Use of Rapid Ascertainment Process for Institutional Deaths (RAPID) to identify pregnancy-related deaths in tertiary-care obstetric hospitals in three departments in Haiti.

BACKGROUND: Accurate assessment of maternal deaths is difficult in countries lacking standardized data sources for their review. As a first step to investigate suspected maternal deaths, WHO suggests surveillance of "pregnancy-related deaths", defined as deaths of women while pregnant or within 42 days of termination of pregnancy, irrespective of cause. Rapid Ascertainment Process for Institutional Deaths (RAPID), a surveillance tool, retrospectively identifies pregnancy-related deaths occurring in health facilities that may be missed by routine surveillance to assess gaps in reporting these d

Assessing School Nurses' Mental Health Competence Training Needs.

BackgroundSchool nurses play a critical role in addressing K-12 students' mental and behavioral health (MH) needs, yet many report limited preparation to manage these concerns. Existing MH training often excludes school nurses or focuses narrowly on literacy, leaving gaps in communication, referral processes, and evidence-based interventions. This study evaluated school nurses' experiences and preferences for ongoing professional development.MethodsA descriptive survey was conducted with 15 school nurses from diverse educational settings assessed priority training areas and preferred formats.

Commercially Available Artificial Intelligence Solutions for Gynaecologic Cytology Screening and Their Integration Into Clinical Workflow.

Historically, gynaecologic cytology, particularly cervical screening through Pap smear tests, has been instrumental in early cancer detection, but not without its challenges. These include variability in interpretation and the labour-intensive nature of manual screening processes. The advent of artificial intelligence (AI) technologies, especially machine learning and deep learning, heralds a new era in cytology, offering enhanced accuracy, consistency, and efficiency. These advancements promise to mitigate traditional limitations by automating routine analyses, aiding early cancer detection,

Risk Analysis by Failure Modes, Effects and Criticality Analysis and Biosafety Management During Collective Air Medical Evacuation of Critically Ill Coronavirus Disease 2019 Patients.

In March 2020, coronavirus disease 2019 (COVID-19) caused an overwhelming pandemic. To relieve overloaded intensive care units in the most affected regions, the French Ministry of Defence triggered collective air medical evacuations (medevacs) on board an Airbus A330 Multi Role Tanker Transport of the French Air Force. Such a collective air medevac is a big challenge regarding biosafety; until now, only evacuations of a single symptomatic patient with an emergent communicable disease, such as Ebola virus disease, have been conducted. However, the COVID-19 pandemic required collective medevacs

Rethinking mechanisms, diagnosis and management of endometriosis.

Endometriosis is a chronic inflammatory disease defined as the presence of endometrial tissue outside the uterus, which causes pelvic pain and infertility. This disease should be viewed as a public health problem with a major effect on the quality of life of women as well as being a substantial economic burden. In light of the considerable progress with diagnostic imaging (for example, transvaginal ultrasound and MRI), exploratory laparoscopy should no longer be used to diagnose endometriotic lesions. Instead, diagnosis of endometriosis should be based on a structured process involving the com

Rapid Detection of Bacterial Susceptibility or Resistance to Quinolones.

The emergence of multidrug resistant microorganisms together with the decline in discovery and development of new antibiotics is of great concern in health-care policy. In this alarming context, an early and well-tailored antibiotic therapy is a relevant strategy not only to improve clinical outcome but also to avoid appearance and spreading of perilous resistant strains. One of the most common antibiotic classes is fluoroquinolones. They trap the DNA girase and/or topoisomerase IV on the DNA, resulting in DNA fragmentation. We have developed the Micromax® assay to determine, in situ, the inte

Quality assurance and quantitative imaging biomarkers in low-dose CT lung cancer screening.

After years of assessment through controlled clinical trials, low-dose CT screening for lung cancer is becoming part of clinical practice. As with any cancer screening test, those undergoing lung cancer screening are not being evaluated for concerning signs or symptoms, but are generally in good health and proactively trying to prevent premature death. Given the resultant obligation to achieve the screening aim of early diagnosis while also minimizing the potential for morbidity from workup of indeterminate but ultimately benign screening abnormalities, careful implementation of screening with

Elucidating the biophysical processes responsible for the chromatic attributes of peripheral cyanosis.

The purple or blue coloration of hands and feet, known as peripheral cyanosis, can represent one of the initial signs of potentially life threatening medical conditions. Consequently, procedures aimed at its early detection and interpretation can help health-care professionals to select the appropriate treatment for these conditions. The effectiveness of such procedures, in turn, depends on the correct assessment of the biophysical processes responsible for eliciting this abnormal skin appearance. However, despite the diverse body of existing clinical research involving cyanosis, the interplay

Do trajectories of at-home dementia caregiving account for burden after nursing home placement? A growth curve analysis.

Transitioning to the nursing home setting is a complex process for family caregivers of older adults with dementia. While nursing home placement (NHP) can alleviate certain caregiving responsibilities, new stressors can also emerge. In the present study, the researchers examined how care-related factors can change leading up to NHP and how these factors influence caregiver outcomes following NHP. A sample of 634 family dementia caregivers (n = 634) were surveyed at three six-month intervals prior to NHP and once during the 12 month period following institutionalization. Growth curve modeling r

Using American Sign Language-Fluent Community Health Navigators to Advance Cancer Screening Adherence through Videoconferencing With Deaf, Deafblind, and Hard of Hearing Adults: Protocol for a Randomized Controlled Trial.

