Quantification of late complications after radiation therapy.

BACKGROUND: An increasing number of patients survive cancer after having received radiation therapy. Therefore, the occurrence of late normal tissue complications among long-term survivors is of particular concern. METHODS: Sixty-three patients treated by radical surgery and irradiation for rectal carcinoma were subjected to an unconventional sandwich therapy. Preoperative irradiation was given in four fractions of 5 Gy each applied within 2 or 3 days; postoperative irradiation consisted mostly of 15 x 2 Gy (range, 20-40 Gy). A considerable proportion of these patients developed severe late c

Stereotactic Body Radiation Therapy Augmented Checkpoint Inhibitor Immunotherapy Response in Heavily Pretreated Metastatic Osteosarcoma.

PURPOSE: Immunotherapy for relapsed/refractory (R/R) osteosarcoma has shown limited success because of its immunosuppressive microenvironment. Recent evidence suggests stereotactic body radiation therapy (SBRT) as not only a local therapy but also a modality with an immunomodulatory role. However, combining SBRT with immune checkpoint inhibitors (ICIs) is yet to be explored in osteosarcoma. METHODS AND MATERIALS: We retrospectively reviewed patients with R/R osteosarcoma (n = 62) receiving ICIs with concurrent SBRT radiation therapy (ICI+SBRT, n = 18), conventional radiation therapy (CRT) (IC

Radiation Vasculopathy of the Coronary Arteries: State-of-the Art Review.

Radiation-associated coronary artery disease (RACAD) is a distinct form of coronary artery disease with several specific biological and clinical aspects as well as key knowledge gaps. RACAD is prevalent in patients with cardiac exposure to radiotherapy. As radiotherapy is integral to the effective treatment of many thoracic malignancies, improved long-term survivorship is coupled with increased risk for RACAD. Ionizing radiation induces free radical generation and activation of proinflammatory cascades, leading to endothelial dysfunction and accelerated atherosclerosis. Risks for RACAD are det

Ferulic Acid Protects Human Lens Epithelial Cells Against UVA-Induced Oxidative Damage by Downregulating the DNA Demethylation of the Keap1 Promoter.

Ultraviolet (UV) radiation-triggered production of reactive oxygen species (ROS) is a primary contributor to apoptosis in human lens epithelial cells (HLECs), which can ultimately result in cataract formation. The nuclear factor erythroid-2-related factor 2 (Nrf2)-Kelch ECH associating protein 1 (Keap1) pathway, a fundamental oxidative stress regulation mechanism, plays a crucial role in the development of cataracts. Ferulic acid (FA), recognized for its potent antioxidant properties can activate the Nrf2 signaling pathway to mitigate oxidative damage and cell apoptosis. In this study, we have

Identification of lncRNAs involved in response to ionizing radiation in fibroblasts of long-term survivors of childhood cancer and cancer-free controls.

INTRODUCTION: Long non-coding ribonucleic acids (lncRNAs) are involved in the cellular damage response following exposure to ionizing radiation as applied in radiotherapy. However, the role of lncRNAs in radiation response concerning intrinsic susceptibility to late effects of radiation exposure has not been examined in general or in long-term survivors of childhood cancer with and without potentially radiotherapy-related second primary cancers, in particular. METHODS: Primary skin fibroblasts (n=52 each) of long-term childhood cancer survivors with a first primary cancer only (N1), at least

Derivation of Dose/Volume Constraints for the Anorectum from Clinician- and Patient-Reported Outcomes in the CHHiP Trial of Radiation Therapy Fractionation.

PURPOSE: The CHHiP trial randomized 3216 men with localized prostate cancer (1:1:1) to 3 radiation therapy fractionation schedules: 74 Gy in 37 fractions over 7.4 weeks; 60 Gy in 20 fractions over 4 weeks; and 57 Gy in 19 fractions over 3.8 weeks. Literature-based dose constraints were applied with arithmetic adjustment for the hypofractionated arms. This study aimed to derive anorectal dose constraints using prospectively collected clinician-reported outcomes (CROs) and patient-reported outcomes (PROs) and to assess the added predictive value of spatial dose metrics. Knee and Hip Joint Cartilage Damage from Combined Spaceflight Hazards of Low-Dose Radiation Less than 1 Gy and Prolonged Hindlimb Unloading.

