Treatment of bilateral choanal atresia in the premature infant.

An innovative technique for choanal atresia repair was used for a unique case of bilateral choanal atresia in a 28-week premature newborn. Endoscopic transnasal KTP laser was employed as an alternative to standard instrumentation to reach the very small atretic plate of a premature nose, providing good visualization of the operative field, thereby avoiding damage to the adjacent structures. The successful repair allowed for early extubation and avoidance of a tracheotomy. The clinical and surgical aspects of the case are discussed. With the likelihood of encountering more premature infants wit

The apparent role of blood transfusions in the development of retinopathy of prematurity.

During a 30-month-period, 184 very low birth weight infants from two Liverpool neonatal intensive care units were screened for evidence of retinopathy of prematurity (ROP). Seventeen clinical variables previously considered relevant to the development of ROP, blood gas and blood pressure data over the first 7 days, and the maximum stage of ROP reached in either eye were recorded, together with the need for cryotherapy and current visual status. Ninety-two infants developed any stage of ROP and 15 required cryotherapy or became blind. Logistic regression showed that only gestational age and fre

[Initial experience with monoclonal antibodies in therapy of life threatening nosocomial infections in the neonatal period].

Despite the use of modern broad spectrum antibiotics nosocomial infections are an unsolved problem, especially in the field of neonatal intensive care (preterm babies and newborns). In patients with septic shock human monoclonal antibodies in combination with appropriate antibiotics have proven effective and compatible for children older than one year. So far, there have been no reports in the literature on the application of such kind of antiendotoxin immunotherapy for pre-term babies and newborns. We describe the effectiveness of monoclonal antibodies in two newborns. Already 12 respectively

The clinical course in the periventricular leukomalacia complex.

Intraventricular hemorrhage (IVH) and classical periventricular leukomalacia complex are considered the two most common forms of perinatal anoxic-ischemic brain injury. However, recently, a third entity, the periventricular leukomalacia complex (PLC) was described and was seen in 31 percent of 61 premature infants coming to autopsy from the University of Connecticut Neonatal Intensive Care Unit (NBIC) and in several other centers. Periventricular leukomalacia complex consists of necrotizing lesions of the periventricular white and grey matter, hippocampus and subiculum, cerebellum and basis po

Sonography of periventricular leukomalacia.

Periventricular leukomalacia (PVL) is a pathological entity characterized by ischemic changes in areas adjacent to the external angles of the lateral ventricles. It is associated with serious neurodevelopmental sequelae, especially spastic diplegia. With the increased use of ultrasound in the Neonatal Intensive Care Unit, changes consistent with PVL are being noted. Hemorrhagic and nonhemorrhagic forms of this condition exist, both of which may appear sonographically as increased periventricular echogenicity. Two cases of PVL are described; one of them was associated with ventricular dilatatio

Congenital diaphragmatic hernia.

Congenital diaphragmatic hernia continues to be a critical problem in neonatal surgery. Despite the apparent simplicity of the anatomic defect, the physiology is complex, and survival remains uncertain. Surgical success has been achieved, but we recognize that the barrier to survival is pulmonary parenchymal and vascular hypoplasia as well as the complex syndrome of persistent fetal circulation. In many ways the problem of diaphragmatic hernia is as much of an enigma to today's physician-scientist as it was to Bochdalek in the nineteenth century. The treatment of respiratory distress after rep

Neonatal surgery in clubfoot.

Neonatal surgery for clubfoot is conducted according to the following rules: One must work as close to the lesions as possible. It is important to avoid going beyond abnormal tissues. Dissection should be precise and economical. Opening of Lisfranc's joint or sectioning of the medial ligament must be avoided. One must operate with maximal accuracy. All areas of fibrosis must be excised, but care must be taken not to enter the interarticular space. If a bone deformity is present, one must accept an incomplete correction. One must act as soon as possible. This is the inverse of classic managemen

The impact of prematurity and neonatal complications on hearing function in children born to HIV-Positive mothers: A secondary data analysis in the South African context.

BACKGROUND: Children born to HIV-positive mothers are at increased risk for a range of neonatal complications that may negatively impact auditory development. In low- and middle-income countries (LMICs) like South Africa, where both HIV exposure and adverse neonatal outcomes are prevalent, understanding the relationship between early clinical risk factors and hearing loss is critical for informing early hearing detection and intervention (EHDI) strategies. METHODS: This study aimed to examine the association between neonatal complications-such as prematurity, low birth weight, hyperbilirubine

Outcomes in reported penicillin allergic mothers and neonates requiring Group B streptococcal prophylaxis: a retrospective observational cohort study.