BACKGROUND: Cancer screening nonadherence persists among adults who are deaf, deafblind, and hard of hearing (DDBHH). These barriers span individual, clinician, and health care system levels, contributing to difficulties understanding cancer information, accessing screening services, and following treatment directives. Critical communication barriers include ineffective patient-physician communication, limited access to American Sign Language (ASL) cancer information, misconceptions about medical procedures, insurance navigation difficulties, and intersectional barriers for multiply marginaliz

Group-delivered cognitive behavioural therapy versus waiting list in the treatment of insomnia in primary care: study protocol for a pragmatic, multicentre randomized controlled trial.

BACKGROUND: Insomnia is common in the general population and is a risk factor for ill-health, which highlights the importance of treating insomnia effectively and cost-efficiently. Cognitive-behavioural therapy for insomnia (CBT-I) is recommended as first-line treatment due to its long-term effectiveness and few side-effects, but its availability is limited. The aim of this pragmatic, multicentre randomized controlled trial is to investigate the effectiveness of group-delivered CBT-I in primary care compared to a waiting-list control group. METHODS: A pragmatic multicentre randomized controll

Evaluating Point-of-Care Electroencephalogram for Mild Traumatic Brain Injury Triage and Referral Pathways Within the Emergency Department: A Mixed-Design Cohort Quality Improvement Study.

INTRODUCTION: Traumatic brain injuries, or TBIs, represent a common military health care system challenge due to the high frequency seen throughout the 21st century battlefield, workplace injuries, and other traumatic accidents. There is clear-cut guidance for the identification and treatment of moderate-to-severe TBIs, but most presentations are classified as mild. This quality improvement study evaluated whether incorporating a quantitative electroencephalogram (qEEG) device could improve triage accuracy, refine the referral pathway, and align mild TBI care with Defense Health Agency (DHA) r

A questionnaire-based study to comprehensively assess the status quo of rare disease patients and care-givers in China.

BACKGROUND: There are over 16.8 million rare disease patients in China, representing a large community that should not be neglected. While the public lack the awareness of their existence and difficult status quo, for one reason that they exist as a rare and special group in our society, for another reason that all sectors of the community haven't introduced and propagandized them suitably. However, as a special group with more difficulties in all aspects than normal healthy persons, they need enough care and love from us. To provide a basis for policy-makers to better understand the status qu

Development of a Surgical Data Availability and Transferability Assessment Tool and Its Use in Evaluating Mozambique's Surgical Data Ecosystem.

BACKGROUND: In 2015, the Lancet Commission on Global Surgery promoted the adoption of six perioperative care indicators and increased their visibility in national health policies. However, regular reporting of these surgical indicators remains limited due to challenges with data infrastructure and workforce capacity. This study defines the surgical data ecosystem in a low-income country and develops a tool for harmonizing national data ecosystems with validated global indicators. METHODS: This exploratory, mixed-methods study examined the scope of Mozambique's surgical data ecosystem in 2023.

Automaticity and habit in recognition and responses to clinical deterioration among ward nursing staff: An international cross-sectional study.

BACKGROUND: Rapid response systems aim to reduce preventable harm through early recognition and response to clinical deterioration. While most interventions target knowledge and skills, limited attention has been paid to the role of non-reflective cognitive processes, including habit and automaticity, in shaping nurses' recognition and escalation of care behaviours. OBJECTIVES: To explore automaticity in recognition and escalation of care behaviours and to examine individual and contextual factors associated with behavioural automaticity. DESIGN: International, multicentre cross-sectional su

Assessing the appropriateness of the management of gastro-oesophageal reflux in Australian children: a population-based sample survey.

Gastro-oesophageal reflux (GOR) is a common physiological state in infants and young children, with gastro-oesophageal reflux disease (GORD) its pathological manifestation. Management of GOR/GORD requires elimination of possible underlying causes, parental reassurance, modification of feeding and symptom mitigation, monitoring, and referral to paediatricians if warning signs are present. Published clinical practice guidelines (CPGs) seek to support clinicians and improve management. This study aimed to measure the proportion of Australian GOR/GORD paediatric care that was in line with CPG reco

Decision Making about Localized Esophageal Cancer Treatment: An Observational Study on Variation in Clinicians' Communication Behavior.

UNLABELLED: Background. For localized esophageal cancer, more than 1 curative treatment option is available. As these different options are associated with substantially different treatment outcomes, decision making can be complex. Moreover, treatment decision making for a patient involves multiple health care providers (HCPs) from different disciplines over time, who might have their own role and perspective on the decision-making process. This study aims to describe how HCPs communicate during treatment decision consultations with patients with localized esophageal cancer. Methods. Audio rec

Reporting involvement activities with children and young people in paediatric research: a framework analysis.

BACKGROUND: The active involvement of patients and the public in the design and delivery of health research has been increasingly encouraged, if not enforced. Knowledge of how this is realised in practice, especially where children and young people (CYP) are concerned, is limited, partly due to the low level of reporting of patient and public involvement (PPI) in general. The aim of this work was to assess how researchers funded by the National Institute for Health and Care Research (NIHR) report the involvement of CYP in the design and conduct of child health research to better understand the
