Pattern of Renal Diseases among Children in The Niger Delta Region, Nigeria.

INTRODUCTION: Childhood renal diseases are increasing in Nigeria. It is necessary to monitor the nature of these diseases in each locality for effective health care planning. Our aim was to document the nature of renal diseases in the Niger Delta Region of Nigeria and compare it with other regions in the country. METHODS: We reviewed the documents of all children presenting with renal diseases between January 2010 and December 2012 to the Delta State University Teaching Hospital in Oghara and GN Children's Clinic in Warri, the two major pediatric nephrology service providers in the Niger Delt

Prevalence and Risk Factors of Central Venous Stenosis among Prevalent Hemodialysis Patients, a Single Center Experience.

INTRODUCTION: Central vein stenosis (CVS) is a common complication of central venous catheter (CVC) insertion. In this study we evaluated the prevalence and risk factors of CVS among hemodialysis (HD) patients in a single center in Sudan, using Doppler ultrasound as a screening tool. METHODS: The study included 106 prevalent HD patients. For every patient, we performed Duplex Doppler for the right and left jugular, subclavian and femoral veins. A patient was considered to have hemodynamically significant stenosis if the pre-stenosis to the post-stenosis velocities ratio was ≥ 2.5 or they had

Cardiovascular manifestations of allograft dysfunction in renal transplant recipients: a review.

INTRODUCTION: Cardiovascular complications are the leading cause of death among renal transplant recipients; renal graft dysfunction has special effects on cardiovascular morbidity and mortality. REVIEW: Several studies have demonstrated a significant correlation between creatinine level and major coronary events, congestive heart failure and cerebrovascular events. Cardiovascular mortality has been related in different reports to serum creatinine levels and duration on renal replacement therapy. A low estimated glomerular filtration rate (eGFR) was also found to be a significant predictor of

Pelvi-Ureteric Junction Obstruction at Red Cross Children's Hospital, Cape Town: a Six Year Review.

INTRODUCTION: Congenital abnormalities of the kidney and urinary tract (CAKUT) are significant causes of end stage renal disease (ESRD) in children. Some of these abnormalities, when identified early, are amenable to treatment. In developing countries, very few published reports exist concerning the pattern and scope of CAKUT in childhood. METHODS: This is a retrospective review of all patients with confirmed Pelvi-Ureteric Junction (PUJ) obstruction who were diagnosed antenatally with hydronephrosis between Jan 2002 and Dec 2007 at the Red Cross Children's Hospital, Cape Town. The clinical c

Prognosis of acute post-streptococcal glomerulonephritis in Sudanese children.

INTRODUCTION: Acute post streptococcal glomerulo-nephritis (APSGN) is a form of acute nephritic syndrome characterized by edema, hematuria, proteinuria, and hypertension. The immediate prognosis of acute post-streptococcal glomerulonephritis in children is usually excellent, however, the long-term prognosis has been a subject of debate. METHODS: This is a retrospective and prospective cohort study of Sudanese children with APSGN followed in a tertiary care hospital between 2006 and 2010. Patients who presented for follow-up 1-5 years after initial diagnosis were assessed for proteinuria, hema

Management of Anemia and other Hematologic Derangements in Patients with Chronic Kidney Disease.

INTRODUCTION: Chronic Kidney Disease (CKD) has adverse consequences on almost all body systems. The kidney does not function merely as an excretory organ, but participates in normal erythropoiesis, normal bone mineral deposition and blood pressure regulation. REVIEW: Anemia is prevalent in CKD with known deleterious effects on the car diovascular system. It is mostly due to erythropoietin deficiency, inhibition of erythropoiesis by uremic solutes, and reduction in red blood cell life span. Other possible causes include iron, B12 or folic acid deficiency or blood loss. Dysfunction of the endog

Treatment of hepatitis B virus reactivation in a cadaveric renal transplant recipient with entecavir.

INTRODUCTION: Risk of reactivation of Hepatitis B virus (HBV) infection and other liver related complications continues to be a major cause of concern in HBV carriers undergoing cadaveric renal transplantation. Antiviral medications have been recommended post renal transplantation in patients who are chronic HBV carriers. CASE REPORT: Here we present the case of a 15-year-old girl known to have kidney failure and chronic active HBV infection indicated by positivity of both HBsAg and HBeAg. She was treated with interferon alpha for six months resulting in clearance of HBeAg and reduction of HB

Dense deposit disease: a 29-years electron microscopy experience.

INTRODUCTION: Dense Deposit Disease (DDD) is a devastating renal disease that leads to renal failure within 10 years of diagnosis in about half of affected patients. In this study, we evaluated the relative prevalence and pathological features of DDD diagnosed at our center over a 29 years period. METHODS: We reviewed the clinical and pathological features of all cases of DDD diagnosed at the Electron Microscopy Unit of Ain Shams University (ASUSH) between January 1983 and December 2011. RESULTS: From a total of 3283 renal biopsies, 33 (1%) were diagnosed with DDD (10 children and 23 adults)

Idiopathic membranous nephropathy preceding the onset of rheumatoid arthritis: a case report.

