells. This is the first case report of EBV-positive EATL occurring in the setting of immunosuppression.

A giant condyloma acuminatum developed on the penis of a one and a half-year-old Japanese boy in two months. The histological features of this tumor were compatible with those of ordinary condyloma acuminatum. Although we detected the presence of human papilloma virus (HPV) type 16 by using the polymerase chain reaction system, we could not rule out the possibility that this HPV was present concurrently with other HPV types that cause condyloma acuminatum. The lesion was successfully treated with cryotherapy and topical application of 5% fluorouracil ointment under occlusion.

Polyadenylated RNA, extracted from rat hypothalami, cross-hybridized with a RNA probe complementary in sequence to rat PTH (rPTH) messenger RNA (mRNA). Amplification of complementary DNA (cDNA) by the polymerase chain reaction also demonstrated the presence of rPTH mRNA in the rat hypothalamus and parathyroid gland. rPTH mRNA was localized by in situ hybridization in the paraventricular and supraoptic nuclei of the rat hypothalamus. These results demonstrate the expression of the PTH gene in the central nervous system of the rat in areas which suggest roles for PTH in neuroendocrine function.
