Validation of Prediction Rules for Computed Tomography Use in Children With Blunt Abdominal or Blunt Head Trauma: Protocol for a Prospective Multicenter Observational Cohort Study.

BACKGROUND: Traumatic brain injuries (TBIs) and intra-abdominal injuries (IAIs) are 2 leading causes of traumatic death and disability in children. To avoid missed or delayed diagnoses leading to increased morbidity, computed tomography (CT) is used liberally. However, the overuse of CT leads to inefficient care and radiation-induced malignancies. Therefore, to maximize precision and minimize the overuse of CT, the Pediatric Emergency Care Applied Research Network (PECARN) previously derived clinical prediction rules for identifying children at high risk and very low risk for IAIs undergoing a

Successful Treatment of Functional Neurologic Symptom Disorder (Conversion Disorder) With Subdissociative Dose Ketamine in the Emergency Department.

Functional neurologic disorder (FND) is a neuropsychiatric condition that causes psychosomatic symptoms-commonly pain, paralysis, and seizure-like activity. Because presentation can be relatively heterogeneous with symptoms that overlap with other life-threatening or disabling neurologic disorders, FND is often misdiagnosed and incorrectly managed. This results in inappropriate resource utilization and patient care, including but not limited to unnecessary hospital admissions, procedures, imaging resource use, and radiation exposure. Effective treatment of FND often includes a combination of p

Current Concepts in Neurogenic Heterotopic Ossification.

Heterotopic ossification (HO) is abnormal bone deposition, most commonly in the hip or elbow, that can significantly impair patients due to pain, stiffness, and contractures, which prevents them from carrying out activities of daily living. A traumatic brain (TBI) or spinal cord injury may prompt the formation of heterotopic ossification, creating so-called neurogenic heterotopic ossification (NHO). The pathophysiology of this condition is not fully understood, but probably involves a complex interplay between a biological scaffold of demineralized bone, CNS signaling, and local tissue signal

Imaging techniques for the diagnosis of male traumatic urethral strictures.

Urethral injures, pelvic trauma, and certain congential abnormalities in male can often cause urethral strictures and scarring. Adequate and accurate imaging diagnosis of urethral strictures is critial to select a surgical approach. Although urethrography is commonly used in evaluating male urethral injury and strictures, its limitation is that the contrast offers images only at the urethral lumen but not of the periurethral structures. Sonourethrography (SUG) has the ability to define the periurethral tissues and without the radiation exposure, the degree of periurethral fibrosis can be shown

Response of human organs to single (or fractionated equivalent) doses of irradiation.

Characteristics of the kinetics of radiation response of human tissues and organs are exemplified by effects in the testis and the ovary. Also, published dose-incidence curves for specified levels of injury in bone marrow, liver, bladder and lung are characterised in terms of single doses as well as single-dose equivalents calculated from fractionated doses using the alpha/beta equation. It is shown that these curves, analysed using a Poisson model, have slopes characterised by D0-equivalents ranging between 1.25 and 2.5 Gy. These values are higher or within the range of values reported in gen

Antegrade ESIN technique via the Kocher interval reduces radiation exposure and accelerates recovery in pediatric DRDMJ fractures: A comparative study with cadaveric validation.

BACKGROUND: Distal radius diaphyseal-metaphyseal junction (DRDMJ) fractures in children often require surgical intervention due to the unique anatomical characteristics and high failure rate of nonoperative treatment. However, the choice of internal fixation remains a challenge for pediatric orthopedic surgeons. Traditional fixation methods, including plate and screw fixation, crossed Kirschner wires (K-wires), and external fixators, have drawbacks such as extensive local trauma and the risk of physeal injury. This study evaluates the clinical efficacy of antegrade elastic stable intramedullar

Diagnostic ionizing radiation exposure in a population-based sample of children with inflammatory bowel diseases.

OBJECTIVES: The degree of diagnostic radiation exposure in children with inflammatory bowel diseases (IBD) is largely unknown. In this study, we describe this exposure in a population-based sample of children with IBD and determine the characteristics associated with moderate radiation exposure. METHODS: We ascertained radiological study use, demographic characteristics, IBD medication use, and the requirement for hospitalization, emergency department (ED) encounter, or inpatient gastrointestinal surgery among children with IBD within a large insurance claims database. Characteristics associa

A comparison of ultraviolet-curing and self-curing polymers in preventive, restorative and orthodontic dentistry.

Both self-cured and UV-cured resin-base dental materials are used in preventive, restorative, and orthodontic dentistry. Polymerization is initiated in both systems by free radicals. Self-curing materials generate free radicals by means of chemical compounds included in their formulation. UV-curing systems rely upon externally-supplied, long wavelength, ultraviolet radiation to produce free radicals within the material. Therefore, although the major chemical components of both systems are similar in many respects, each system has particular advantages and disadvantages over the other, which mu

Candida pulmonary infection mimicking steroid-refractory radiation pneumonitis during corticosteroid taper: a case report.

