A case of pulmonary Mycobacterium kansasii presenting with an atypical extensive consolidation involving the entire right lower lobe.

A 76-year-old man with a 3-year history of total gastrectomy presented with extensive consolidation involving the entire right lower lobe on chest CT. Pulmonary Mycobacterium kansasii infection was diagnosed based on sputum and bronchoscopic specimen analyses. Treatment with rifampicin, ethambutol, and clarithromycin was initiated, resulting in improvement of symptoms and radiography findings. M. kansasii infection typically presents with thin-walled cavities in the upper lung lobes. We report a rare case of atypical radiological findings characterized by extensive consolidation in the right l…

A Rare Case of Disseminated Mycobacterium avium Complex Disease in a Patient Living With HIV Presenting With Endobronchial Lesions.

Disseminated Mycobacterium avium complex (MAC) infection is a well-recognized opportunistic disease in patients with advanced HIV infection; however, endobronchial involvement is exceedingly rare. Here, we report a case of disseminated MAC infection presenting with progressive endobronchial lesions associated with immune reconstitution inflammatory syndrome (IRIS). A 32-year-old man living with HIV who had discontinued antiretroviral therapy (ART) presented with severe immunosuppression (CD4+ count: 23 cells/µL) and was diagnosed with disseminated MAC infection based on lymph-node biopsy and p…

Exploring the Inhibitory Effects of Some Detergents on Mycobacterium avium subspecies paratuberculosis with Specific Focus on Zwitterionic Sulfobetaines.

Mycobacterium avium subspecies paratuberculosis (MAP) infects ruminants with Johne's disease and is also involved in human inflammatory bowel syndrome, such as Crohn's disease. With increasing drug resistance and zoonosis, there is a critical need for novel antimicrobial compounds having the potential to inhibit disease-causing bacteria. The present work examines the antimycobacterial activity of six detergent-like compounds, among which four are sulfobetaine-based zwitterionic detergents: amidosulfobetaine-14 (ASB-14), lauryl sulfobetaine (DDAPS), caprylyl sulfobetaine (D4266), and sulfobetai…

Targeting the antigen 85 complex of Mycobacterium tuberculosis: A three-fold validation of antimycobacterial activity of Centella asiatica.

INTRODUCTION: One of the biggest challenges associated with tuberculosis is the emerging drug resistance which emphasizes the need of alternative therapeutic approaches. In recent years, multitargeting approach for TB therapy has garnered considerable attention. The Antigen85 complex of proteins represent key enzymes which are involved in Mtb cell wall biosynthesis. This study aims at investigating the antimycobacterial effects of Centella asiatica and its two compounds - asiatic acid and quercetin against these three proteins and validate them as multi targets for drug targeting. METHODS: Th

Assessment of the efficacy of plant-derived essential oils against Mycobacterium tuberculosis: A systematic review and meta-analysis.

Mycobacterium tuberculosis (MTB), the causative agent of tuberculosis (TB), remains a major global health challenge, exacerbated by the rise of multidrug-resistant (MDR) and extensively drug-resistant (XDR) strains. Limitations in current antibiotic therapies have intensified interest in alternative antimicrobials, including plant-derived phytochemicals. Essential oils (EOs), with their complex chemical composition and long-standing traditional use, represent promising candidates for novel anti-TB agents. This study conducted a systematic review and meta-analysis to evaluate the in vitro antim…

The Mycobacterium tuberculosis specific antigen MPT64 in BALF has potential diagnostic value in the diagnosis of pulmonary tuberculosis.

BACKGROUND: Recent research on the use of the MPT64 antigen of Mycobacterium tuberculosis (MTB) in tuberculosis diagnosis has intensified. However, its detection in bronchoalveolar lavage fluid (BALF) has not been previously documented. This study aims to fill that gap. METHODS: We included a total of 176 patients, divided into a pulmonary tuberculosis (PTB) group of 104 cases and a non-tuberculosis (Non-TB) group of 72 cases as the control group. The PTB group includes 59 with bacteriologically confirmed PTB (BC-PTB) and 45 with clinically diagnosed PTB with negative pathogens (CD-PTB). The …

Enhanced In Vitro Stability of Bedaquiline with Ascorbic Acid and Pyruvate During Long-Term Incubation in Mycobacterium Species.

BACKGROUND: Drug susceptibility testing in Mycobacterium species typically requires prolonged incubation periods during which the chemical integrity of antibiotics may not be maintained, potentially compromising the reliability and accuracy of minimum inhibitory concentration (MIC) determinations. OBJECTIVES: This study evaluated the in vitro stability of several antibiotics, including recently introduced agents (bedaquiline [BDQ], pretomanid, delamanid and clofazimine) used for treating multidrug-resistant mycobacteriosis (linezolid and moxifloxacin), and those commonly included in combinati…

Evaluation of Xpert MTB/XDR and Deeplex Myc-TB for the Rapid Detection of Drug Resistance in Mycobacterium tuberculosis.

