A randomized controlled trial to prevent patient lift and transfer injuries of health care workers.

STUDY DESIGN: Randomized controlled trial (RCT). OBJECTIVES: To compare the effectiveness of training and equipment to reduce musculoskeletal injuries, increase comfort, and reduce physical demands on staff performing patient lifts and transfers at a large acute care hospital. SUMMARY OF BACKGROUND DATA: Back injury to nursing staff during patient handling tasks is a major issue in health care. The value of mechanical assistive devices in reducing injuries to these workers is unclear. METHODS: This three-armed RCT consisted of a "control arm," a "safe lifting" arm, and a "no strenuous lifti

Results and return to sports at 5 years after Latarjet for anterior instability with or without pre-operative glenoid bone loss.

INTRODUCTION: The Latarjet procedure provides reliable functional outcomes and a high rate of return to sport in the treatment of anterior shoulder instability. While traditionally indicated in cases of significant glenoid bone loss (GBL) or engaging Hill-Sachs lesions, its use has progressively expanded to a broader range of clinical situations. However, few studies have specifically evaluated outcomes of the Latarjet procedure according to the degree of preoperative GBL. HYPOTHESIS: We hypothesized that preoperative glenoid bone loss does not significantly influence clinical outcomes or ret

Sutures, Screws, Buttons, and Anchors: A Review of Current Bone Graft Fixation Devices for Glenoid Bone Loss in the Unstable Shoulder.

PURPOSE OF REVIEW: Anterior shoulder instability is associated with concomitant injury to several stabilizing structures of the shoulder, including glenoid bone loss. While instability is most common in young athletes and patients with predisposing conditions of hyperlaxity, recurrent shoulder instability can occur throughout various age ranges and may lead to longer term effects including pain and shoulder arthritis. Glenoid bone loss exceeding certain thresholds is generally treated by glenoid reconstruction via bone block augmentation to adequately stabilize the glenohumeral joint. These pr

Conserve or reverse? Outcomes of conservative treatment vs. reverse shoulder arthroplasty in displaced 3- and 4-part proximal humeral fractures in patients over 60 years.

BACKGROUND: The optimal treatment of 3- and 4-part proximal humeral fractures in older adults remains controversial. The aim of this study was to compare patient reported outcomes following reverse shoulder arthroplasty (RSA) or nonoperative management in patients over 60 years old. METHODS: A retrospective review was undertaken of patients following 3- or 4-part proximal humeral fractures treated with RSA or nonoperative treatment with minimum 2-year follow-up. Patient-reported outcome measures were collected including Oxford Shoulder Score (OSS), Quick-Disabilities of the Arm Shoulder and H

Sling and forget it? A systematic review of operative versus nonoperative outcomes for scapula fractures.

BACKGROUND: The majority of scapula fractures have historically been treated nonoperatively. The current literature describing patient outcomes following scapula fractures is limited. Our objective was to determine differences in outcomes between operatively and nonoperatively treated scapular fractures. The goal of our study was to provide an updated and comprehensive systematic review for scapula body, neck, and glenoid fractures focusing on several outcomes including union rate, return to work, pain, shoulder active range of motion, strength, functional scores, and any recorded complication

Fibro-adhesive Bursitis: A Novel Sonographic Finding in Adhesive Capsulitis Patients and a Proposal of Management.

INTRODUCTION: Adhesive capsulitis, also known as "frozen shoulder," is a debilitating shoulder condition increasingly linked to fibroadhesive bursitis, particularly after COVID-19 and related vaccinations. There is no definitive gold standard for its treatment, the primary therapeutic objectives of which are the reduction of pain and the restoration of shoulder range of motion. The aim of our study was to analyze treatment outcomes based on quantitative measures of shoulder function and symptom relief. METHOD: Conducted between January 2022 and April 2023, the research involved 45 patients in

Adhesive capsulitis after COVID-19 vaccine injection: a peculiar case treated with combined bursa distention and glenohumeral capsular hydrodilatation.

