CASE SUMMARY: This report describes the appearance of facial nerve paralysis in a 16-year-old hypertensive cat. MRI was helpful in visualising and characterising mesencephalic and facial nerve lesions thought to be induced by hypertension. Neurological signs rapidly resolved under antihypertensive therapy.
RELEVANCE AND NOVEL INFORMATION: Systemic hypertension is an important medical condition in geriatric cats causing damage in various target organs, including the brain. Hypertensive encephalopathy is an umbrella term for a multitude of different clinical manifestations of cerebral target or

Mortality data for Magellanic penguins (Spheniscus magellanicus) housed in zoos and aquariums in the United States has not previously been published. Necropsy and histopathology records were examined for Magellanic penguins housed at 12 Association of Zoos and Aquariums institutions from 2008 through 2018. If birds lived through the first year, the mean longevity was found to be 18.9 years of age (standard deviation: 7.9). Prefledge chicks and geriatric penguins experienced the highest mortality rates. Aspergillosis was a major cause of death in this species. There was no significant differenc

In the older population, especially the hospitalized patients who are prone to dehydration and hypovolemia, orthostatic hypotension (OH) presents as a debilitating disease. How different pharmacological and non-pharmacological interventions affect the incapacitating symptoms (falls and episodes of syncope), morbidity, and mortality related to OH has become a topic of debate. OH can predispose to ischemic heart disease (IHD). A non-pharmacological approach consisting of mobilization, early lifestyle changes, and therapeutic maneuvers is the first choice in the management of these patients. Indi

Over a short period of one month we undertook a systematic review of all medications of our residents in the nursing home. Prior to this we examined the patients with great care and where necessary, laboratory tests or other further investigations took place. By the end of this procedure, we analysed the medications prescribed and on the recommendations of the program Stopp and Start, several adaptations were done. In summary, some medications increased, mainly for osteoporosis prevention and some other could be reduced, mainly the psychotropic drugs. A systematic review for patients in nursin

An 81-year-old woman presented with cachexia, malaise, deranged liver function tests (LFTs), hepatomegaly and hyperpigmentation. Hereditary haemochromatosis (HH) was ultimately diagnosed on genetic testing. Haemochromatosis is a difficult diagnosis in older people because not only are the symptoms protean and occur commonly in other geriatric conditions, but HH is also considered an unlikely new diagnosis in the over 80s; it is generally felt to present in the 5th decade for men or 6th decade for women. However, older people can present with only mild disease, so diagnosis is important not jus

Men are playing greater roles in the provision of care for older adults with chronic health conditions. Husbands, in particular, encounter many role transformations as they witness their wives grow in levels of dependence as a result of their illnesses. This qualitative study examines the changes that occurred in the roles, responsibilities, and relationships of 17 husbands who were providing care for their wives with memory loss and other chronic health conditions. The results suggest that husbands experience changes in the ways that they adapt their marital roles to the new roles they assume

The assessment and treatment of urinary incontinence in aged women have age-related specificities that we present from a literature review. Urinary incontinence is most often mixed: stress incontinence and urge incontinence. The coexistence of chronic retention is common and should be taken into account in treatment. Urinary incontinence de novo must find a trigger that will be corrected if possible. The treatment of urinary incontinence in aged women and young women are the same as long as no frailty is present. The definition of frailty is not consensual, but some criteria are regularly cite

Tension-type headache (TTH) is a common disease that is usually minimally impairing. However, more chronic TTH can be associated with significant disability and psychological comorbidity. Psychological assessment can help elucidate the biopsychosocial issues that complicate TTH and reveal the patient's coping styles. Psychological therapies, such as relaxation training, cognitive behavioral therapy, and biofeedback, may be helpful in reducing the frequency of TTHs and decrease the associated disability. Special populations, including pediatric patients, pregnant patients, and geriatric patient

Anxiety disorders affect more than 19 million American adults and are prevalent in the geriatric population. Generalized anxiety disorder, panic disorder, and other anxiety types can affect quality of life, precipitate social withdrawal, interfere with self-care, and exacerbate health problems. Anxiety disorders also may be associated with physical or mental disorders such as heart disease and depression. Patients may volunteer information about some anxiety disorders and be guarded about others. A proactive patient work-up can help reveal anxiety disorders in the primary care setting. Effecti

Two cases of sudden death due to perforation of a benign oesophageal ulcer into a major blood vessel are reported. In one man, anaemia and aspiration pneumonitis dominated the clinical picture. He had an oesophageal stricture and a chronic peptic ulcer associated with an incarcerated hiatus hernia. Death was due to haemorrhage caused by perforation of the ulcer into the thoracic aorta. The second patient presented with confusion and falls, backache and indigestion. She had a hiatus hernia and a large benign chronic oesophageal ulcer. Death was due to perforation of the ulcer into the left pulm

