Background and Objectives: Distal tibia fractures are high-energy injuries characterized by a mismatch between standard plate designs and the patient's specific anatomical bone structure, which can lead to severe soft tissue damage. Recent advancements have focused on the development of customized metal plates using three-dimensional (3D) printing technology. However, 3D-printed metal plates using titanium alloys have not incorporated a locking system due to the brittleness of these alloys. Therefore, this study aimed to determine whether a locking mechanism can be effectively implemented usin

INTRODUCTION: Sacroiliac joint pain (SIJP) is one of the pathological conditions of adjacent segment disorders occurring after adult spinal deformity (ASD) surgery. This study aimed to test the hypothesis that even in ASD surgery using S2 alar-iliac (S2AI) screws, SIJP can develop much earlier than reported previously and can be rescued by ultrasound-guided sacroiliac joint block.
METHODS: Overall, 94 patients with ASD treated with long spinal fusion using S2AI screws were prospectively investigated for SIJP postoperatively, and the effect of ultrasound-guided sacroiliac joint block was evalu

Proximal junctional kyphosis (PJK) is a common problem after multilevel spine instrumentation for adult spinal deformity. Various anti-PJK techniques such as junctional tethers for ligamentous augmentation have been proposed. We present an operative video demonstrating technical nuances of junctional tether "weave" application. A 70-yr-old male with prior L2-S1 instrumented fusion presented with worsening back pain and posture. Imaging demonstrated pathological loss of lumbar lordosis (flat back deformity), proximal junctional failure, and pseudarthrosis. The patient had severe global and segm

The combination of Massive epidural scarring and spinal deformity represents the ultimate challenge for the spinal deformity surgeon. This is observed more and more as the population is aging and the number of spine surgery is increasing. In assessing the patient with spinal deformity and epidural scarring, one should carry out a thorough medical work up including Dexa scan, comorbidities, and in most cases a Myelo-CT scan that will identify the extent of the previous fusion, the fixed or semi-rigid nature of the deformity with complete anterior fusion or only bone bridges, the evaluation of t

While biomechanical testing has shown a correlation between decortication and anchor failure load, the effects of partial decortication on the biomechanical properties of all-suture anchors remain unclear. We aimed to evaluate the biomechanical effects of partial decortication on all-suture anchors and conventional suture anchors in Sawbones of varying densities. Suture anchors were tested in nondecorticated, partially decorticated, and completely decorticated Sawbones. Two types of all-suture anchors and one type of conventional anchor were evaluated. Two types of biphasic polyurethane foam w

Rupture of the scapholunate interosseous ligament can cause the dissociation of scaphoid and lunate bones, resulting in impaired wrist function. Current treatments (e.g., tendon-based surgical reconstruction, screw-based fixation, fusion, or carpectomy) may restore wrist stability, but do not address regeneration of the ruptured ligament, and may result in wrist functional limitations and osteoarthritis. Recently a novel multiphasic bone-ligament-bone scaffold was proposed, which aims to reconstruct the ruptured ligament, and which can be 3D-printed using medical-grade polycaprolactone. This s

INTRODUCTION AND IMPORTANCE: After tooth extraction, horizontal bone loss and a reduction in the bucco-lingual ridge dimension may occur. This often necessitates bone augmentation to enable implant placement. For this reason, techniques such as ridge splitting and expansion have been developed; however, they present challenges in single-tooth sites and in the mandible. Complications such as buccal plate fractures remain a concern.
CASE PRESENTATION: A 49-year-old female, seeking fixed replacement of a missing mandibular molar, presented with a narrow alveolar ridge at the extraction site. Alv

AIM: To investigate the significance of the initial stability of dental implants for the establishment of osseointegration in an experimental capsule model for bone augmentation.
MATERIAL AND METHODS: Sixteen male rats were used in the study. In each rat, muscle-periosteal flaps were elevated on the lateral aspect of the mandibular ramus on both sides, resulting in exposure of the bone surface. Small perforations were then produced in the ramus. A rigid, hemispherical Teflon capsule with a diameter of 6 mm and a height of 4 mm and with a hole in its middle portion, prepared to fit the circumf

BACKGROUND AND AIM: The development of percutaneous pedicle screws (PPSs) has led to the development of minimally invasive spine stabilization (MISt) procedures, which has decreased the invasiveness of spine surgery. Conventionally, PPSs were available as polyaxial screws only, making it technically challenging to perform percutaneous repair of thoracolumbar vertebral fractures with implants. This study aimed to evaluate percutaneous posterior fixation for thoracolumbar vertebral fractures using a monoaxial screw system and determine imaging and clinical outcomes until after implant removal.

PURPOSE: Comminuted olecranon fractures are commonly fixed with posterior locking plates (PLPs). Though biomechanically validated, this method comes with risks of symptomatic implant prominence and wound breakdown. Dual locking plates (DLPs) placed on the medial and lateral surfaces of the olecranon theoretically avoid these risks and allow for fixation of proximal fracture fragments in multiple planes. A biomechanical comparison of DLP and PLP fixation would help to validate the use of DLPs in comminuted olecranon fractures.
METHODS: Seven matched pairs of cadaveric upper extremities were ev

BACKGROUND CONTEXT: There are several options for the stabilization of high-grade lumbosacral spondylolisthesis including transdiscal screws, the Bohlman technique (transdiscal fibular strut) and the modified Bohlman technique (transdiscal titanium mesh cage). The choice of an optimum construct remains controversial; therefore, we endeavoured to study and compare the biomechanical performance of these 3 techniques.
PURPOSE: The aim of this study was to compare 3 types of transdiscal fixation biomechanically in an in vitro porcine lumbar-sacral spine model.