Frozen shoulder is a common and self-limiting condition affecting the soft tissues of the shoulders, characterized by severe pain, impaired range of motion (ROM) and limitation of daily activities. Its prevalence is 5% and it occurs most commonly in the fifth and sixth decades of life; women are more affected [DePalma in Clin Orthop Relat Res 466:552-560, 2008]. It can be idiopathic or associated with other conditions such as metabolic disorders, diabetes, thyroid diseases, prolonged immobilization, trauma [DePalma in Clin Orthop Relat Res 466:552-560, 2008], or complications after vaccine adm

Is coracoclavicular reconstruction necessary in hook plate fixation for acute unstable acromioclavicular dislocation?

BACKGROUND: Acromioclavicular joint (ACJ) dislocation is a relatively common shoulder injury. For the treatment of cases of severe ACJ dislocation (Rockwood type III-V), hook plate fixation is an easy-to-master and minimally-invasive approach to surgical intervention. Over stress on the acromion following hook plate fixation often leads to acromial complications such as osteolysis and loss of reduction. We hypothesized that suspensory reconstruction alongside hook plate fixation might provide a superior stability and reduce complications as compared with hook plate fixation alone. The purpose

Mid-clavicle fractures in adults: end result study after conservative treatment.

OBJECTIVE: The aim of the present study was to analyze the long-term outcome of mid-clavicle fractures in adults and to evaluate the clinical importance of displacement and fracture comminution. DESIGN: Two hundred twenty-five mid-clavicular fractures that had been nonsurgically treated at Malmö University Hospital were retrospectively evaluated, both clinically and radiographically, an average of seventeen years after injury. There were seventy-one undisplaced fractures, sixty-nine displaced two-fragment fractures, and eighty-five displaced and comminuted fractures. The average patient age a

Upper extremity deep vein thrombosis after arthroscopic rotator cuff repair.

Arthroscopic shoulder surgery is generally associated with a low complication rate. While complications such as neurological injury, stiffness and iatrogenic damage are relatively well recognised, the occurrence of deep vein thrombosis (DVT), particularly of the upper extremity deep vein thrombosis (UEDVT), is exceedingly rare. We report two unusual postoperative cases of UEDVT following arthroscopic rotator cuff repair in two elderly sisters. No additional predisposing factors were identified beyond the perioperative setting; thrombophilia screening was negative. These cases underscore the im

A Rare Cause of Deep Vein Thrombosis in a Young Orchestra Conductor.

Upper extremity deep vein thrombosis (DVT) of the axillary/subclavian veins is rare (5-10% of DVT). After clinical suspicion and duplex ultrasound, anticoagulation, surgical decompression and sometimes thrombolysis are mandatory due to complications. We discuss the case of a young healthy orchestra conductor with primary DVT of the left upper extremity and concomitant left shoulder musculo-tendinous traumatic injury. Symptoms of both conditions and subtle signs of upper extremity DVT delayed the diagnosis until full-blown DVT occurred. After successful anticoagulation and surgical TOS (thoraci

Breast Surgery: Management of Postoperative Complications Following Operations for Breast Cancer.

Breast cancer surgery is associated with low rates of surgical morbidity. Postoperative complications related to breast surgery include seroma, infection, hematoma, mastectomy flap necrosis, wound dehiscence, persistent postsurgical pain, Mondor disease, fat necrosis, reduced tactile sensation after mastectomy, and venous thromboembolism. Postoperative complications related to axillary surgery include seroma, infection, lymphedema, nerve injury, and reduced shoulder/arm mobility. The overall rate of complication related to axilla surgery may be confounded by the type of breast surgery performe

Unilateral hypoglossal neurapraxia following endotracheal intubation for total shoulder arthroplasty.

A case is described of postoperative right hypoglossal neurapraxia after general anesthesia and interscalene block with endotracheal intubation for left total shoulder arthroplasty. Postoperative hypoglossal neurapraxia has been reported in cases, yet it remains a rare complication of anesthesia-related interventions. In this case report, postulated causes of hypoglossal neurapraxia are presented. A review of the literature pertaining to anesthesia-related causes of hypoglossal nerve injury is included. Anesthesia providers should be aware of the course of cranial nerve XII as it relates to th

Scapulothoracic stabilisation for winging of the scapula using strips of autogenous fascia lata.