BACKGROUND: Infectious morbidity and mortality in the first week of life is commonly caused by early-onset neonatal Group B streptococcus (GBS) disease. This infection is spread from GBS positive mothers to neonates by vertical transmission during delivery and results in serious illness for newborns. Intrapartum prophylactic antibiotics have decreased the incidence of early-onset neonatal GBS disease by 80%. Patients labeled with a penicillin allergy (PcnA) alternatively receive either vancomycin or clindamycin but effectiveness is controversial. We evaluated the influence of a reported PcnA l

Pharmacokinetics of Antibiotics in Pediatric Intensive Care: Fostering Variability to Attain Precision Medicine.

Children show important developmental and maturational changes, which may contribute greatly to pharmacokinetic (PK) variability observed in pediatric patients. These PK alterations are further enhanced by disease-related, non-maturational factors. Specific to the intensive care setting, such factors include critical illness, inflammatory status, augmented renal clearance (ARC), as well as therapeutic interventions (e.g., extracorporeal organ support systems or whole-body hypothermia [WBH]). This narrative review illustrates the relevance of both maturational and non-maturational changes in ab

Behavioral effects of perinatal opioid exposure.

Opioids are among the world's oldest known drugs used mostly for pain relief, but recreational use is also widespread. A particularly important problem is opioid exposure in females, as their offspring can also be affected. Adverse intrauterine and postnatal environments can affect offspring development and may lead to various disabilities later in life. It is clear that repetitive painful experiences, such as randomly occurring invasive procedures during neonatal intensive care, can permanently alter neuronal and synaptic organization and therefore later behavior. At the same time, analgesic

Prevalence of Group B Streptococcus in pregnant women in Kerala and relation to neonatal outcomes: a prospective cross-sectional study.

BACKGROUND AND OBJECTIVES: Early onset sepsis (EOS) in neonates is a scourge that contributes to morbidity and mortality. Prominent stakeholders recommend universal screening of antenatal women for Group B Streptococcus (GBS) and intrapartum antibiotic prophylaxis (IAP) for those who are carriers. However, there are controversies. Other guidelines allow region-specific protocols due to sociodemographic, geographical and ethnic differences. We planned to analyze the prevalence of GBS rectovaginal carriage at 36-37 weeks gestation and its effect on early neonatal status. METHODS: This prospecti

Modelling mortality within 28 days among preterm infants at a tertiary hospital in Lusaka, Zambia: a retrospective review of hospital-based records.

INTRODUCTION: globally, almost half of all deaths in children under five years of age occur among neonates. We investigated the predictors of mortality within 28 days among preterm infants at a tertiary hospital in Lusaka, Zambia. METHODS: we reviewed admission records linked to birth, mortality, and hospital discharge from 1st January 2018 to 30th September 2019. Information was retrieved with a follow-up period of 28 days post-delivery to discharge/mortality. We used the Weibull hazards regression to establish the best predictor model for mortality among the neonates. RESULTS: a total of 3

Risk factors of meconium-related ileus in very low birth weight infants: patients-control study.

Very low birth weight (VLBW) neonates experience various problems, including meconium-related ileus (MRI). This study investigated the risk factors of MRI and surgical MRI in VLBW infants. VLBW neonates admitted to the Neonatal Intensive Care Unit of Seoul National University Children's Hospital from October 2002 to September 2016 were included in the study. The diagnostic criteria for MRI were a decreased frequency of defecation with intolerable feeding, vomiting, and increased gastric residue (>50%); meconium-filled bowel dilatation in an imaging study; and no evidence of necrotizing enterit

Predictors of Nonelective Removal of Peripherally Inserted Central Catheters in Infants.

BACKGROUND: Nonelective removal of peripherally inserted central catheters (PICCs) due to complications continues to be a major concern in neonatal intensive care units (NICUs) around the world. Nonelective removal results in interruption of intravenous therapy, added costs, stress, and negative impacts on infant's health. Identification of predictors of complications that lead to nonelective removal of PICCs would allow for the initiation of preventive strategies to improve the quality of care. AIM: To identify predictors of nonelective removal of PICCs in neonates. METHODS: A prospective c

Very low birth weight neonates who survive early-onset sepsis do not have an increased risk of developing late-onset sepsis.

BACKGROUND: Very low birth weight neonates (≤ 1500 g, VLBWs) have a high rate of infection and distinct baseline immune function compared with more mature populations. In critically ill children and adults, sepsis increases subsequent infection risk. It is unknown whether sepsis modifies the risk of subsequent infection in VLBWs. METHODS: We conducted a retrospective cohort study of VLBWs≤32weeks of gestation at birth cared for in 312 neonatal intensive care units in the United States from 1997 to 2011 (n=103,376). Early-onset sepsis (EOS, culture-positive only) and late-onset sepsis (LOS, cu