Reduced weight bearing, and to a lesser extent radiation, during spaceflight have been shown as potential hazards to astronaut joint health. These hazards combined effect to the knee and hip joints are not well defined, particularly with low-dose exposure to radiation. In this study, we examined the individual and combined effects of varying low-dose radiation (≤1 Gy) and reduced weight bearing on the cartilage of the knee and hip joints. C57BL/6J mice (n = 80) were either tail suspended via hindlimb unloading (HLU) or remained full-weight bearing (ground). On day 6, each group was divided and

Mapping Radiation Injury and Recovery in Bone Marrow Using 18F-FLT PET/CT and USPIO MRI in a Rat Model.

UNLABELLED: We present and test the use of multimodality imaging as a topological tool to map the amount of the body exposed to ionizing radiation and the location of exposure, which are important indicators of survival and recovery. To achieve our goal, PET/CT imaging with 3'-deoxy-3'-(18)F-fluorothymidine ((18)F-FLT) was used to measure cellular proliferation in bone marrow (BM), whereas MRI using ultra-small superparamagnetic iron oxide (USPIO) particles provided noninvasive information on radiation-induced vascular damage. METHODS: Animals were x-ray-irradiated at a dose of 7.5 Gy with 1

Separate and combined responses to water deficit and UV-B radiation.

Crops and other plants in natural conditions are routinely affected by several stresses acting simultaneously or in sequence. In areas affected by drought, plants may also be exposed to enhanced UV-B radiation (280-315nm). Each of these stress factors differently affects cellular metabolism. A common consequence of plant exposure to the separate action of water deficit and UV-B radiation is the enhanced generation of reactive oxygen species (ROS) causing damage to proteins, lipids, carbohydrates and DNA. Despite this destructive activity, ROS also act as signalling molecules in cellular proces

Feasibility of intraoperative radiation therapy for early breast cancer in Japan: a single-center pilot study and literature review.

BACKGROUND: Intraoperative radiation therapy (IORT) is under evaluation in breast-conserving surgery because the feasibility of the IORT procedure including transportation of the patient under general anesthesia is not well established. Thus, this prospective single-center study aimed to test the feasibility of IORT at a single dose of 21 Gy in Japanese breast cancer patients. METHODS: The primary endpoint was early toxicity; the secondary endpoint was late toxicity. Patients with histologically or cytologically proven primary early breast cancer were eligible. Inclusion criteria were as foll

PRP therapy for chest-wall radiation ulcer: a case report.

BACKGROUND: Radiation ulcers are common complications resulting from radiotherapy. Traditional treatment methods primarily involve thorough wound debridement, continuous vacuum sealing drainage (VSD), and skin flap surgery; however, these approaches often have limited effectiveness and are prone to recurrence. METHODS: We report a case of platelet-rich plasma (PRP) therapy for a chest-wall radiation ulcer. A 50-year-old woman underwent radiation therapy to a chest wall scar, which led to persistent, recurrent skin rupture and exudation. Initial treatments, including antibiotics, antihistamine

The Challenge of Healing Wounds in Radiation-Injured Skin.

SIGNIFICANCE: Wound healing in irradiated skin remains a clinical challenge, with radiation therapy (XRT) resulting in excess collagen deposition and cell death resulting in poor tissue quality. This frequently results in chronic wounds that compromise patient outcomes and quality of life. RECENT ADVANCES: Improved understanding of the pathophysiology behind radiation-induced soft tissue injury has led to the development of promising treatments. These include dermal substitutes, placental derivatives, fibronectin, injectable fat-derived and plasma-derived compounds, hyperbaric oxygen, and def

Early induction and increased risk of precursor B-cell neoplasms after exposure of infant or young-adult mice to ionizing radiation.

Epidemiological studies of atomic-bomb survivors have revealed an increased risk of lymphoid neoplasm (i.e. acute lymphoblastic leukemia) associated with radiation exposure. In particular, children are more susceptible to radiation-induced precursor lymphoid neoplasm than adults. Although ~75% of human lymphoid tumors are B-cell neoplasms, the carcinogenic risk associated with each stage of differentiation of B-cells after radiation exposure is poorly understood. Therefore, we irradiated mice at infancy or in young adulthood to investigate the effect of age at exposure on the risk of developin

Growth Differentiation Factor 11 does not Mitigate the Lethal Effects of Total-Abdominal Irradiation.

Total-body exposure to radiation causes widespread tissue injury. Damage to the hematopoietic and intestinal stem cell compartments is particularly lethal and mitigators of this damage are critical in providing effective treatment. Parabiosis radiation experiments, in which the vasculatures of two rodents are anastomosed prior to irradiation of one of the animals, have shown that there is a circulating factor that protects mice from radiation-induced intestinal death. Recently reported studies have suggested that growth differentiation factor 11 (GDF11) is responsible for the rejuvenation of s
