Venezuelan equine encephalitis in a teenager visiting Central America.

BACKGROUND: Stupor, coma, and other alterations of consciousness are among the most serious life-threatening emergencies faced by the emergency department physician. When a patient arrives with altered mentation from Central or South America, the usual causes that occur in the United States must be considered; however, other unusual tropical disease must be excluded, such as Venezuelan equine encephalitis (VEE). OBJECTIVE: This study aimed to review the clinical features of VEE. CASE: A 17-year-old female traveled to Belize and developed vomiting, diarrhea, fever, headaches, and myalgias. Ov

New PARP gene with an anti-alphavirus function.

Alphaviruses represent a highly important group of human and animal pathogens, which are transmitted by mosquito vectors between vertebrate hosts. The hallmark of alphavirus infection in vertebrates is the induction of a high-titer viremia, which is strongly dependent on the ability of the virus to interfere with host antiviral responses on both cellular and organismal levels. The identification of cellular factors, which are critical in orchestrating virus clearance without the development of cytopathic effect, may prove crucial in the design of new and highly effective antiviral treatments.

A phase I dose escalation trial of vaccine replicon particles (VRP) expressing prostate-specific membrane antigen (PSMA) in subjects with prostate cancer.

PSMA-VRP is a propagation defective, viral replicon vector system encoding PSMA under phase I evaluation for patients with castration resistant metastatic prostate cancer (CRPC). The product is derived from an attenuated strain of the alphavirus, Venezuelan Equine Encephalitis (VEE) virus, and incorporates multiple redundant safety features. In this first in human trial, two cohorts of 3 patients with CRPC metastatic to bone were treated with up to five doses of either 0.9×10(7)IU or 0.36×10(8)IU of PSMA-VRP at weeks 1, 4, 7, 10 and 18, followed by an expansion cohort of 6 patients treated wit

Early activation of the host complement system is required to restrict central nervous system invasion and limit neuropathology during Venezuelan equine encephalitis virus infection.

Venezuelan equine encephalitis virus (VEEV) is a mosquito-borne RNA virus of the genus Alphavirus, family Togaviridae, that is responsible for sporadic outbreaks in human and equid populations in Central and South America. In order to ascertain the role that complement plays in resolving VEEV-induced disease, complement-deficient C3(-/-) mice were infected with a VEEV mutant (V3533) that caused mild, transient disease in immunocompetent mice. In the absence of a functional complement system, peripheral inoculation with V3533 induced much more severe encephalitis. This enhanced pathology was as

Assessment of evidence for a protective role of vitamin D in multiple sclerosis.

Evidence for a role of vitamin D insufficiency in determining risk in Multiple Sclerosis (MS) is supported by studies in both pediatric- and adult-onset patients. The potential role of vitamin D in modulating MS disease activity is an area of active clinical trials research, and the possibility of primary disease prevention with vitamin D supplementation in early life is an emerging concept. With Sir Austin Bradford Hill's criteria as a framework, the present review assesses the evidence for a causal relationship between vitamin D insufficiency and the pathobiology of MS, and discusses rationa

A mouse-based assay for the pre-clinical neurovirulence assessment of vaccinia virus-based smallpox vaccines.

Post-vaccinal encephalitis, although relatively uncommon, is a known adverse event associated with many live, attenuated smallpox vaccines. Although smallpox vaccination ceased globally in 1980, vaccine manufacture has resumed in response to concerns over the possible use of smallpox virus as an agent of bioterrorism. To better support the production of safer smallpox vaccines, we previously reported the development of a mouse model in which a relatively attenuated vaccine strain (Dryvax) could be discerned from a more virulent laboratory strain (WR). Here we have further tested the performanc

Suppression of ongoing disease in a nonhuman primate model of multiple sclerosis by a human-anti-human IL-12p40 antibody.

IL-12p40 is a shared subunit of two cytokines with overlapping activities in the induction of autoreactive Th1 cells and therefore a potential target of therapy in Th1-mediated diseases. We have examined whether ongoing disease in a nonhuman primate model of multiple sclerosis (MS) can be suppressed with a new human IgG1kappa Ab against human IL-12p40. Lesions developing in the brain white matter were visualized and characterized with standard magnetic resonance imaging techniques. To reflect the treatment of MS patients, treatment with the Ab was initiated after active brain white matter lesi

Evaluation of a Western Equine Encephalitis recombinant E1 protein for protective immunity and diagnostics.

