Dysphagia. Pathophysiology, causes, and evaluation.

The two types of dysphagia, oropharyngeal and esophageal, involve different phases of swallowing, are accompanied by different symptom complexes, and have different etiologies. They can usually be distinguished by history, which often will also suggest the specific cause. The initial evaluation of oropharyngeal dysphagia entails a general history and physical examination, careful examination of the pharynx and hypopharynx, and barium esophagography (preferably with videotape recording). The initial evaluation for esophageal dysphagia entails barium esophagography and fiberoptic endoscopy. Esop

Gastroesophageal reflux and severe mental retardation.

Recurrent vomiting is common in children with severe mental retardation and leads to significant morbidity with malnutrition, anemia, and aspiration pneumonitis. Spasms of the abdominal muscles and diaphragm, uncoordinated peristalsis, and central nervous system disorders are causes of dysphagia and continuous gastroesophageal reflux. It is desirable that mentally retarded children with vomiting have a barium swallow and esophagoscopy as early as possible. Fundoplication should be performed before complications develop. Spasms with aspiration followed by apnea, in particular, are life-threaten

Giant posterior inferior cerebellar artery aneurysm with dysphagia.

Presented is a case of giant aneurysm of the posterior inferior cerebellar artery with the unusual clinical presentation of dysphagia to the point of inability to swallow even oral secretions, resulting in severe malnutrition. A computed tomography scan with contrast-medium enhancement appeared normal. The correct diagnosis was suspected from the results of metrizamide myelocisternography and was confirmed at angiography. The patient underwent ligation of the feeding right vertebral artery with subsequent improvement in her dysphagia and resolution of her other neurological deficits. Discussio

Vestibular-induced vomiting after vestibulocerebellar lesions.

Vestibular stimulation, by means of sinusoidal electrical polarization of the labyrinths of decerebrate cats, can produce vomiting and related activity resembling that seen in motion sickness. The symptoms include panting, salivation, swallowing, and retching as well as vomiting. These symptoms can be produced in cats with lesions of the posterior cerebellar vermis. In contrast to a previous proposal by Wang and Chinn in 1956, we suggest that a transcerebellar pathway from the vestibular apparatus through the nodulus and uvula to the 'vomiting center' is not essential for vestibular-induced vo

Prosthetic replacement of bilateral maxillectomy.

A patient had an adenocystic carcinoma that involved both maxillary sinuses, the entire hard palate, and the lower third of both nasal chambers. Treatment consisted of bilateral maxillectomy and a split-thickness skin graft applied to the inside of the cheeks. A dental obturator constructed to fit undercuts in the surgical cavity allowed speech and swallowing. Because the patient objected to the sunken appearance of the upper lip, she obtained the central incisor portion of her late husband's upper denture and bonded this segment to the dental appliance. This achieved satisfactory cosmesis.

Report on the problems in total removal of extensive tumors of the petrosal bone and the skull base in this region. These problems are: 1. Freeing of the tumor involved internal carotid artery from the carotid foramen to the cavernous sinus. 2. Covering the resulting large defects of the dura and covering the new ostium of the Eustachian tube to protect the posterior fossa from infection. 3. Stopping the blood flow during the removal of extensive glomus jugulare tumors. 4. Reconstruction of swallowing and voice in paralysis of the involved vagal nerve. Operative measures for the solution of th

Prosthetic reconstruction following resection of the hard and soft palate.

The restoration of the soft palate presents a challenge completely different from that of the hard palate. The mobility of the soft palate tends to interfere with velar extensions. The reduction in size of the soft palate extension to prevent impingement upon the mobile margins of the defect will lead to insufficient oronasal separation during functional activities. The solution is to construct a specially designed prosthesis to attain the maximum utilization of the remaining structures and their motility. Although each pharyngeal extension is different in shape, they give the patient an effec

[Immediate one-stage total oesophagenal replacement by transmediastinal transposition of the stomach without thoracotomy in a case of hypopharyngeal carcinoma (author's transl)].

A case of hypopharyngeal carcinoma (in a 36-year-old man) is reported in which, after blunt dissection of the oesophagus via a posterior mediastinal tunnel without thoracotomy, the stomach was mobilized and its fundus anastomosed to the pharynx at the level of the vallecula. This form of oesophagectomy was first reported in 1936 by Turner for carcinoma of the thoracic oesophagus. Adopting this procedure in cases of carcinoma of the hypopharynx or cervical oesophagus (as recently reported) has as its main advantage the restoration of normal swallowing within a few days, as also took place in th

[Clinical Management of Dysphagia and Nutritional Disorders in Neurodegenerative Diseases].

Dysphagia is common in patients with neuro degenerative diseases. It is associated with aspiration pneumonia, malnutrition, and reduced quality of life. Swallowing assessment should incorporate therapeutic perspectives, including the use of compensatory strategies. In conditions such as amyotrophic lateral sclerosis, weight loss is associated with a poor prognosis, and nutritional therapy may function as a disease-modifying intervention. Clinical ethical issues may arise, including decisions regarding gastrostomy or care for patients with impaired decision-making capacity. A multidisciplinary

Office Procedures in Dysphagia.

