Techniques of Laparoscopic Donor Nephrectomy in the United Kingdom: A narrative review.

Laparoscopic living donor nephrectomy, first carried out in 1995 by Lloyd Ratner, is now the established gold standard technique of donor nephrectomy globally and in the United Kingdom. Its minimally invasive nature with associated patient benefits, and concomitant excellent recipient outcomes underpin surgeon preference as the technique of choice in living kidney donation. There are 23 transplant centres in the United Kingdom currently offering laparoscopic donor nephrectomy. There are however variations in the laparoscopic approach amongst and within the different centres. These variations h

Which benign-appearing impassable sigmoid strictures are safe to manage conservatively? Insights from a multicenter cohort.

BACKGROUND: Impassable sigmoid strictures with benign endoscopic appearance present a clinical dilemma, balancing the risk of missed malignancy against the morbidity of surgical resection. Historical guidelines have recommended resection by default, but contemporary imaging and multidisciplinary team (MDT) input may allow for more selective, conservative management. OBJECTIVE: To evaluate clinical features and decision-making factors associated with safe non-operative management of benign-appearing impassable sigmoid strictures. METHODS: A multicentre retrospective analysis was conducted acr

Cardiovascular-Kidney-Metabolic (CKM) syndrome staging as a unified predictor of morbidity and mortality following pancreatic resection: A national cohort analysis.

BACKGROUND: Pancreatic resection is a profound physiologic stress test. Traditional risk assessment often focuses on procedure-specific technical failures, yet postoperative recovery depends heavily on a patient's systemic physiologic reserve. We evaluated the association between the American Heart Association's Cardiovascular-Kidney-Metabolic (CKM) syndrome framework and outcomes following pancreatic resection. METHODS: This retrospective study utilized the National Inpatient Sample to identify patients undergoing pancreatectomy. Patients were stratified into three CKM stages (0/1, 2/3, and

Constructing surgical confidence through simulation: A qualitative exploration in orthopaedic training.

BACKGROUND AND PURPOSE: Simulation-based training is widely used within surgical education. Although confidence is frequently evaluated as a post-training outcome, less attention has been given to how trainees experience confidence development in relation to preparedness for operative participation. This study explored how orthopaedic trainees develop confidence and perceived preparedness during simulation-based Dynamic Hip Screw (DHS) fixation training. METHODS: A qualitative study was undertaken with ten Core Surgical Trainees (CT1-CT2) at a UK teaching hospital. Participants completed DHS

Efficacy of negative-pressure wound therapy with adjunctive silver nanoparticle spray in post-revascularization diabetic foot wounds: A randomized controlled trial.

BACKGROUND: Diabetic foot wounds persisting after successful revascularization are challenging to heal and prone to infection. Negative-pressure wound therapy (NPWT) facilitates granulation, while silver nanoparticles offer antimicrobial benefits; their combination may improve clinical outcomes. OBJECTIVE: To compare the efficacy of NPWT with adjunctive silver nanoparticle spray versus standard NPWT alone in patients with post-revascularization diabetic foot wounds. METHODS: This single-center, randomized, double-blinded controlled trial enrolled 115 patients with diabetic foot wounds follow

The trials of conservative treatment of acute uncomplicated appendicitis in the paediatric population: A pilot study.

AIM OF THE STUDY: Acute appendicitis is the most common paediatric surgical emergency. Although appendicectomy remains the gold standard, conservative management with antibiotics has gained attention following promising results from recent trials such as CONTRACT. This study reviews outcomes of conservatively managed, ultrasound-confirmed appendicitis in a paediatric cohort. METHOD: A retrospective review was conducted of paediatric patients (<16 years) admitted with appendicitis between April 2020 and April 2022. Only cases with ultrasound-confirmed appendicitis were included. Patients with

Weight changes following total hip and total knee arthroplasty - A systematic review.

THE BACKGROUND AND PURPOSE OF THE STUDY: Symptomatic osteoarthritis can result in reduced mobility and exercise tolerance, leading to weight gain. Patients often feel that arthroplasty will result in improved mobility, reduced pain, and subsequent weight loss. The purpose of this systematic review was to assess weight changes following lower limb arthroplasty from available published literature. METHODS: A systematic review of PubMed, EMBASE and Cochrane Library databases were performed in accordance to PRISMA guidelines. Studies reporting significant weight changes of 5 % of pre-operative we

Environmental impact of single-use items and their contamination levels in elective orthopaedic surgery.