INTRODUCTION: Membranous nephropathy (MN) in the context of rheumatoid arthritis (RA), is often an iatrogenic complication due to the nephrotoxic effects of antirheumatic drugs. Rare cases of non-iatrogenic association between these two diseases were reported in the literature. CASE REPORT: A 30-year-old female patient presented in September 2005 with nephrotic syndrome. Renal biopsy showed features consistent with MN. Search for etiology was negative, particularly lupus serology which remained negative throughout the course of her illness. Accordingly, she was diagnosed as a case of idiopath

Nephrogenic diabetes insipidus with intracranial calcification in a child with thalassemia minor.

INTRODUCTION: There are numerous causes for intracranial calcification in children. We describe an unusual cause of intracranial calcification in a child, namely, nephrogenic diabetes insipidus (NDI). CASE REPORT: A 12-year-old boy presented with seizures and developmental delay. MRI of the brain revealed intracranial calcification. Evaluation showed findings suggestive of NDI. The lack of evidence of any other metabolic defect suggests that these calcifications were secondary to NDI. He also had anemia for which he was investigated and diagnosed as thalassemia minor. Detailed literature revi

Acute tubulo-interstitial nephritis with positive anti-neutrophil cytoplasmic antibodies.

INTRODUCTION: Anti-neutrophil cytoplasmic antibody (ANCA) associated vasculitis involving the kidney usually comprises pauci-immune, necrotizing glomerulonephritis with crescents. Mononuclear tubulo-interstitial infiltrates are common in ANCA associated vasculitis, but these have usually been described in conjunction with glomerulitis. Acute tubulo-interstitial nephritis (ATIN) is a common cause of acute kidney injury that is most frequently induced by drugs or infections. Idiopathic ATIN has rarely been reported in association with the presence of a positive ANCA. These two entities seem to s

ANCA negative pulmonary renal syndrome with pathologic findings of thrombotic microangiopathy.

INTRODUCTION: Thrombotic microangiopathy (TMA) is characterized by aggregation of platelets in the renal and/or systemic circulation, thrombocytopenia and intravascular hemolysis. The syndrome classically spares the lung. The term pulmonary-renal syndrome describes a number of diseases in which pulmonary hemorrhages and glomerulopathy coexist. CASE REPORT: We report a 44-year-old man admitted to hospital because of chronic unexplained fever. Six days after admission he developed hemoptysis, respiratory distress and biochemical evidence of acute renal failure. High-resolution computed tomograp

Short-term use of cinacalcet in children on regular hemodialysis.

INTRODUCTION: Therapy with vitamin D3 analogs suppress the parathyroid hormone (PTH) secretion in chronic kidney disease (CKD) patients suffering from secondary hyperparathyroidism (sHPT). The concurrent administration of calcium-containing phosphate binders further increases the likelihood of developing hypercalcemia. Calcimimetics, such as cinacalcet, lower PTH level while limiting the risks of hyperphosphatemia and/or hypercalcemia. In adults, clinical experience with calcimimetics has shown significant reductions in PTH levels in cases with sHPT, but the clinical experience in children wit

Acute renal failure following the Saharan horned viper (Cerastes cerastes) bite.

INTRODUCTION: The Saharan horned viper (Cerastes cerastes) is a common snake in the sandy and rocky regions in the south of Morocco. Although nearly all snakes with medical relevance can induce acute renal failure (ARF), it's unusual except with bites by some viper species. ARF has very rarely been reported following Cerastes cerastes bite. CASE REPORT: A 55-year-old Moroccan man was bitten on his right hand by a Saharan horned viper, Cerastes cerastes. He presented 24 hours later in a state of confusion, agitation and hypotension with marked swelling of his right hand. Investigations reveale

Delayed graft function, allograft and patient srvival in kidney transplantation.

INTRODUCTION: Delayed Graft Function (DGF) is a common complication of renal transplants and the long-term relation between DGF and survival of patients and grafts is not well established. METHODS: This is a historical cohort study of transplanted patients in Taleghani Hospital of Shahid Beheshti University in Iran between 1994 and 2010. Patients who required dialysis during the first week after transplantation were considered to have DGF. The patients' conditions were updated to determine existing graft function, graft loss or patients' death at one year and five years post transplantation i

The pattern of histologically-proven acute post-infectious glomerulonephritis in Tunisian adults seen in 1976-2004.

INTRODUCTION: Acute post-infectious glomerulonephritis (APIGN) is uncommon in adults. It is widely recognized that the prognosis of APIGN is good in children. There is however little information about its long-term prognosis in adults. METHODS: Between December 1976 and October 2004, 148 adult cases of APIGN were managed in our center. We retrospectively reviewed these patients' records and evaluated their clinical course and outcome. RESULTS: The mean age of studied patients was 36 ± 15 years, and the male to female ratio was 2.3. The most common site of preceding infection was the respirat

Crossed renal ectopia coexisting with nephrolithiasis in a young Nigerian man.

INTRODUCTION: Renal ectopia or ectopic kidney is a congenital anomaly in which one or both kidneys are located in an unusual position. It results from the kidney failing to ascend properly from its origin in the true pelvis. In some cases, one kidney may cross over (crossed renal ectopia) so that both kidneys are on the same side of the body. When a crossover occurs, the two kidneys may grow together and become fused (crossed fused renal ectopia). Renal ectopia is generally uncommon; its coexistence with nephrolithiasis is even rarer. Due to its variable presentations, it is usually discovered
