Discipline-specific responses to a complex migraine case: a vignette-based survey among neurologists, psychiatrists, and family physicians.

BACKGROUND: Chronic migraine (CM), especially when complicated by medication overuse headache (MOH), frequently coexists with psychiatric and somatic comorbidities that challenge conventional monodisciplinary management. Integrated, interdisciplinary care has been proposed as a solution, but real-world implementation remains limited. OBJECTIVE: To evaluate and compare the diagnostic reasoning, treatment preferences, and follow-up strategies among neurologists, psychiatrists, and family physicians when managing a complex case of CM with comorbidities and medication overuse. METHODS: A case-ba

Atomoxetine Reduces Decisional Impulsivity in Human Cocaine Addiction.

BACKGROUND: Impulsivity is a well-known determinant of maladaptive behavior in cocaine use disorder (CUD), but there are currently no effective strategies for managing excessive impulsivity. Growing evidence from preclinical and clinical studies suggests that atomoxetine, a selective noradrenaline reuptake inhibitor, is effective in improving impulse control in both healthy individuals and individuals with neuropsychiatric conditions. METHODS: We investigated the effects of atomoxetine on decisional impulsivity in patients with CUD. In a randomized, double-blind, placebo-controlled, crossover

Preterm Birth: Screening and Prediction.

Preterm birth (PTB) affects approximately 10% of births globally each year and is the most significant direct cause of neonatal death and of long-term disability worldwide. Early identification of women at high risk of PTB is important, given the availability of evidence-based, effective screening modalities, which facilitate decision-making on preventative strategies, particularly transvaginal sonographic cervical length (CL) measurement. There is growing evidence that combining CL with quantitative fetal fibronectin (qfFN) and maternal risk factors in the extensively peer-reviewed and valida

A Review of Levodopa Formulations for the Treatment of Parkinson's Disease Available in the United States.

Purpose: The safety and efficacy of levodopa formulations are evaluated to inform clinical decision making for the treatment of Parkinson's disease. Summary: Levodopa is a cornerstone of treatment for Parkinson's disease due to its proven efficacy. Although many patients can initially be managed using immediate release tablets, as their disease progresses they often require escalating doses as well as more frequent dosing to prevent wearing off effects. Additionally, patients who experience time in the off state may struggle with the delay between medication administration and onset of action.

Tracking the disease progression of lumbar spinal stenosis using objective gait metrics: a case report.

We present the case of an 85-year-old woman who presented to our clinic with neurogenic claudication due to lumbar spinal stenosis (LSS) ove … he weeks prior to her surgery. Therefore, the extent of walking deterioration may be able to inform clinical decision-making regarding appropriate walking assistance. To our knowledge, this is the first report that objectively documents the deterioration of LSS using walking metrics for such a prolonged duration of time.

The Effect of I155T, K156Q, K156E and N186K Mutations in Hemagglutinin on the Virulence and Reproduction of Influenza A/H5N1 Viruses.

Data analytics has emerged as a crucial tool for understanding the multifaceted impacts of the COVID-19 pandemic. By collecting and analyzing extensive datasets, researchers have gained valuable insights into the virus's transmission, severity, and the effectiveness of public health measures. Yet, many contradictive and non-reproducible results have been published. The significance of cohort representativeness in this context cannot be overstated, as diverse cohorts provide a comprehensive understanding of how various demographic and clinical factors influence COVID-19 outcomes. This study inv

Steroid Profiling and Immunohistochemistry for Subtyping and Outcome Prediction in Primary Aldosteronism-a Review.

PURPOSE OF REVIEW: Steroid profiling and immunohistochemistry are both promising new tools used to improve diagnostic accuracy in the work-up of primary aldosteronism (PA) and to predict treatment outcomes. Herein, we review the recent literature and present an outlook to the future of diagnostics and therapeutic decision-making in patients with PA. RECENT FINDING: PA is the most common endocrine cause of arterial hypertension and unilateral forms of the disease are potentially curable by surgical resection of the overactive adrenal. Recent studies have shown that adrenal steroid profiling by

Chronic ciguatoxin poisoning causes emotional and cognitive dysfunctions in rats.

Ciguatoxins are marine biotoxins that induce the human poisoning syndrome known as ciguatera fish poisoning (CFP). In humans, different kinds of neurological symptoms have been reported after CFP, including anxiety, depression and memory loss. Repetitive exposures to sub-threshold levels of ciguatera toxins may cause irreversible sub-clinical damage, and eventually cause more severe illness. Our previous study has shown that an acute single dose of Pacific ciguatoxin-1 (P-CTX-1) induced synaptic facilitation and blockage of the induction of electrical stimulation-induced long-term potentiation

Comorbidity-age index: a clinical measure of biologic age before allogeneic hematopoietic cell transplantation.