The E1 and E2 glycoproteins of Western Equine Encephalitis (WEE) are candidate antigens for WEE subunit vaccine development. We have cloned the E1 gene of WEE virus and expressed it in Escherichia coli as inclusion bodies. The inclusion bodies were successfully solubilised, refolded and the immunogenicity of this unglycosylated protein was assessed in mice. Immunization of mice with recombinant E1 protein generated both humoral and cell-mediated immune responses, indicating the recombinant E1 protein is immunogenic. Challenge of E1-immunized mice with live WEE virus demonstrated little or no p

A wild-type porcine encephalomyocarditis virus containing a short poly(C) tract is pathogenic to mice, pigs, and cynomolgus macaques.

Previous studies using wild-type Encephalomyocarditis virus (EMCV) and Mengo virus, which have long poly(C) tracts (61 to 146 C's) at the 5' nontranslated region of the genome, and variants of these viruses genetically engineered to truncate or substitute the poly(C) tracts have produced conflicting data on the role of the poly(C) tract in the virulence of these viruses. Analysis of the nucleotide sequence of an EMCV strain isolated from an aborted swine fetus (EMCV 30/87) revealed that the virus had a poly(C) tract that was 7- to 10-fold shorter than the poly(C) tracts of other EMCV strains a

Eastern equine encephalitis (EEE): a description of the 1989 outbreak, recent epidemiologic trends, and the association of rainfall with EEE occurrence.

An Eastern equine encephalitis (EEE) outbreak in 1989 led to nine human and 196 equine cases, chiefly in coastal Atlantic and Gulf Coast counties. In the past two decades, EEE age-specific incidence and mortality rates have declined compared with earlier years. Analysis of rainfall patterns in areas where human EEE cases occurred between 1983 and 1989 revealed an association between occurrence of human cases and excess rainfall. The association was stronger with data from local weather stations than from statewide rainfall averages and the predictive models were best when applied to northern s

Infections of the central nervous system.

Several discussions of the neuroimaging of inflammatory diseases with emphasis on computerized tomography (CT) and magnetic resonance imaging (MRI) findings and on rare diseases, such as sparganosis, neuroborreliosis and various types of encephalitis, have been published during the past year. Infectious processes in immunosuppressed patients have been described by many authors, especially in the field of acquired immune deficiency syndrome (AIDS). The importance of gadolinium-diethylenetriamine-penta-acetic acid (Gd-DTPA) in MRI for delineation, and differential diagnosis of the inflammatory p

Serum epidemiology of eastern, western and Venezuelan equine encephalitides in Mexico.

The presence of HI antibodies for the Eastern, Western and Venezuelan equine encephalitis was investigated in 16,405 human sera collected in 47 communities in the Mexican Republic. It was found that there is a high percentage of individuals who have antibodies against Venezuelan equine encephalitis. In turn, it was discovered that persons with Western equine encephalitis virus antibodies live in the northwestern states of the country. Very few cases with Eastern equine encephalitis antibodies were detected.

Arboviruses in New York State: an attempt to determine the role of arboviruses in patients with viral encephalitis and meningitis.

In a reveiw of 2,963 patients with signs of infections of the central nervous system in New York State in 1966--1977, arboviruses were found to be the confirmed or presumptive etiologic agents in 60 patients. California encephalitis (CE) virus was the most common (44 patients), followed by Powassan (POW) virus (8), St. Louis encephalitis virus (7), and eastern equine encephalitis (EEE) virus (1). Most patients (47) were children. The incidence of encephalitis was higher in patients with arbovirus findings than in infections with any other of the common neurotropic viruses. The disease was fata

Transovarial transmission of arboviruses by mosquitoes: a review.

One interesting aspect of mosquito-borne arboviruses is their endemic persistence, espeically during adverse climatic conditions in the temperate zones. The hypothesis that transovarial transmission of alpha- and flaviviruses by their respective mosquito vectors could serve as an overwintering mechanism has been proposed; however, experimental data in support of this hypothesis are not very convincing. Recently, some of the California encephalitis group arboviruses have been shown to be transmitted transovarially by Aedes species mosquitoes, and studies indicate that this mechanism contributes
