Epidemiology, etiology, and diagnosis of osteoporosis.

Osteoporosis, a major public health problem, is becoming increasingly prevalent with the aging of the world population. Osteoporosis is a skeletal disorder characterized by compromised bone strength, which predisposes the individual to an increased risk of fractures of the hip, spine, and other skeletal sites. The clinical consequences and economic burden of this disease call for measures to assess individuals who are at high risk to allow for appropriate intervention. Many risk factors are associated with osteoporotic fracture, including low peak bone mass, hormonal factors, the use of certai

Bone lead levels in adjudicated delinquents. A case control study.

BACKGROUND: Lead exposure shares many risk factors with delinquent behavior, and bone lead levels are related to self-reports of delinquent acts. No data exist as to whether lead exposure is higher in arrested delinquents. The goal of this study is to evaluate the association between lead exposure, as reflected in bone lead levels, and adjudicated delinquency. METHODS: This is a case-control study of 194 youths aged 12-18, arrested and adjudicated as delinquent by the Juvenile Court of Allegheny County, PA and 146 nondelinquent controls from high schools in the city of Pittsburgh. Bone lead w

Histopathological structure of the pial arteriovenous malformation in adults: observation by reconstruction of serial sections of four surgical specimens.

Surgical specimens of four adult cases of cerebral pial arteriovenous malformation (AVM) were examined by reconstruction of serial sections to disclose the actual anatomical structure and to analyze the mechanism of gradual growth and rupture. More than seven arteriovenous shunts (AV-shunts) per case were found in all four cases, and these were divided into two types; one or a few shunts larger than 700 microm, and those with several shunts smaller than 700 microm in diameter. The former were located exclusively in the subarachnoid space and showed severe mural fibrosis. The vessels just ahead

Integrated defense system overlaps as a disease model: with examples for multiple chemical sensitivity.

The central nervous, immune, and endocrine systems communicate through multiple common messengers. Over evolutionary time, what may be termed integrated defense system(s) (IDS) have developed to coordinate these communications for specific contexts; these include the stress response, acute-phase response, nonspecific immune response, immune response to antigen, kindling, tolerance, time-dependent sensitization, neurogenic switching, and traumatic dissociation (TD). These IDSs are described and their overlap is examined. Three models of disease production are generated: damage, in which IDSs fu

International perspectives of lead exposure and lead toxicity.

Three approaches have been used to examine how human body burdens of lead depend on different environments: (1) In paleopathologic studies, lead concentrations have been determined in well-preserved human bones or teeth, and pre-pollution samples generally show lead concentrations of about 1% of current levels in industrialized countries. (2) Geographic comparisons of blood-lead concentrations show low levels in, Nepal, Faroe Islands, and Sweden, while high levels occur in Mexico and Malta; average blood-lead levels may vary by a factor of 10 or more. (3) In analytical epidemiology, major expo…

The prevalence of occupational exposure to ergonomic risk factors: A systematic review and meta-analysis from the WHO/ILO Joint Estimates of the Work-related Burden of Disease and Injury.

BACKGROUND: The World Health Organization (WHO) and the International Labour Organization (ILO) are developing joint estimates of the work-related burden of disease and injury (WHO/ILO Joint Estimates), with contributions from a large network of experts. Evidence from mechanistic and human data suggests that occupational exposure to ergonomic (or physical) risk factors may cause osteoarthritis and other musculoskeletal diseases (excluding rheumatoid arthritis, gout, and back and neck pain). In this paper, we present a systematic review and meta-analysis of the prevalence of occupational exposu

[Risk factors for the development of lymphatic fistula after ilioinguinal lymph node dissection before isolated limb perfusion and its potential clinical relevance].

INTRODUCTION: After ilioinguinal radical lymph node dissection (RLND), the therapy for lymph fistulas constitutes a challenge. Risk factors for the genesis of lymph fistulas have not been sufficiently evaluated. We investigated possible factors that could influence the development of lymph fistulas in patients suffering from malignant melanoma after iloinguinal RLND. PATIENT AND METHODS: The analysis was related to patients with intransit and lymphonodal metastasised malignant melanoma of the lower limb, who underwent RLND and isolated limb perfusion (ILP). Prospective data acquisition from p

Risk of lung cancer and leukemia from exposure to ionizing radiation and potential confounders among workers at the Portsmouth Naval Shipyard.

Significantly elevated lung cancer deaths and statistically significantly positive linear trends between leukemia mortality and radiation exposure were reported in a previous analysis of Portsmouth Naval Shipyard workers. The purpose of this study was to conduct a modeling-based analysis that incorporates previously unanalyzed confounders in exploring the exposure-response relationship between cumulative external ionizing radiation exposure and mortality from these cancers among radiation-monitored workers in this cohort. The main analyses were carried out with Poisson regression fitted with m

Invasive Meningococcal Disease in Hospitalized Children in Argentina, 2020-2024: Clinical, Epidemiologic and Molecular Findings From a Multicenter Active Surveillance Study.

