Sweet's syndrome, also referred to as erythema sweetobullosum, is a rare dermatologic condition that may serve as a clinical diagnostic clue in the emergency department (ED) for coccidioidomycosis, commonly known as Valley fever. This case describes a patient who presented with the acute onset of painful bullous lesions on the forearms, chest, and anterior legs. Recognition of these cutaneous findings as Sweet's syndrome, in conjunction with a high index of suspicion for Valley fever, led to the initiation of appropriate treatment with fluconazole and prednisone prior to the return of confirma…

Yellow fever (YF) is an arbovirus caused by a flavivirus. In 90% of cases, it has a benign course with few symptoms. However, a few cases evolve with a moderate or severe form of high mortality. The severe disease usually starts with minor symptoms, with a period of apparent improvement that progresses to toxemia. Yellow fever liver failure is characterized by jaundice, albuminuria, and bleeding. Very high levels of bilirubin and aminotransferases appear to be associated with a worse prognosis. Treatment is usually nonspecific, based on clinical support for complications. However, recently, st…

In Brazil, yellow fever is endemic in the Amazon region, but recently it was responsible for outbreaks in other regions of the country, becoming a serious public health problem. Vaccination is the most effective way to prevent and control the disease. Bio-Manguinhos is the world's largest producer of the attenuated yellow fever vaccine, which is highly effective and safe. However, the occurrence of rare serious adverse events related to the vaccine boosted the need to develop a purified inactivated vaccine for yellow fever that could be administered to the elderly, pregnant and lactating women…

Dengue fever (DF), which is caused by the dengue virus (DENV) and transmitted through Aedes mosquitoes, is well recognized for its systemic manifestations, with its ocular involvement gaining recent attention. We present a case of a 41-year-old Taiwanese female who developed acute macular neuroretinopathy (AMN) following a DF diagnosis related to DENV-1, emphasizing the need for awareness of this complication. The patient, with a history of completely resolved optic neuritis (ON) and comorbidities, experienced blurred vision on day 10 after the onset of DF. The ophthalmic examination revealed …

African Swine Fever (ASF) is a viral disease affecting both wild and domestic swine, with the potential for major lethality rates. In addition to direct losses for producers, its notification in a free country or zone leads to international trade restrictions. The disease has spread globally at concerning levels, with outbreaks reported in recent years across five continents. Time series analysis of ASF outbreak notifications indicates an increasing trend in Europe and Asia. For Europe, including both domestic pigs and wild boar, seasonality was pronounced in the summer and autumn (July, Augus…

BACKGROUND: Recurrent tonsillitis is a common indication for tonsillectomy in children and has phenotypic overlap with periodic fever, aphthous stomatitis, pharyngitis, and cervical adenitis (PFAPA) syndrome. We sought to characterize symptoms associated with PFAPA among children undergoing tonsillectomy.
METHODS: Parents/guardians of children undergoing tonsillectomy at Vanderbilt Children's Hospital over a six-week period were queried regarding symptoms of recurrent fever. Follow-up questionnaires were administered 3 and 12 months after tonsillectomy.
RESULTS: 82% (120/147) of patients who…

Subacute thyroiditis (SAT) is an inflammatory disease of the thyroid gland. It can present with overlapping features of other etiology of thyroiditis. It can present with thyroid enlargement and systemic symptoms such as fever as well as neck pain and may be confused with infectious thyroiditis. It can be difficult to diagnose and present as fever of unknown origin (FUO). A good history, physical examination, laboratory investigation, as well as imaging may aid in the correct diagnosis and prevent the inappropriate use of antibiotics. Treatment is usually with nonsteroidal anti-inflammatory dr…