The free thyroxine index (FTI) was measured in 307 elderly patients admitted to two geriatric units. The initial FTI was abnormal in 41 (13.3%), being raised in 27 and low in 14 patients. On further testing, these abnormalities were found to be either transient or of no clinical significance in 36 of the 41 patients (88%). Only five of the 307 patients (1.6%) were finally treated for previously undiagnosed thyroid disease and of these three improved (1%). Thyroid disease identified by routine screening, and not suspected from the history and clinical findings, was present in only two patients

Patients (N = 36) on a geropsychiatric acute care unit were administered the Wechsler Memory Scale (WMS) at admission and discharge. The results indicated that there were significant increases in WMS scores from admission to discharge. Mean scores on the WMS were generally below those previously reported for normal geriatric groups, but above those reported for chronically ill or demented groups. As in previous studies, a significant positive relationship was found between education and WMS score, although no such relationship was found between age and WMS score. Overall, the results highlight

Calcification of the mitral ring and of the aortic valve cusps was studied in an unselected autopsy series of 219 patients over 60, dying in a department of geriatric medicine. Possible correlations with age, sex, heart size, systolic and diastolic blood pressures, bone density and serum calcium, phosphate and alkaline phosphatase were examined. There were no significant findings for aortic valve calcification though there was a weak association with advanced age. Mitral ring calcification was significantly associated with higher age, female sex and with increased heart weight after correction

The records of 59 patients above the age of 65 operated on for diverse neurosurgical problems between January 1975 and September 1976 were reviewed. It was found that in only about half of the cases was the referral diagnosis correct. Most of the referred patients diagnosed as cerebrovascular accident or senile dementia were found to be suffering from intracranial space-occupying lesions or normal pressure hydrocephalus. Post-operatively there was a significant improvement in 48% of this group of geriatric patients, with no change in 23% and a poor result in 29%. when the data on patients with

This study explored how engagement with older adults promotes the development of cultural humility in prelicensure nurses. Using a descriptive phenomenological approach informed by Mezirow's transformative learning theory and Kolb's experiential learning theory, nine nursing students were interviewed following their volunteer experience at an international senior athletic competition. Four themes emerged: reframing aging, patient-centered connection, dismantling power dynamics, and shifting from medicalization to holistic care. Findings suggest that meaningful interactions with older adults fo

Duodenal perforation is a rare but serious complication of endoscopic retrograde cholangiopancreatography (ERCP), particularly when anatomical distortions, such as large hiatal hernias, complicate endoscopic access. This case describes a 93-year-old patient with a type IV hiatal hernia who suffered an iatrogenic duodenal injury during a procedure for acute cholangitis. Due to the size of the defect and the patient's complex anatomy, an emergent laparoscopic repair was performed to reduce the hernia and repair the perforation. The patient's successful recovery highlights that prompt recognition

Lung cancer is a common disease with a high mortality rate. It is often diagnosed at an advanced stage, with a median survival of 12 to 16 months. Immune checkpoint inhibitors are part of the first-line therapeutic armoury in metastatic stages. They have been shown to improve overall survival, including in patients aged 64 to 75, but the results are less clear in older patients, who are underrepresented in clinical trials. Although the management of adverse events associated with these treatments is now well protocolized, the accumulation of toxicities, even low-grade toxicities, can have an a

SummaryGiant cell tumours of bone are benign and locally aggressive tumours that usually occur in young adults and at the epiphysial locations after physeal closure. Occurrence outside of epiphysial locations and appearance in geriatric patients is rare. We report a case of a woman in her late 60s with a giant cell tumour of the mid-shaft of the right tibia. Extended curettage and biological reconstruction were performed with autologous double-barrel fibular struts and tri-cortical iliac crest bone grafting. At the 28-month follow-up examination, we noted full bony union at both ends with succ

Anxiety disorders are the most common psychiatric illness and include disorders such as generalized anxiety disorder (GAD), panic disorder (PD), and social anxiety disorder (SAD). Psychotherapy and pharmacotherapy are both effective treatments for anxiety disorders, with efficacy between 60% and 85%. Selective serotonin reuptake inhibitors and serotonin-norepinephrine reuptake inhibitors are first-line pharmacologic treatment for GAD, PD, and SAD. Recommendations for treating pediatric and geriatric populations vary slightly, but first-line treatments remain the same. Recent advancements in th