PurposeAge has long been used as a major factor for assessing suitability for allogeneic hematopoietic cell transplantation (HCT). The HCT-comorbidity index (HCT-CI) was developed as a measure of health status to predict mortality risk after HCT. Whether age, comorbidities, or both should guide decision making for HCT is unknown.Patients and methodsData from 3,033 consecutive recipients of HLA-matched grafts from five institutions contributed to this analysis. Patients were randomly divided into a training set to develop weights for age intervals and a validation set to assess the performance

Relevance of the gut/joint axis for the management of spondyloarthritis in daily clinical practice.

PURPOSE OF REVIEW: Thirty years ago, the concept of microscopic gut inflammation in spondyloarthritis (SpA) was established. Over the past decade, there has been tremendous progress in the earlier diagnosis of SpA. In addition, it has been suggested that, because of improved hygiene over the past years, exposure to microorganisms has changed, leading to a shift in diseases, for example, a decreased incidence of reactive arthritis. It is therefore necessary to re-establish the role of gut inflammation in SpA. RECENT FINDINGS: The prevalence of microscopic gut inflammation could be confirmed in

APACHE II scoring for the prediction of immediate surgical complications in head and neck cancer patients.

BACKGROUND: Aggressive surgical treatment of oral and oropharyngeal cancers may be compromised by significant surgical complications. Early identification of patients at risk for postoperative complications may assist in clinical decision-making. The objective of this study was to assess the value of the Acute Physiology and Chronic Health Evaluation (APACHE II) score in predicting early postoperative surgical complications. METHODS: There were 510 cases of free tissue transfer surgery identified on the Liverpool Oncology Head and Neck Database for 1995 to 2002. APACHE II data were collected …

Challenging cases in infection prevention and control: proceedings from SHEA Spring 2025.

Hospital epidemiologists and infection prevention professionals are frequently required to make high-stakes decisions in complex clinical scenarios where evidence-based guidance is limited or absent. These decisions often carry significant implications for patient safety, healthcare worker protection, hospital operations, and legal or financial risk. This article presents three challenging cases originally featured during the "Interesting Cases" session at the Society for Healthcare Epidemiology of America (SHEA) Spring 2025 Conference, with the goal of sharing practical lessons learned and co

Patient-rated importance and receipt of information for colorectal cancer screening.

BACKGROUND: Physician recommendation is one of the most important determinants of obtaining colorectal cancer (CRC) screening; however, little is known about the degree to which CRC screening discussions include information that patients report as important to guide screening decisions. This study examines and compares both patient rated importance and physician communication of key information elements about CRC screening during annual physical examinations. DESIGN: Cross-sectional cohort. SETTING: 26 ambulatory clinics of an integrated delivery system in the Midwest. PARTICIPANTS: 64 prim

[Neurofilament light chain as a tool for assessing disease activity and treatment response in Multiple Sclerosis].

Serum neurofilament light chain (sNfL), a neuronal structural protein, is released upon neuroaxonal damage and correlates with clinical and imaging activity, brain atrophy, and disability progression, though it is not disease-specific. This review addresses measurement methods (SiMoA, automated platforms), pre-analytics/matrix effects, and central confounders (esp. age, BMI/volume of distribution, renal function, intercurrent events), emphasizing standardization via age- and BMI-corrected reference data using Z-scores or percentiles.Clinically, sNfL supports baseline risk stratification and tr

Editorial Commentary: Subpectoral Biceps Tenodesis Is a Safe Alternative to Arthroscopic Repair for Unstable SLAP Tear in the Young Athlete: Should We Do the Last Operation First?

The clinical significance of structural pathology affecting the biceps-superior labrum complex may be highly variable. Among younger, physically active patients with symptomatic superior labrum anterior-posterior (SLAP) tears that have failed to respond to nonoperative treatment, we continue to lack clear high-level evidence to guide surgical decision making, including a decision between arthroscopic SLAP repair or primary biceps tenodesis for more unstable, type II lesions. Rates of patient satisfaction, return to play, return to prior level of activity, and secondary revision rate are widely

Tragedy and Grenzsituationen in genetic prediction.

Philosophical anthropologies that emphasise the role of the emotions can be used to expand existing notions of moral agency and learning in … ays in which these two positions may be used for reflecting upon different perspectives involved in clinical decision-making, those of patients, clinicians and bioethicists. We conclude that the ideas we introduce can be used as hermeneutic tools for situating learning and dialogue within a broader cultural field in which literature and art may also play important roles.

[Percutaneous cementing techniques of the spine -- chances and limits].

Vertebroplasty and Kyphoplasty represent minimal-invasive techniques for cement augmentation of vertebral bodies. Both procedures are successfully used for pain-relieving stabilizations of osteoporotic fractures or malignant processes. Advantages of kyphoplasty over vertebroplasty are to be seen in the possibility of deformity correction as well as in a decreased risk of cement extrusions which represent the most important potential for clinical complications. Long-term experiences with the effect of cementing are sparse. Thus it seems even more important, to judge indications and possibilitie