OBJECTIVES: Rapid identification of drug-resistant tuberculosis remains critical for the initiation of appropriate treatment regimens, improved success rates and reduction in the development of drug resistance. In this study, we compared the diagnostic performance of two novel diagnostic tests, Xpert MTB/XDR and Deeplex Myc-TB (both CE marked). METHODS: Twenty clinical isolates of Mycobacterium tuberculosis (MTB) with known drug resistance patterns were used to determine the concordance/discordance of the two rapid platforms when performing drug susceptibility testing. The limit of detection …

Ubiquitylation by the GID/CTLH complex regulates the metabolic and innate immune response of macrophages to infection by Mycobacterium tuberculosis.

The GID/CTLH E3 ligase complex is implicated in several biological processes, yet its full substrate repertoire remains poorly defined. We recently identified the complex as a broad modulator of macrophage responses to Mycobacterium tuberculosis (Mtb) infection. Here, we use label-free proteomics and diGly capture analysis of Mtb-infected macrophages to define the GID/CTLH-dependent ubiquitylome. We identify thousands of dynamically altered ubiquitylation sites, with strong enrichment among proteins involved in cellular metabolism and innate immune signaling. Concurrent proteome analysis revea…

The Impact of Second-Shell Residues on Substrate Binding at the Active Site of Thymidylate Synthase from Mycobacterium tuberculosis.

Employing classical alchemical free-energy simulations, we quantitatively evaluated how mutations in second-shell residues (Y44, Q106, Y108, and T181) affect the energetics of substrate (dUMP) binding to the active site of MtbThyX, an important drug target for Mycobacterium tuberculosis. The substrate binding affinity of nine mutants was compared with the wild-type MtbThyX. We found that specific mutations (viz., T181F or T181L) in the second-shell residue can decrease substrate binding by more than 8 kcal/mol. The decrease in substrate binding affinity is clearly demonstrated by the disruptio…

Efficacy and safety of an intravenous amikacin-containing regimen as initial therapy in Mycobacterium avium complex pulmonary disease.

INTRODUCTION: The global incidence of Mycobacterium avium complex pulmonary disease (MAC-PD) has been increasing, particularly in Japan. Although current guidelines recommend adding intravenous amikacin (AMK-IV) for cavitary or severe disease, evidence supporting its use as initial therapy remains limited, especially among elderly patients. METHODS: This retrospective study included patients with cavitary MAC-PD who received an AMK-IV-containing regimen as initial therapy at Kurashiki Central Hospital between September 2021 and July 2024. AMK-IV dosing was adjusted based on therapeutic drug m…

Genomic Epidemiology of Mycobacterium abscessus on the Island of Montréal Is Not Suggestive of Health Care-Associated Person-to-Person Transmission.

BACKGROUND: Mycobacterium abscessus complex (MABC), an opportunistic nontuberculous mycobacteria, can lead to poor clinical outcomes in pulmonary infections. Conflicting data exist on person-to-person transmission of MABC within and across health care facilities. To investigate further, a comprehensive retrospective study across 5 health care institutions on the Island of Montréal was undertaken. METHODS: We analyzed the genomes of 221 MABC isolates obtained from 115 individuals (2010-2018) to identify possible links. Genetic similarity, defined as ≤25 single-nucleotide polymorphisms (SNPs), …

Conversion or Reversion of Interferon γ Release Assays for Mycobacterium tuberculosis Infection: A Systematic Review and Meta-analysis.

BACKGROUND: Interferon γ release assays (IGRAs) are widely used for diagnosis of latent tuberculosis infection. However, with repeated testing, IGRA transformation (conversion or reversion) may be detected and is challenging to interpret. We reviewed the frequency of and risk factors for IGRA transformation. METHODS: We screened public databases for studies of human participants that reported the frequency of IGRA transformation. We extracted study and participant characteristics, details of IGRA testing and results. We calculated the pooled frequency of IGRA transformation (and transient tra

Synthesis of Heparan Sulfate Hexadecasaccharides and Their Molecular Interaction with Mycobacterial Heparin-Binding Hemagglutinin for the Detection of Mycobacterium tuberculosis.

Heparin-binding hemagglutinin (HBHA), located on the surface of Mycobacterium tuberculosis, binds to heparan sulfate (HS) on respiratory epithelial cells, initiating extrapulmonary dissemination and contributing to the development of latent tuberculosis. Previous characterization suggested that the lysine-rich domain of HBHA may accommodate an HS chain that is approximately twice the length of an octasaccharide. Herein, we prepared eight HS-based hexadecasaccharides through convergent assembly of a precursor, followed by divergent functional group modifications to generate varying sulfonation …

The efflux pumps Rv1877 and Rv0191 play differential roles in the protection of Mycobacterium tuberculosis against chemical stress.

BACKGROUND: It was previously shown that GlnA3sc enabled Streptomyces coelicolor to survive in excess polyamines. However, subsequent studies revealed that Rv1878, the corresponding Mycobacterium tuberculosis (M.tb) ortholog, was not essential for the detoxification of spermine (Spm), in M.tb. On the other hand, the multi-drug efflux pump Rv1877 was previously shown to enable export of a wide range of compounds, while Rv0191 was shown to be more specific to chloramphenicol. RATIONALE: Therefore, we first wanted to determine if detoxification of Spm by efflux can be achieved by any efflux pump…
