A 16-year-old boy sustained a traumatic posterior dislocation of the right hip. After initial closed reduction, the hip spontaneously redislocated within 2 days. Four days after the initial dislocation, the patient was transferred to the authors' institution, where a radiographic workup showed a severe capsular and labral disruption from the posterior aspect of the acetabulum. Subsequently, the patient underwent operative treatment using the technique of surgical hip dislocation through a digastric osteotomy as described by Ganz. Using this approach, the avulsed capsulolabral complex as well a…

The outcomes of 18 primary or revision total hip (THA) and knee arthroplasties (TKA) in 9 patients with cardiac transplants were reviewed. Primary total joint arthroplasties were performed for osteonecrosis (5 hips) or osteoarthritis (5 hips, 4 knees). There were no infections in any of these patients. Final Harris Hip Scores were 71.8 for patients with osteonecrosis and 88.6 for osteoarthritis. Eight of 10 hips were pain-free at final follow-up. Two of the 10 primary THAs required late revision at 7 and 10 years after the index arthroplasty. One patient (2 hips and 1 knee) had chronic bilater…

Simultaneous bilateral hip arthroplasty has been reported to have varying results in the literature when performed using conventional postoperative care. Unilateral hip and knee arthroplasty as well as bilateral simultaneous knee arthroplasty may be associated with reduced length of stay and low complication rates when performed in a fast-track setting. We believed it would be useful to assess hip arthroplasty in this context, and we report 50 consecutive fast-track simultaneous bilateral hip arthroplasty procedures. The median length of stay was 4 days. Mortality within 90 days was 4% and 8% …

Combined component placement of cup and stem is closely correlated to stability, functionality and wear in total hip replacement (THA). Computer-navigated orthopedic surgery offers a reliable control method for a complex three-dimensional situation. Imageless navigation systems without the need of preoperative or intraoperative image acquisition and exposure to radiation have been proven to increase the accuracy of positioning the acetabular component and measure intraoperative leg length and offset changes precisely. A new development in this field is the noninvasive external femoral referenc…

Reconstructive and salvage procedures have continued to evolve in orthopedic surgery with changing functional demands of the population as w … e gravity of the procedure. As infrequent as it is in today's clinical practice, arthrodesis of the hip joint has a role and remains a solid option for a well selected case. The purpose of this review is to discuss the current indications in the pediatric population and outline surgical techniques for hip arthrodesis while pointing out limitations and …

Psoas impingement is a rare cause of persisting pain after hip arthroplasty. Diagnosis is based on clinical examination and exclusion of other complications after arthroplasty by radiographs, ultrasound, CT and blood analysis. The diagnosis is strongly supported by temporary pain relief after infiltration. When conservative measures fail, treatment can consist of a psoas tenotomy or a revision arthroplasty. When there is no obvious cause such as malpositioning for component impingement, psoas release reliably improves pain and function. This procedure is mostly performed through an open approa…

Resurfacing hip arthroplasty is an alternative to conventional arthroplasty and it is indicated in young and active patients. Good results and the prevention of complications stem from a meticulous surgical technique and proper patient selection. We present herein the case of a 43 year-old patient who, after undergoing bilateral hip replacement with resurfacing prostheses, sustained a non-simultaneous fracture of both femoral necks due to avascular necrosis. He was treated by placing a metaphyseal anchoring stem. Postoperative X-rays showed proper implant placement without femoral notching, wi…

Total hip arthroplasty is an exceptionally cost-effective and successful surgical procedure. Dislocation, infection, osteolysis, and limb-length inequality are among the most common complications affecting the long-term success of total hip arthroplasty. Instability is a challenging complication to treat. The surgeon frequently must try to achieve a stable hip at the cost of increasing the length of the operated extremity. It is important to understand the factors associated with stability and limb length; the surgical options available; the effect and role of the various surgical approaches; …

BACKGROUND: Guillain-Barré syndrome (GBS) is a rare, acute, inflammatory demyelinating polyneuropathy with a presentation of progressive ascending motor weakness of the extremities, which may extend to the respiratory muscles and require mechanical support.
CASE DESCRIPTION: This case describes a healthy male who developed GBS 1 week following otherwise an uncomplicated bilateral total hip arthroplasty surgery. The diagnosis was made based on physical exam significant for bilateral lower extremity weakness and diffuse arreflexia, and confirmed by nerve conduction studies. There were no effect…

PURPOSE: To obtain a better understanding of the issues that influence hip protector use among community-dwelling older adults.
METHODS: The study used a longitudinal, crossover design. A convenience sample of 32 participants used four different brands of hip protectors, each for a 1-week period. Data were collected by weekly telephone interviews and a mailed questionnaire administered at baseline and follow-up. Participant perceptions regarding hip protectors were assessed using both open-ended questions and Likert-type items.
RESULTS: The most common concerns about hip protectors mentioned…

