Superficial radial nerve damage due to Kirschner wiring of the radius.

For displaced fractures of the radius, the use of Kirschner wires (K Wires) is accepted practice either alone or to supplement external fixation. Complications related to K wires include infection, migration and damage to tendons and nerves. We set out to investigate to incidence of superficial radial nerve damage due to radial styloid K wires. Injury to the sensory branch of the superficial radial nerve was seen in eight (20%) out of the 40 patients. K wiring is a popular technique to help maintain anatomic reduction of distal radial fractures. It has the advantage of being a semi-closed proc

Intrafocal pinning for juxtaarticular phalanx fractures.

Juxtaarticular phalanx fractures can present a challenge to the treating physician. Because they are not a common occurrence, we wanted to discuss our treatment protocol for this entity. Goals of treatment include anatomic realignment, fracture stability, and early range of motion. Improper treatment can lead to malunion resulting in deformity or loss of function as well as joint stiffness. Other treatment modalities can also result in unsatisfactory results including decreased range of motion. Intrafocal pinning provides a treatment alternative for the irreducible fracture normally requiring

Compartment pressures after submuscular fixation of proximal tibia fractures.

INTRODUCTION: Tibial fractures are known to be associated with elevated compartment pressures, which may potentially result in a compartment syndrome. The Tibial Less Invasive Stabilization System (LISS PLT) is an internal fixator device that is designed for fixation of proximal tibia fractures. The technique includes submuscular placement of the implant along the lateral border of the tibia. Such placement of the Tibia LISS into the anterior leg compartment could increase compartment pressures, and therefore increase the risk of compartment syndrome. The objective of this prospective study is

Periprosthetic fractures of the femur.

Periprosthetic fractures of the femur represent a heterogeneous and challenging problem for the orthopedic surgeon. The incidence of these fractures is dramatically increasing, as there are more and more patients with aging total joint replacements. The fractures may occur as the result of a traumatic event, but more often are the result of minor trauma spontaneous fracture, and they are frequently associated with preexisting, sometimes neglected, problems with the associated joint replacement.

Operative treatment of a unilateral bicondylar fracture of the femur.

A case of isolated tangential fracture of the dorsal part of both condyles of the femur (bicondylar Hoffa fracture) is described. After open reduction, four lag screws were introduced through the fractured condyles to fix the fragments. A plaster cast was applied for a period of four weeks. Full weight-bearing was authorized after six weeks. Full range of motion of the knee was observed as early as after three months, as well as after twenty-one months. The mechanism and therapeutic implications are discussed and the literature is reviewed.

Vertical shear fractures of the femoral neck. A biomechanical study.

A biomechanical cadaver study was performed to compare the strength and stability of three cannulated cancellous lag screws with a sliding hip screw for fixation of a vertically oriented fracture of the femoral neck (Pauwels Type III). Using eight matched pairs of human cadaveric femurs, vertically oriented femoral neck osteotomies were created, reduced, and randomized to one of the two fixation methods. The constructs were tested with incremental axial loading from 100 N to 1200 N and cyclical loading at 1000 N for 10,000 cycles; fracture displacements and ultimate load to failure were determ

The mechanical effect of blocking screws ("Poller screws") in stabilizing tibia fractures with short proximal or distal fragments after insertion of small-diameter intramedullary nails.

OBJECTIVES/HYPOTHESIS: To evaluate the mechanical effects of medial and lateral blocking screws in supplementing intramedullary nail fixation of high proximal and low distal tibial fractures treated with small-diameter intramedullary nails. STUDY DESIGN: Intact fresh human cadaveric tibiae were sectioned to provide ten distal segments measuring seventy millimeters and ten proximal segments measuring ninety millimeters. In the distal segments, stainless steel solid eight-millimeter tibial nails were advanced to eight millimeters from the ankle joint. Two transverse and one anterior-posterior (

[Rib fractures and flail chest in closed chest trauma--stabilizing operations and the results].

A contingent of patients presenting closed chest injuries, sustained over a 12-year period (1985-1996), are analyzed. Of the total of 6938 traumas, chest injuries amount to 3286 (47.06 per cent) of which 2842 (40.7 per cent)-closed. Of the latter 2248 (79.09 per cent) are located in the left thoracic half, 420 (14.77 per cent)-in the right thoracic half, and in 174 (6.12 per cent) it is a matter of bilateral involvement. At the point of heaviest impact, involving 5th to 9th rib segment, left side injury is recorded in 2034 cases (71.56 per cent), right side--408 (14.35 per cent), and bilateral

Tarsometatarsal (Lisfranc's) fracture-dislocation.

Tarsometatarsal (Lisfranc's) fracture-dislocations account for 0.2% of all fractures. As many as 20% of these injuries are misdiagnosed or overlooked, especially in the polytrauma patient with other distracting injuries. The mechanism of injury, clinical diagnosis, treatment, and prognosis are reviewed. Results of arthrodesis of the tarsometatarsal joint for patients with symptomatic posttraumatic arthritis of the tarsometatarsal joint are discussed. A thorough understanding of this often misdiagnosed injury should facilitate the physician's prompt diagnosis and management of this potentially

[Osteomyelitis after malleolar fractures].

Over a ten-year period, 485 patients suffering from malleous fracture were operated at the Department for Orthopaedic Surgery and Traumatology in Novi Sad. Postoperative infection appeared in ten cases (2.06%). Eight of these patients primarily had open fractures, and two of them had closed ones. Osteosynthesis of the lateral malleous had most frequently been treated by the use of panel and screws. Staphylococcus aureus was isolated as symptom of the infection in eight cases. In treatment of malleous osteomyelitis, the debridement of focus of infection together with fixation material extractio

Closed Ender nailing of femur fractures in older children.

Twenty-eight patients ranging from 8 to 13 years old were treated at Childrens Hospital of Alabama for diaphyseal femur fractures. We studied two groups: 14 patients (Group I) were treated by conventional 90-90 balanced skeletal traction with late spica cast application; 14 patients (Group II) with multiple associated injuries or hyperkinetic problems were treated with closed retrograde Ender nailing. Groups I and II compared closely with regard to age, sex, fracture type and location, and mechanism of injury. The alignment, leg length, length of hospitalization, and length of therapy were ana

The heel of achilles: calcaneal avulsion fracture from a gunshot wound.

Greek mythology relates that the legendary warrior Achilles was made invincible by his mother Thetis, who dipped him in the River Styx while holding him by his heel. Because his heel was never immersed, it remained his one area of vulnerability. After the fall of Troy, Achilles met his demise when he was shot in the heel by Paris, whose arrow was guided by the Greek god Apollo. This is the derivation of the term "Achilles tendon." Avulsion fractures of the tuberosity of the calcaneus are rare injuries. Schonbauer reviewed a series of 870,000 accident cases treated at the Vienna Trauma Hospital

Operative treatment of Medial epicondyle fractures in children.

Thirty-one patients with fractures of the medial epicondyle displaced more than 2 mm were evaluated an average of four years after their injury. Twenty-three patients had good results regardless of the degree of displacement or the presence of an elbow dislocation. One patient, had a poor result due to a technical error in pin placement. The remaining seven patients had the fracture fragment trapped in the joint and did worse, with three poor results. There was no correlation between range of motion and degree of displacement, length of immobilization, time from injury to surgery, presence of