Dysphagia is a common complaint encountered by otolaryngologists in the United States. Usually, patients are referred to an otolaryngologist or other physician specializing in dysphagia (deglutologist) once empiric therapies have been tried and failed. Further investigation often required the utilization of office esophagoscopy, swallowing fluoroscopy, and now high-resolution pharyngoesophageal manometry with impedance. Over the past decades, significant advances have been made in improving care of patients with dysphagia in the clinic. In this article, diagnostic and therapeutic interventions

Tonsillitis.

Acute tonsillitis is a common illness that affects patients of all ages but the risk of complication increases with the young. Tonsillitis commonly presents with a sore throat and difficulty swallowing and will often have swelling of the tonsils with exudate on physical examination. Although acute tonsillitis secondary to group A beta-hemolytic Streptococcus should be considered so that antibiotic therapy can be initiated in a timely fashion to prevent complications such as a retropharyngeal abscess or acute rheumatic fever, the most common etiology of tonsillitis is viral. Evaluation of Rheological Properties of Saliva by Determining the Spinnbarkeit.

Saliva is crucial to maintaining oral health and facilitating chewing, swallowing, and speech functions. Decreased saliva secretion, known as hyposalivation, impairs these functions and increases the risk of dental caries and other infectious diseases in the oral cavity.Saliva exhibits various rheological properties, with mucin being a factor in determining these properties. Alterations in these properties can also affect the sensation of dry mouth. In this article, we focus on the spinnbarkeit of saliva using the Neva Meter instrument and provide a methodology for fully understanding the appr

Diagnostic accuracy of bedside screening tools for aspiration risk in acute stroke, a commentary.

Dysphagia is common after stroke and can lead to serious complications including pneumonia and mortality. Bedside swallow screening tools for aspiration risk associated with dysphagia are available for use by healthcare professionals to quickly assess patients, put in place the necessary interventions and make referral to speech and language therapy. A Cochrane Systematic review aimed to identify the diagnostic accuracy of such tools for use in practice (Boaden et al. 2021). This commentary critically appraises and evaluates the systematic review and expands upon the findings in the context of

Recurrent Nasopharyngeal Carcinoma and the Role of Surgery.

Recurrent nasopharyngeal carcinoma (rNPC) presents unique challenges as reirradiation comes with significant treatment-related morbidity in swallowing, middle ear function, and large-vessel integrity. Advances in endoscopic technology have made surgery for rNPC an increasingly viable option for select patients and may play a role in providing a better quality of life to patients with this challenging disease. In carefully selected patients, endoscopic and open surgical approaches may provide comparable disease control while mitigating long-term treatment-related morbidity.

This is a classic case report of a rare giant fibrovascular polyp (GFP) of the oesophagus in a 50-year-old male patient, who two years prior had regurgitated the polyp and swallowed it again. He only sought medical professionals after losing 25 kg and having severe dysphagia. The GFP, weight 520 g and size 21 × 9 × 7 cm, was removed in toto with a cervical excision following intraluminal marking of the base of the polyp during oesophagoscopy. On histological examination the GFP was found to represent a well-differentiated liposarcoma.

Bilateral Tongue Necrosis in a Young Patient as a Complication of Prolonged Oral Endotracheal Intubation: A Case Report.

We present a unique case of complete tongue necrosis caused by a compression of an endotracheal tube (ETT). A 39-year-old female underwent endotracheal intubation secondary to respiratory failure following sudden altered mental status. Tongue swallowing developed and worsened with obvious pallor on examination. Extensive ischemic changes with tongue necrosis developed dramatically due to the compression during her prolonged intubation. This case of tongue necrosis highlights the importance of proper ETT sizing and positioning during prolonged intubation in ICU patients.

Acquired haemophilia A (AHA) is a rare autoimmune disorder resulting from antibodies against coagulation factor VIII. AHA causes severe, unexpected bleeding which may be life-threatening and should be considered when a patient with no previous history of bleeding presents with spontaneous bleeding and a prolonged APTT. This case report describes onset of AHA of a 75-year-old male who was admitted to Vejle Hospital with acute need for intubation due to breathing and swallowing problems caused by supraglottic haematoma. The case was complicated by anticoagulant treatment with rivaroxaban.

Use of Immediate Obturator Prosthesis as a Rehabilitation Option in Cases of Squamous Cell Carcinoma.

Maxillary defects caused by surgical resection generate abnormal communication between the nasal and oral cavities, hindering proper eating and phonetics, and causing aesthetic damage. Prosthetic intervention is necessary to rehabilitate these patients, in order to block communication and allow correct speech and swallowing functions. This article reports the manufacture of an immediate palatal obturator prosthesis after hemimaxillectomy due to the surgical removal of an intraoral squamous cell carcinoma, which guaranteed better conditions for postoperative recovery, as well as better adaptati