We have used a modified technique in five patients to correct winging of the scapula caused by injury to the brachial plexus or the long thoracic nerve during transaxillary resection of the first rib. The procedure stabilises the scapulothoracic articulation by using strips of autogenous fascia lata wrapped around the 4th, 6th and 7th ribs at least two, and preferably three, times. The mean age of the patients at the time of operation was 38 years (26 to 47) and the mean follow-up six years and four months (three years and three months to 11 years). Satisfactory stability was achieved in all p

Prognosis and early management of birth injuries to the brachial plexus.

Twenty-four cases of birth injury to the brachial plexus were seen in 21 infants over 15 years. Obstetric complications were common, and in 11 cases traction was needed to deliver the shoulders. Three out of every four arms fully recovered after exercises. Splints were not needed. Reconstructive procedures were performed on three permanently paralysed arms when the children were aged about 4. There was no way of predicting which patients would recover. The muscles supplied by the suprascapular nerve were paralysed in all patients, and this paralysis persisted in those whose arms failed to reco

Ranges of Return to Sport Outcomes Following Anterior Shoulder Instability Surgery Are Influenced by Procedure, Athletic Level, and Follow-Up Duration: A Systematic Review.

PURPOSE: To evaluate and qualitatively synthesize studies performed in the past decade that report return to sport (RTS) after anterior shoulder instability surgery and identify study, patient, and surgical characteristics that reduce heterogeneity in RTS outcomes, to help guide surgeons and patients in collaborative decision-making and set appropriate expectations of postoperative athletic performance. METHODS: A systematic review of PubMed, Embase, and Web of Science databases was conducted for all studies published between January 2015 and March 2025 reporting RTS or return to preinjury le

Can imaging determine if a rotator cuff tear is traumatic?

AIM: We reviewed the last decade of literature to update a previous publication on this topic by the senior author. In New Zealand, traumatic causation has implications for entitlement for treatment though the Accident Compensation Corporation (ACC). Acuity and chronicity may also be relevant in determining repairability. METHODS: Literature was reviewed regarding acromial morphology, greater tuberosity (GT) cysts, acromiohumeral interval (AHI), fatty degeneration and atrophy, acromioclavicular (AC) arthrosis, tendinopathy, bursal changes and other features. RESULTS: Some factors can be cons

Traumatic posterior sternoclavicular joint dislocation - Current aspects of management.

The posterior sternoclavicular joint dislocation is a rare and potentially life-threatening injury, as massive haemorrhage can occur at the time of trauma, during reduction manoeuvres and drilling. These injuries are rare and a collective experience of managing them is of paramount importance. We present our multidisciplinary experience of managing several of these injuries in our centre, with learning points we have identified. Assessment should include Computerised Tomography Angiography (CTA) to assess the anatomy of the joint including the proximity to the underlying innominate vein and to

[Scapula dyskinesis in medium size full-thickness supraspinatus muscle tears].

INTRODUCTION: Scapular motion abnormality in rotator cuff tears is a well-known symptom, but its significance is not clear. Some authors consider it as a cause of rotator cuff tear, others as a consequence of the disease. OBJECTIVE: The aim of our study was to assess the changes in scapular motion in medium size full-thickness rotator cuff tear of degenerative origin compared to a healthy control group. MATERIAL AND METHOD: 10 healthy (control group) and 9 subjects with a medium size (1-3 cm), complaining rotator cuff tear (study group) were included in our study, in whom we analyzed the mov

Editorial Commentary: Social Determinants of Health Influence the Success of Shoulder Rotator Cuff Repair Outcomes: When You Do Everything Right, Yet Things Still Go Wrong.

Our ability to perform a technically sound surgery is not sufficient to ensure patients have an excellent clinical outcome. Social determinants of health disparities (SDHDs) profoundly impact health equality. Health disparities that exist in the United States are risk factors for inferior patient-reported outcomes and result in greater complication rates following rotator cuff repair surgery. The presence of SDHDs was associated with an increased risk of revision, stiffness, emergency department visits, medical complications, and costs. Economic and educational SDHDs were associated with the g
