Anatomy of the portal tract for endoscopic decompression of the first branch of the lateral plantar nerve.

PURPOSE: Our purpose is to study the anatomy of the portal tract for endoscopic decompression of the first branch of the lateral plantar nerve. METHODS: The anatomy of the portals and portal tract with endoscopic release of the first branch of the lateral plantar nerve was studied in 12 feet in 6 cadaveric bodies. RESULTS: The proximal portal is located at the fascial opening for the first branch of the lateral plantar nerve and is about 16 mm inferior and 23 mm posterior to the tip of the medial malleolus. The distal portal is located at the inferior edge of the deep fascia of the abductor

Traumatic occurrence of chest wall tamponade secondary to subcutaneous emphysema.

Subcutaneous emphysema is a physical finding that itself is usually perceived as benign yet rarely may, in and of itself, be life-threatening. We present an unusual case of a 67-year-old woman who developed delayed severe subcutaneous emphysema and tension pneumothorax from a rib fracture subsequent to a fall. We review the pathophysiology, manifestations and management options of this disorder. In patients whose clinical condition allows it, chest tube placement prior to intubation should be considered. Furthermore, positive end-expiratory pressure should be minimized. We present a case that

Teflon granuloma with midbrain cysts after microvascular decompression of the trigeminal nerve.

Granuloma formation is a known complication after microvascular decompression using Teflon fibers. Such granulomas commonly present with recurrent neuralgia whereas other symptoms are exceedingly rare. We report the first case of a multicystic lesion due to a Teflon granuloma that is also uncommon for the lack of recurrent neuralgia.

Release of the transverse carpal ligament alone is associated with elevated pressure beneath the distal volar forearm fascia in a cadaver model of carpal tunnel syndrome.

PURPOSE: The purpose of this study is to determine whether release of the distal volar forearm fascia (DVFF) is necessary at the time of median nerve decompression for carpal tunnel syndrome. METHODS: Five fresh-frozen cadaver specimens were mounted vertically with the hand dependent and a 2.27-kg weight suspended from the fingers. A pressure sensor wire was used to measure pressures starting just distal to the transverse carpal ligament (TCL). The wire was withdrawn proximally in 5-mm increments and into the forearm until pressure was below 10 mm Hg. An incision in the forearm was extended d

Acute lung injury and multiple organ dysfunction syndrome secondary to intra-abdominal hypertension and abdominal decompression in extensively burned patients.

BACKGROUND: Secondary abdominal compartment syndrome (ACS) is a lethal complication after resuscitation from burn shock, even after abdominal decompression (AD) is performed. This study investigated increased susceptibility to multiple organ dysfunction syndrome (MODS) in extensively burned patients with ACS. METHODS: Patients admitted to our burn unit between 2002 and 2005 with burns affecting 40% or more of the total body surface area without severe inhalation injury were analyzed. Hemodynamic parameters, blood gas analysis, and intrabladder pressure as intra-abdominal pressure were recorde

A review of the management of thoracolumbar burst fractures.

BACKGROUND: Burst fractures account for more than half of all thoracolumbar fractures, which often cause a neurologic deficit and present a significant economic burden to the family and society. Accepted methods of treatment of thoracolumbar burst fractures include conservative therapy, posterior reduction and instrumentation, and anterior decompression and instrumentation. However, the management of thoracolumbar burst fractures has been the subject of much controversy. METHODS: Publications reporting clinical data relating to the thoracolumbar burst fractures were reviewed. These articles w

[Acute spinal epidural hematoma complicated with essential thrombocytosis].

An 82-year-old woman was brought to our hospital with back pain and paraplegia. Her platelet level was elevated to 456 x 10(4) x mm(-3). She was diagnosed as having acute spinal epidural hematoma complicated with essential thrombocytosis. After emergency decompression surgery, chemotherapy with hydroxycarbamide was performed and platelets decreased to 80 x 10(4) x mm(-3). There were no perioperative complications, such as hemorrhage or thrombosis in other organs.

Complications of intestinal milking: experimental model.

BACKGROUND: Intestinal milking is a frequently used method in abdominal surgery for various purposes; although it is frequently used for eliminating distension, moving faeces, bezoar or other foreign objects proximal or distal, there are no experimental or prospective studies in the surgical literature with respect to complications of the method. The purpose of the present study was to investigate complications of the milking process in an experimental model. METHODS: Forty-four outbred, 8-month old Wistar albino male rats with weights ranging between 195 and 225 g were used. Four rats were u

Spinal extradural arachnoid cyst causing cord compression in a 15-year-old girl: a case report.

The authors describe the case of a fifteen-year-old girl with progressive paraparesis of the lower limbs that was caused by an intraspinal extradural dorsal arachnoid cyst at the level of Th 3-6. Diagnosis was established with MRI and MRI myelography. The latter revealed the CSF-like content of the cyst. The patient underwent laminotomy and en bloc resection of the cyst. Ligation of the pedicle of the cyst was done with laminoplasty. Quick and complete recovery was observed after surgery.

Simple decompression or subcutaneous anterior transposition of the ulnar nerve for cubital tunnel syndrome.

The purpose of this prospective randomised study was to evaluate which operative technique for treatment of cubital tunnel syndrome is preferable: subcutaneous anterior transposition or nerve decompression without transposition. This study included 66 patients suffering from pain and/or neurological deficits with clinically and electromyographically proven cubital tunnel syndrome. Thirty-two patients underwent nerve decompression without transposition and 34 underwent subcutaneous transposition of the nerve. Follow-up examinations evaluating pain, motor and sensory deficits as well as motor ne

Biomechanical rationale of endoscopic decompression for lumbar spondylolysis as an effective minimally invasive procedure - a study based on the finite element analysis.

We evaluated the biomechanical behavior of the endoscopic decompression for lumbar spondylolysis using the finite element technique. An experimentally validated, 3-dimensional, non-linear finite element model of the intact L3 - 5 segment was modified to create the L4 bilateral spondylolysis and left-sided endoscopic decompression. The model of Gill's laminectomy (conventional decompression surgery of the spondylolysis) was also created. The stress distributions in the disc and endplate regions were analyzed in response to 400 N compression and 10.6 Nm moment in clinically relevant modes. The r

Cerebral perfusion pressure for prediction of recurrent intracranial hypertension after primary decompressive craniectomy.

BACKGROUND: Decompressive craniectomy (DC) with dural grafting may be performed in patients with moderate (Glasgow-Coma-Scale [GCS] score 9-12 points) or severe traumatic brain injury (TBI; GCS score <or= 8 points) and threatening herniation. However, its effectiveness especially after primary craniectomy is still discussed due to missing evidence of improved outcome. The objectives of this study were to show the incidence of recurrent intracranial hypertension after primary DC, to identify predictive parameters for secondary DC, and to evaluate the long-term neurological performance 12 months

Living with achondroplasia: quality of life evaluation following cervico-medullary decompression.

Achondroplasia is the most common of the heritable skeletal dysplasias. Cervico-medullary compression is a frequently encountered and potentially lethal neurological complication. Cervico-medullary decompression (CMD) at the foramen magnum is often employed to relieve the pressure on the emerging cervical cord. Given the inherent risks associated with major surgery, there has been a substantial debate regarding the best criteria for CMD. Our objectives for this study are to explore the quality of life of patients who had undergone CMD, and to assess whether surgery is associated with mortality
