Dissociation between airway and transpulmonary pressures during external chest wall restriction: a prospective self-controlled physiological study.

BACKGROUND: External chest wall restriction can alter how airway pressure is transmitted to the lung. This may make airway pressure an unreliable surrogate for lung stress during mechanical ventilation. We aimed to determine whether short-term chest strapping dissociates airway pressure from transpulmonary pressure by increasing chest wall elastance in mechanically ventilated postoperative intensive care unit (ICU) patients. METHODS: This was a single-center, prospective, self-controlled physiological study. Postoperative ICU patients receiving invasive mechanical ventilation were screened fo

Superficial Parasternal Intercostal Plane Block for Analgesia After Cardiac Surgery: An Updated Meta-analysis of Randomized Controlled Trials With Meta-regression and Trial Sequential Analysis.

BACKGROUND: Effective postoperative pain control is essential for enhanced recovery after cardiac surgery, yet optimal multimodal strategies continue to evolve. The superficial parasternal intercostal plane (S-PIP) block has been proposed as a simple and safe technique for median sternotomy analgesia; however, current evidence remains fragmented, and its clinical impact is unclear. METHODS: We conducted a systematic review and meta-analysis of randomized controlled trials assessing the analgesic efficacy and safety of the S-PIP block in adult cardiac surgery. Community Trust Must Be Earned: Building Authentic Relationships Is the Key.

BACKGROUND: After Mercy Hospital closed, the Public Health Management Corporation Public Health Campus on Cedar (Cedar) opened on its site. According to our survey 3 years later, only 17.6% of households in the catchment area used Cedar's services. A history of hospital closures and the former hospital's reputation contributed to deep community mistrust in local health care institutions. OBJECTIVES: This study examined community awareness of Cedar, willingness to try its services, and trust as a predictive factor. METHODS: Using a community-based participatory research approach, the evaluati

Analysis of Complications Associated with Basivertebral Nerve Ablation for Vertebrogenic Low Back Pain: A Multicenter Retrospective Cohort Study of 1,145 Patients.

OBJECTIVE: Low back pain (LBP) is a leading cause of pain and disability worldwide. Recent studies demonstrate a vertebrogenic model involving the basivertebral nerve (BVN) as a cause of chronic LBP. Vertebral endplates are highly innervated by the BVN, and literature shows type 1 and 2 Modic changes at vertebral endplates are a source of vertebrogenic low back pain (VBP). As a result, intraosseous radiofrequency ablation of the BVN is a minimally invasive outpatient procedure used to treat VBP. We conducted a retrospective review of 1,145 patients that have undergone BVN ablation to assess fo

Associations between diet quality, genetic risk of psoriasis, and psoriasis incidence: A prospective cohort study.

BACKGROUND AND AIMS: Psoriasis is a chronic inflammatory skin condition that adversely affects quality of life. Given limited evidence to inform dietary recommendations for psoriasis prevention, this study used data from the UK Biobank to examine associations between diet quality and incident psoriasis, and whether genetic susceptibility moderates this relationship. METHODS: Participants without psoriasis at baseline who completed ≥2 Oxford WebQ 24-h dietary recalls were included. Psoriasis incidence was identified through self-report, primary care, and hospital records. Ten diet quality indi

Ultra-fine micronized human dermal matrix reprograms the chronic pressure-ulcer microenvironment to accelerate healing.

Chronic pressure ulcers persist within a self-sustaining microenvironment marked by unresolved inflammation, inadequate neovascularization, and excessive extracellular-matrix (ECM) degradation, highlighting the need for injectable bioactive scaffolds that can be delivered precisely to reprogram compromised tissue. The ultra-fine micronized human acellular dermal matrix (UFM-hADM; ∼70 μm) is designed to remain suspendable and pass through narrow-gauge injection, enabling uniform distribution along irregular wound margins and undermined tissue planes where sheet-type matrices may have limited co

Development of a Potent Engineered Microbial Lipase for the Treatment of Exocrine Pancreatic Insufficiency.

BACKGROUND AND AIMS: Exocrine pancreatic insufficiency (EPI) is a chronic condition associated with inadequate digestive enzyme secretion from the pancreas, causing impaired digestion, malabsorption, and malnutrition. EPI affects 10%-20% of the general population, and the standard of care includes pancreatic enzyme replacement therapy (PERT) that is exclusively derived from porcine pancreatic extracts. However, PERT is associated with several limitations, including suboptimal efficacy, which results in increased pill burden and high treatment costs. This work aimed to identify and engineer a p

Translating B-Cell and Plasma-Cell Targeting from Oncology to Autoimmunity: Modalities, Quantitative Bridging, and a Development Roadmap.

B cells drive B-cell malignancies and diverse autoimmune diseases through autoantibody production, antigen presentation, and cytokine dysregulation, and persistence of pathogenic B-cell or plasma-cell compartments. Across autoimmune diseases, the dominant pathogenic compartment varies, ranging from precursor-dependent states to tissue-compartment inflammation and long-lived plasma cell-weighted autoantibody production. This review synthesizes the biological rationale for B-cell-axis targeting across oncology and autoimmunity and proposes a pathobiological framework spanning precursor-dependent

Tenofovir alafenamide is superior to tenofovir disoproxil fumarate and entecavir in cost-effectiveness of treatment of chronic hepatitis B in china with new volume-based procurement policy.

BACKGROUND: Evidence supports the long-term efficacy of Nucleos(t)ide Analogs (NAs) therapy in improving chronic hepatitis B (CHB) prognosis. However, determining the most cost-effective first-line NAs remains unclear. China's implementation of the New Volume-Based Procurement Policy (NVBP Policy) in 2019 led to substantial price reductions for entecavir (ETV), tenofovir disoproxil fumarate (TDF), and tenofovir alafenamide (TAF). This study assesses the cost-effectiveness of ETV, TDF, and TAF, both with and without NVBP, for CHB in China. METHODS: A state-transition model, parameterized using