Little has been reported on knee pain after total hip arthroplasty (THA). The purpose of this study was to investigate the incidence and mechanism of knee pain after THA. Two hundred fifty-two patients with hip dysplasia were clinically and radiographically assessed for knee pain before and after THA. Incidences of knee pain and patellofemoral alignment were analyzed with reference to postoperative change in leg length, femoral anteversion, and the femoral offset. Anterior knee pain was present in 16 patients (7.3%). Lateral patellar tilt was increased in all patients with knee pain and signif…

We report a case of synovial hemangioma of the hip causing joint destruction severe enough to warrant arthroplasty in a 32-year-old man who presented with a 2-year history of hip pain and worsening limp. Plain radiographs demonstrated advanced degenerative changes of the hip articular surface. Magnetic resonance imaging revealed changes consistent with pigmented villonodular synovitis throughout the joint and erosion of the acetabular bone. A total hip arthroplasty was performed, and histopathologic examination of the resected tissue demonstrated synovial hemangioma. This case, unique in the l…

BACKGROUND: In most cases, loosening of a prosthesis stem is accompanied by periprosthetic osteolysis. This article presents the short-term clinical and radiological results after revision of total hip arthroplasty (THA) with bone substance deficiencies using a modular noncemented femoral stem system (LINK MP).
PATIENTS AND METHODS: Between March 2003 and November 2005, 91 LINK MP modular revision stems were implanted in 90 patients. Eighty-one patients (51 stem revisions, 24 revisions of stem and shell, and six reimplantations after a Girdlestone situation) were available for prospective cli…

BACKGROUND AND PURPOSE: Variations in hip anatomy limit the femoral canal fit of standard uncemented hip stems. In addition, there are still issues with leg length discrepancy and offset reconstruction, potentially resulting in impingement, dislocation, and wear. Modular stems with different shapes for femoral canal fit and multiple neck options may improve the outcome and reduce complications.
PATIENTS AND METHODS: 173 patients (190 hips) received an uncemented THA with 1 of 2 different stem shapes for canal fit and a modular neck for stature-specific hip reconstruction. Median follow-up tim…

MATERIALS AND METHODS: The postoperative and last follow-up radiograms: 7 to 25 years after surgery (mean: 10.4 +/- 3.9) were evaluated in 123 patients (167 hips). The radiological evaluation was performed according to Hip Society recommendations. The position of the cup and the stem, polyethylene wear, the migration of the cup or stem, radiolucent lines and ectopic ossification were evaluated.
RESULTS: At last follow-up in 17 hips a prior revision was performed and further radiological assessment revealed 16 cases of prosthesis loosening. The retrospective analysis of the postoperative radio…

BACKGROUND: Hip arthroscopy is a common orthopaedic procedure used as a diagnostic and therapeutic tool with a multitude of surgical indications. The complication rate is reportedly between 1.3% and 23.3%. Major complications are related to traction, fluid extravasation, and iatrogenic chondral injury. Although osteonecrosis is a concern with any surgical procedure about the hip, this complication has been primarily a theoretical concern with hip arthroscopy.
CASE DESCRIPTION: We report the case of a 24-year-old man who presented with a 2-year history of left hip pain. He underwent hip arthro…

BACKGROUND AND PURPOSE: Primary and dynamically maintained periprosthetic bone formation is essential for osseointegration of hip implants to host bone. Bone morphogenetic proteins (BMPs) play a role in osteoinductive bone formation. We hypothesized that there is an increased local synthesis of BMPs in the synovial membrane-like interface around aseptically loosened total hip replacement (THR) implants, as body attempts to generate or maintain implant fixation.
PATIENTS AND METHODS: We compared synovial membrane-like interface tissue from revised total hip replacements (rTHR, n = 9) to osteoa…

Clinical cases in mineral and bone metabolism : the official journal of the Italian Society of Osteoporosis, Mineral Metabolism, and Skeletal Diseases2010eng
Osteoporotic bone, structurally altered because of reduction of bone mineral density and quality deterioration, can easily head for fracture after minimum mechanical stress.THE MOST COMMON SITES OF FRACTURE, OTHER THAN SPINE AND HIP, ARE, IN DECREASING ORDER: distal radius, forearm, proximal humerus, other femoral sites, ribs, pelvis, tibia and fibula, metatarsal bone and calcaneum. The role of diagnostic imaging is essential in detecting fractures for their immediate and correct assessment, which is necessary to the planning of treatment, whether conservative or surgical. Imaging is also impo…