BACKGROUND: Single-use items have dominated operating theatres since the 1980s, initially perceived to carry a lower risk of surgical site infection and occupational blood exposure while maintaining hospital expenditure. The topic of reusable surgical gowns and drapes has naturally emerged due to discussions regarding climate change. Considering reusable alternatives is the next step in reducing the carbon footprint of operating theatres. METHODS: Clinical waste attributable to single-use items was collected from 93 cases in an elective orthopaedic hospital over four weeks, comprising knee ar

Feasibility of pancreaticoduodenectomy for octogenarian based on safety and postoperative nutritional recovery.

BACKGROUND: Pancreaticoduodenectomy (PD) for very elderly patients was still controversial. We evaluated the feasibility of PD for octogenarian (patients older than 80 years) from safety and postoperative nutritional changes including body composition. METHODS: We included 141 consecutive patients who underwent PD: 24 patients aged ≥80 years were assigned to the octogenarian group, and the remaining 117 were assigned to the non-octogenarian group. Surgical outcomes and the change in postoperative nutritional status (body weight and serum albumin level during 12 months post-surgery) were compa

Optimal approaches to flap reconstruction following abdominoperineal resection: A systematic review.

PURPOSE: Abdominoperineal resection (APR) frequently results in a large volume perineal defect. Flap reconstruction is commonly undertaken to reduce the rate of perineal complications associated with primary closure. Several techniques can be employed including vertical rectus abdominis (VRAM), gluteal myocutaneous and gluteal fasciocutaneous flaps. We aimed to compare perineal complication rates between flap reconstruction techniques. METHODS: A systematic review was conducted following PRISMA guidelines. Databases were searched for studies reporting perineal complications following flap rec

Target outflow vessel compromise after failed endovascular interventions for infra-inguinal arterial disease.

INTRODUCTION: Even after two major RCTs, the BEST CLI and BASIL II, the debate about best strategy to address infra-inguinal arterial disease continues. Damage of distal outflow vessels after failed angioplasties and compromise to future options remains the main concern of bypass-first strategy advocates versus endovascular enthusiasts. OBJECTIVES: To assess the degree of target outflow vessels damage after failed angioplasties. METHODS: This was a retrospective study looking at all episodes of failed angioplasties for infra-inguinal arterial disease over 5 years. Primary end points were the

Low-cost and bendable patient-specific three-dimensional pulmonary vessel models for lung segmentectomy.

BACKGROUND: Lung segmentectomy offers the advantages of preserving pulmonary function and improving patient survival, leading to its increased adoption. However, this procedure is technically demanding, especially when compared with lobectomy, owing to anatomical shifts from intraoperative lung collapse and manipulation. To support thoracic surgeons and enhance their surgical training, we developed a low-cost, patient-specific, and bendable three-dimensional (3D) pulmonary vessel model. This study describes the development, clinical application, and evaluation of this model. METHODS: A flexib

Multi-disciplinary teams for Crohn's disease: Who should be presented?

BACKGROUND: Multi-disciplinary team (MDT) conferences are used within the realm of colorectal surgery for both benign and malignant disease to assist in both diagnostic workup and to guide treatment. Usually, teams encompass colorectal surgery (CRS), gastroenterology (GE), pathology and radiology. Although it is believed that MDT discussion is associated with improved outcomes, there is a scarcity of evidence regarding the benefit or patient selection criteria among patients with inflammatory bowel disease (IBD). Due to advancements in both medical and surgical management in the management of

Childcare provision for on-call workers in the NHS: Is the 24/7 service ideal matched by reality?

BACKGROUND: The National Health Service (NHS) in the UK aims to deliver healthcare services around the clock. Major Trauma Centres (MTCs) are crucial in this operation, requiring continual 24/7 operations. Despite efforts to provide on-site nurseries catering to children aged 3 months to 5 years, there remains a significant gap in childcare provision for on-call workers. This shortfall particularly affects surgeons, nurses and military medical personnel whose shifts extend beyond standard nursery operating hours. This discrepancy raises concerns about the sufficiency of support for healthcare

Computed tomography scan-guided preoperative identification of the appendicular root as an indicator of the best skin incision site in open appendicectomy: a retrospective single-center study.

BACKGROUND: Appendicectomy is a well-established surgical procedure used for managing of acute appendicitis. In open appendicectomy, McBurney's point is the surgical landmark for locating the appendix, and it is common practice to make an incision there. However, in this study, we identified the root of the appendix via computed tomography, made an incision around that sites, and performed the appendicectomy through this incision. As such, this study aimed to assess the safety and outcomes of using the computed tomography scan-guided appendiceal root as a landmark of the incision site in open