BACKGROUND: Invasive meningococcal disease (IMD) is a low-incidence but high-impact infection associated with substantial mortality and sequelae. Following MenACWY vaccine introduction into Argentina's National Immunization Program in 2017, updated data on residual disease burden are needed. METHODS: A multicenter active hospital-based surveillance study was conducted in 6 pediatric sentinel units in Argentina (January 2020-December 2024) in children younger than 15 years with suspected IMD. Cases were confirmed by culture and/or polymerase chain reaction from a normally sterile body site. Ho

Turning Back the Ageing Clock? Brain Age in Neuroprotective and Clinical Interventions.

Neuroimaging-derived brain age has emerged as an intuitive marker of brain health and cognitive longevity. The difference between brain age and chronological age, termed brain-predicted age difference (brain-PAD), quantifies deviation from normative brain ageing and may help evaluate whether interventions could attenuate brain ageing trajectories. This review synthesizes the methodological foundations of brain age estimation, factors linked to brain-PAD across healthy ageing and disease, evidence from clinical interventions, and emerging translational applications in clinical trials. Across no

Household Chaos in Pediatric Obesity Prevention and Treatment: A Systematic Review.

BACKGROUND: Family-based pediatric obesity prevention and treatment programs require sustained organization within the home, yet the role of household chaos in intervention implementation and response has not been synthesized. We evaluated whether household chaos predicts engagement, adherence, or response and whether interventions reduce household chaos. METHODS: PubMed, Embase, Scopus, Web of Science, PsycINFO, and CINAHL were searched through April 2026, with citation and targeted Google Scholar searches. Eligible quantitative studies examined household chaos within pediatric obesity preve

Changes in Weight and Metabolic Indices after Switching to Aripiprazole Once-Monthly in Patients with Schizophrenia According to Prior Oral Antipsychotics.

INTRODUCTION: Antipsychotic-induced weight gain and metabolic disturbance are common in schizophrenia, but liability varies across agents. Aripiprazole has a relatively favorable metabolic profile, yet evidence on aripiprazole once-monthly, its long-acting injectable formulation, remains limited. We hypothesized that switching from a higher liability antipsychotic would yield a greater weight reduction than switching from a lower liability agent. METHODS: This 16-week, prospective, open-label, multicenter study enrolled patients with stable schizophrenia. Participants were stratified by the m

Outcomes of Mitral Valve Intervention for Patients with Bileaflet Mitral Valve Prolapse and Preoperative Ventricular Arrhythmias.

Bileaflet mitral valve (MV) prolapse may be associated with ventricular arrhythmias and cardiac arrest. Given the paucity of data analyzing the role of MV interventions, we analyzed outcomes of MV surgery in patients with bileaflet MV prolapse and preoperative ventricular arrhythmias. Between 1994 and 2018, 30 adult patients with bileaflet MV prolapse, severe MV regurgitation, and documented preoperative ventricular arrhythmias underwent MV operation at our institution. Ventricular arrhythmia was defined as ventricular tachycardia, ventricular fibrillation, cardiac arrest, or bigeminy. Pre- an

Neuromuscular and Functional Adaptations Promoted by Lower Limb Isometric Training with NMES Conditioning Contractions in Older Adults.

Aging induces sarcopenia and reduces bone mineral density, altering body composition. These modifications contribute to physical decline, increase non-communicable disease risk and increase the likelihood of hospitalization, thereby representing a substantial public health burden. In this study, we assessed the effects of isometric training with neuromuscular electrical stimulation conditioning contractions (ISO-NMES) and dynamic resistance training (DRT) on physical and functional capacities. Moreover, we investigated the impact of ISO-NMES training on the force and power of the trained and u

Elevated Depressive Symptoms Shape Gut Barrier Integrity, LPS Translocation, and PUFA Composition in IBS-D: Evidence from a Low-FODMAP Dietary Intervention.

Introduction: Alterations of the microbiota-gut-brain axis, including increased intestinal permeability (IP), changes in microbial activity, and immune activation, are central to the pathophysiology of irritable bowel syndrome with diarrhea (IBS-D). The low-fermentable oligo-di-monosaccharides and polyols (FODMAP) diet (LFD) is an established therapy for IBS, yet its systemic effects, particularly in patients with elevated depressive symptoms, remain incompletely characterized. Methods: This single-arm pre-post study investigated associations between depressive symptom severity and markers of