Annual outbreaks of Lassa fever have resulted in a public health threat in Nigeria and other endemic countries in Sub-Saharan Africa. While the Lassa Virus (LASV) is endemic in rodent populations, zoonotic spillover to humans causes annual outbreaks. This study reviewed the burden of Lassa fever (LF) in Nigeria between 2020 and 2023 and conducted a cross-sectional survey of Nigerians to evaluate their risk perceptions of LF. During the period under review, 28,780 suspected and 4,036 confirmed cases of LF were reported from 34 of the 37 states of Nigeria. These cases resulted in 762 deaths (a C…

Rift Valley fever virus (RVFV) is an important mosquito-borne virus that can cause severe disease manifestations in humans including ocular damage, vision loss, late-onset encephalitis, and hemorrhagic fever. In ruminants, RVFV can cause high mortality rates in young animals and high rates of abortion in pregnant animals resulting in an enormous negative impact on the economy of affected regions. To date, no licensed vaccines in humans or anti-RVFV therapeutics for animal or human use are available. The development of reverse genetics has facilitated the generation of recombinant infectious vi…

Rift Valley fever (RVF) in ungulates and humans is caused by a mosquito-borne RVF phlebovirus (RVFV). Live attenuated vaccines are used in livestock (sheep and cattle) to control RVF in endemic regions during outbreaks. The ability of two or more different RVFV strains to reassort when co-infecting a host cell is a significant veterinary and public health concern due to the potential emergence of newly reassorted viruses, since reassortment of RVFVs has been documented in nature and in experimental infection studies. Due to the very limited information regarding the frequency and dynamics of R…

Rift Valley fever virus (RVFV) is a globally important mosquito-borne virus that can also be directly transmitted via aerosolization of body fluids from infected animals. RVFV outbreaks cause mass mortality of young livestock and abortions in animals. In most severe human cases, the disease can progress to hemorrhagic fever and encephalitis, leading to death. RVF has a significant economic impact due to the loss of livestock that is a great challenge for people who depend on animals for income and food. Several vaccines are available for animal use, but none are yet licensed for use in human p…

Query (Q) fever is a worldwide infectious disease with acute and chronic manifestations caused by Coxiella burnetii. The clinical manifestations are so variable that the disease is often only diagnosed if systematically considered as a differential diagnosis. Here, we present a case of a 39-year-old man who lived in a countryside house, with cattle and sheep in his field, with acute Q fever hepatitis with the typical granulomatous arrangement in the liver biopsy. The diagnosis was confirmed by polymerase chain reaction (PCR) assay in a serum sample and the presence of phase II antibodies. Anti…

Q fever is frequently associated with the development of antiphospholipid antibodies though rarely causes thromboses. A 44-year-old man presented with diarrhea and fevers and was found to have a splenic infarct. Infectious work-up revealed acute Q fever as well as high anticardiolipin antibody titers. He was treated with doxycycline and hydroxychloroquine and suffered no further thromboembolic complications. The optimal management of thromboembolic complications is uncertain given the rarity of documented cases. However, the presence of these antibodies has been associated with increased risk …

The present work reviews the epidemiologic situation of Anaplasma marginale and Babesia spp. infections and the occurrence of cattle tick fever outbreaks in Brazil. In areas of tick fever enzootic instability, environmental conditions interfere with the development of Rhipicephalus (Boophilus) microplus: chilly winter in the southern region, floods in the Pantanal, and low humidity in the Caatinga. In contrast, the climatic conditions of stable zones (Cerrado, Amazon and Atlantic Forest biomes) favor tick development. In enzootic areas, tick fever is uncommon because the animals are in frequen…

Dengue fever disease (DFD) which is caused by four antigenically distinct dengue viruses (DENV) presents a global health threat, with tropical and subtropical regions at a greater risk. The paucity of epidemiological data on dengue in West African subregion endangers efforts geared toward disease control and prevention. A systematic search of DFD prevalence, incidence, and DENV-infected Aedes in West Africa was conducted in PubMed, Scopus, African Index Medicus, and Google Scholar in line with the Preferred Reporting Items for Systematic reviews and Meta-analyses (PRISMA) guidelines. A total o…