BACKGROUND: Radiation pneumonitis (RP) is a common complication after thoracic radiotherapy, and corticosteroids are the mainstay of treatment. However, prolonged or high-dose steroid therapy predisposes patients to opportunistic infections that may closely mimic steroid-refractory RP, especially during the tapering phase. Distinguishing radiation-induced lung injury from superimposed fungal pneumonia in this setting is a major clinical challenge. CASE DISCRIPTION: We report a 55-year-old man with non-small cell lung cancer (NSCLC) who developed RP after surgery and adjuvant thoracic radiothe

A risk-adapted, dose-optimized esophagus-sparing radiation therapy technique for high-risk patients with lung cancer: feasibility, dosimetry, and acute esophageal toxicity.

BACKGROUND: Acute radiation-induced esophagitis (ARIE) remains a clinically significant toxicity during definitive thoracic radiation therapy (TRT), particularly in patients with close esophagus-planning target volume (PTV) proximity. This study evaluated a risk-adapted, dose-optimized esophagus-sparing (ES) strategy designed to reduce ARIE while maintaining adequate target coverage in high-risk patients. METHODS: Eighty patients with lung cancer treated with definitive TRT between May 2023 and January 2025 were enrolled. The injury-effective esophagus (IEE) was defined as the local high-risk

Effects of radiation therapy on the meibomian glands and dry eye in patients with ocular adnexal mucosa-associated lymphoid tissue lymphoma.

BACKGROUND: Radiation therapy (RT) is the treatment of choice in patients with low-grade ocular adenexal mucosa-associated lymphoid tissue lymphoma (OAML) and many of them experience post-RT dry eye with varying severity. The purpose of the present study was to investigate ocular effects of RT on meibomian glands and dry eye by directly visualizing structural changes. Secondly, we focused on the comparison of two groups of patients according to tumor location and radiation technique. METHODS: Sixty-four eyes with OAML of conjunctiva, orbit, lacrimal gland, or lacrimal sac were grouped into co

Endoscopic gold fiducial marker placement into the bladder wall to optimize radiotherapy targeting for bladder-preserving management of muscle-invasive bladder cancer: feasibility and initial outcomes.

BACKGROUND AND PURPOSE: Bladder radiotherapy is a management option for carefully selected patients with muscle-invasive bladder cancer. However, the inability to visualize the tumor site during treatment and normal bladder movement limits targeting accuracy and increases collateral radiation. A means to accurately and reliably target the bladder during radiotherapy is needed. MATERIALS AND METHODS: Eighteen consecutive patients with muscle-invasive bladder cancer (T1-T4) elected bladder-preserving treatment with maximal transurethral resection (TUR), radiation and concurrent chemotherapy. [A matched cohort analysis of three-dimensional conformal radiotherapy versus conventional radiotherapy for primary nasopharyngeal carcinoma].

BACKGROUND & OBJECTIVE: Comparing with two-dimensional conventional radiotherapy (2D CRT), three-dimensional conformal radiotherapy (3D CRT) features a precise tumor target volume positioning and a three-dimensional display of the tumor and surrounding normal tissues, therefore, helps to spare normal tissues in the radiotherapy for nasopharyngeal carcinoma (NPC). This study was to explore whether 3D CRT can increase the locoregional control rate while reduce the occurrence and severity of radiation-related sequelae for primary NPC patients. METHODS: Clinical data of 95 primary NPC patients, t

Hypothalamic-pituitary dysfunction after radiation for brain tumors.

BACKGROUND: Patients with brain tumors who are treated with radiation frequently have growth hormone deficiency, but other neuroendocrine abnormalities are presumed to be uncommon. METHODS: We studied endocrine function in 32 patients (age, 6 to 65 years) 2 to 13 years after they had received cranial radiotherapy for brain tumors. The doses of radiation to the hypothalamic-pituitary region ranged from 3960 to 7020 rad (39.6 to 70.2 Gy). Nine patients also received 1800 to 3960 rad (18.0 to 39.6 Gy) to the craniospinal axis. Serum concentrations of thyroid, gonadal, and pituitary hormones were

Ultraviolet radiation-induced corneal tumours in the South American opossum, Monodelphis domestica.

Chronic exposure of the South American opossum, Monodelphis domestica, to ultraviolet radiation (UVR) induced 154 primary tumours of the cornea in 152 eyes. Tumours developed gradually; frank neoplasia was preceded by non-neoplastic proliferation of corneal stromal fibroblasts (keratocytes) and extensive neovascularization. Histologically, the majority of tumours (134 of 154) appeared to be fibrosarcomas arising from keratocytes, but about 12 per cent of the tumours (18 of 154) had a highly vascular appearance, suggesting haemangiosarcoma. In two eyes, squamous cell carcinomas overlay mesenchy

Immune and inflammatory radiogenomics of radiotherapy-induced normal-tissue toxicity: genetic susceptibility, mechanisms, risk prediction, and clinical translation.

Radiotherapy is indispensable for cancer control, yet acute and late normal-tissue toxicities remain major limits to treatment intensity and survivorship. Considerable interindividual variation persists after clinical and dosimetric factors are considered, motivating radiogenomic studies of inherited susceptibility. This review compares common and rare germline variation, association and sequencing designs, polygenic risk scores, and functional evidence through an immunological framework. Radiation-induced DNA damage and organelle stress release danger-associated signals and cytosolic DNA that
