[Related factor analysis of brachial-ankle pulse wave velocity in patients with obstructive sleep apnea hypopnea syndrome].

OBJECTIVE: To explore the related factors for the measurement of arterial stiffness by brachial-ankle pulse wave velocity (baPWV) in patients with obstructive sleep apnea hypopnea syndrome (OSAHS). METHODS: The blood pressure and baPWV in 76 patients with OSAHS confirmed by polysomnography (PSG) were measured. Clinical data were collected, and the carotid and extremity arteries were examined by ultrasound. Arterial intima thickening, atherosclerotic plaque formation and calcification, and the morphological changes of the arterial wall were evaluated. The related factors for baPWV in patients …

Excessive reactive lymphoid hyperplasia in a child with persistent obstructive sleep apnea despite previous tonsillectomy and adenoidectomy.

Both lingual tonsil and adenoid are the lymphoid tissue members of Waldeyer ring. Enlargement of the lingual tonsil and adenoid occurs in children with persistent obstructive sleep apnea despite previous tonsillectomy and adenoidectomy relatively frequently. In this article, we present a case with upper airway obstruction and infection findings because of excessive lingual tonsil and adenoid tissue hyperplasia and, additionally, bilateral multiple cervical lymphadenopathies. The patient had had tonsillectomy and adenoidectomy 8 years ago. Etiopathogenetic mechanisms, clinical, radiologic, histo…

Upper airway changes in severe obstructive sleep apnea: upper airway length and volumetric analyses using 3D MDCT.

CONCLUSION: Three-dimensional multi-detector computed tomography (3D MDCT) analysis of the upper airway suggested that the lengthening of the upper airway in the absence of volumetric change may independently contribute to the severity of obstructive sleep apnea syndrome (OSAS) in adults. OBJECTIVES: We sought to investigate the relationships among the length and volume of the upper airway to the severity of OSAS. METHODS: A total of 73 subjects underwent 3D MDCT scanning and standard polysomnography. We measured the upper airway length (UAL), which was defined as the vertical distance from …

High prevalence of obstructive sleep apnea in patients with resistant paroxysmal atrial fibrillation after pulmonary vein isolation.

OBJECTIVES: To address the question whether obstructive sleep apnea (OSA) is associated with the recurrence of paroxysmal atrial fibrillation (AF) in patients treated with ≥2 pulmonary vein isolation procedures. PATIENTS AND METHODS: In this study, we included adults with therapy-resistant symptomatic paroxysmal AF, defined as AF recurring after ≥2 PV-isolation procedures (n = 23). For comparison, we selected another cohort of patients being successfully treated by one PV isolation without AF recurrence within 6 months (n = 23). PV isolation was performed by radiofrequency with an open irriga…

Visual analogical well-being scale for sleep apnea patients: validity and responsiveness : a test for clinical practice.

INTRODUCTION: Health-related quality-of-life (HRQL) tests used in sleep apnea-hypopnea syndrome (SAHS) are time-consuming, complicating their application in clinical practice. The objective was to examine the validity and responsiveness of a simple visual analogical well-being scale (VAWS) for the clinical use. METHOD: The subjects proceed from a cohort of SAHS patients treated with CPAP for 12 weeks. We correlated the VAWS with other HRQL tests, related clinical and polysomnographic measures to concurrent and construct validities. Responsiveness by: (1) comparison of HRQL tests between befor…

[Comparative analysis of polysomnography and micro-sensitive mattress-sleep monitor used for obstructive sleep apnea hypopnea syndrome].

OBJECTIVE: To assess the efficiency of mattress-type micro sensitive monitor (MMSM) in sleep monitoring. METHODS: Forty patients diagnosed as obstructive sleep apnea-hypopnea syndrome (OSAHS) by polysomnography (PSG) were randomly chosen, then received MMSM examination within 2 - 5 days. The results of both methods were compared, using the respiratory disturbance index, sleep efficiency, and obstructive factors as indicators. RESULTS: All 40 patients were diagnosed as OSAHS, which were consistent with the results of PSG. There was no statistical difference when comparing the apnea hypopnea i…

Concomitant existence and interaction of cardiovascular abnormalities in obstructive sleep apnea subjects with normal clinic blood pressure.

This study was conducted to examine the existence of increased arterial stiffness of the central to middle-sized arteries and left ventricular (LV) structural/functional abnormalities in subjects with obstructive sleep apnea (OSA) who had normal clinic blood pressure. Brachial-ankle pulse wave velocity (baPWV) measurement and echocardiography were conducted in 164 consecutive subjects with normal clinic blood pressure evaluated for sleep disorders. Multivariate linear regression analysis showed that the apnea-hypopnea index (AHI) was an independently associated variable, even after adjustments…

[Late infantile GM1 gangliosidosis with progressive dilatation of common bile duct and obstructive apnea--an autopsy case].

We report a 5-year-old autopsy case of late infantile type of GM1 gangliosidosis, which developed rare respiratory and intestinal complications. Obstructive apnea by adenoidal hypertrophy was improved by adenoidectomy, but the right bronchus was compressed by hilar lymph node swellings. The lymph nodes could not be treated surgically because of her poor general condition. There was hyperplasia showing foamy histiocyte including mucopolysaccharide which demonstrated a vacuolar formation containing irregular arranged fibrillar material on electron microscopy. Cerebral atrophy progressed gradually…
