This article focuses on the lateral arm free flap as the option that comes closest to meeting the diverse aesthetic and functional goals of phalloplasty. The author evaluates the results of extension of 11 humeri in 10 children and adolescents with an average age of 12.5 years. The most frequent cause of the abbreviation was neonatal osteomyelitis. Fifteen years after their forearm bone mineral content was measured, 366 women were measured again with the same single photon technique. 96 of the women had sustained one or more fragility fractures during that period. The initial bone mineral content was less in those women who were to have fractures. The rate of loss over the years did not differ between fracture and non-fracture women--the initial bone mass was the better predictor. Peak bone mass in the women in this study occurred before the age of 40.

A case is described of reflex sympathetic dystrophy (RSD) of the upper limb following cerebral arteriography via the subclavian artery. The pain started in the hand and forearm but, over several weeks, spread to involve the whole arm. After identifying the ulnar nerve in the axillary bundle with a stimulator, a series of small volume injections (bupivacaine 0.5% with epinephrine 2.5 ml and methyl-prednisolone 40 mg) was given. Fractures of the radial shaft associated with disruption of the distal ulnar radial joint (Galeazzi fracture) account for a small percentage of forearm fractures in adults but occur even more rarely in children. An equivalent injury in children is a fracture of the radius with separation of the distal ulnar physis in the form of a Salter-Harris type II epiphyseal fracture. Although this lesion has been mentioned in the literature, to our knowledge it has not been described in detail. We report three cases of this fracture. All cases were treated by open reduction internal fixation.

A follow-up study of 219 patients infected with parasitologically confirmed cutaneous leishmaniasis in French Guiana was made between 1981 and 1987. Cutaneous leishmaniasis appeared to be common in young male adults entering the forest for professional activities (84.2% of the cases). The lesions were generally of the classical ulcerative type. They were limited to the skin and preferentially located on the legs and forearms (20.7% and 19.8% respectively). Most of the cases (86.6%) represented primary infections, but 6.8% had a recurrent lesion at the site of an old, previously cured lesion.

This study was designed to determine the forearm exchange of glucose and potassium in four members of a family exhibiting acanthosis nigricans and insulin resistance. Four unrelated patients are reported with short stature, stiffness of the joints, short fingers, inability to make a fist, and thickened skin on the forearms. Investigations have failed to show a lysosomal storage disorder and radiographs show non-specific changes with a delayed carpal bone age. The clinical features in the four children are very similar to the recently described acromicric dysplasia. There are also similarities to Moore-Federman syndrome which has only been described in one family. The case is made that acromicric dysplasia and Moore-Federman syndrome are the same entity.

Twenty-three patients with an isolated fracture of the ulnar shaft were treated by functional bracing as proposed by Sarmiento. Complete functional recovery was achieved quickly in all cases. No pseudarthroses occurred. In those cases in which the ulna had healed with some angulation, no clear restriction of rotation of the forearm was observed. The patients tolerated the treatment well. It is concluded that functional bracing of isolated fractures of the ulnar shaft is an elegant method of treatment with a minimal risk of complications. We report on a case of a 45 years old man suffering from ulcerative colitis accompanied by a protein-loosing-syndrome (hypoalbuminaemia). The significant decrease of cholinesterase (CHE) and quick level, edemas of the legs and forearms, pleural effusion and ascites had been misinterpreted as signs of liver insufficiency. Low protein diet was given and, thus, the full picture of kwashiorkor developed soon as a logical consequence. Liver-induced fluid dysbalance was assumed and the low-protein diet continued. We describe the difficulties of treatment and the favourable outcome of the disease.

Children with spastic diplegia, hemiparetic and hyperkinetic forms of infantile cerebral paralysis were treated with local cryoapplications onto the hands. Nine patients with tremor on writing and one patient with tremor only on swinging a golf club were investigated. None of the patients had any other neurological symptoms or signs. The frequency of the tremor ranged from 5 to 6 Hz. Rapid passive supination or pronation of the forearm by a torque motor evoked a short burst of alternating tremor in seven patients. The tremor was improved by alcohol or propranolol in six patients. These characteristics of writing tremor (and of other isolated action tremors) suggest that it is a variant of benign essential tremor.

Secondary hyperparathyroidism is one of the major complications encountered in patients on maintenance dialysis. Medical management is frequently unsuccessful, and there is often a need to perform a parathyroidectomy. We present a technique of providing complete skin cover for total degloving injuries of the hand and distal third of the forearm. This involves the use of a Y-shaped hypogastric groin flap. The proximal portion of the Y is tubed allowing the distal portion of each flap to be applied to the dorsal and volar surfaces respectively. This technique allows complete skin cover of both surfaces and yet retains the advantages of the groin flap in that it still has a long pedicle allowing the hand to be put through a full range of motion, speeding rehabilitation.

An ornamental succulent plant sold in many plant stores in the Chicago area has been identified as Euphorbia hermentiana Lem. Open and closed patch testing using undiluted latex from this species was performed on five Caucasian volunteers. Open testing on flexor forearms resulted in irritant follicular dermatitis, while closed testing to the flexor surfaces of both upper arms in each subject produced bullae and vesiculation with residual desquamation and hyperpigmentation. Dermatological signs persisted for over a week following latex application.
