Pyriform sinus malformations: a cadaveric representation.

BACKGROUND/PURPOSE: The most important aspects in management of pyriform sinus malformations are awareness of the diagnosis, familiarity with the clinical manifestations, and complete surgical excision of the entire tract. Pyriform sinus anomalies are the least common branchial apparatus malformations and present anatomically as sinus tracts with or without cystic dilatation. The clinical presentations can include lateral neck mass, thyroid abscess, suppurative thyroiditis, retropharyngeal abscess, neonatal airway obstruction, and even carcinoma. Recurrent symptoms after surgery suggest incomp

A preclinical model for laryngeal transplantation: anatomy and mucosal immunology of the porcine larynx.

BACKGROUND: A major step in translating work on laryngeal transplantation into clinical practice is the establishment of a preclinical model. We have investigated the anatomy and mucosal immunology of the porcine larynx in eight Minnesota Minipigs (12-37 weeks). METHODS: Neck dissections were carried out and the vascular tree was mapped. Snap-frozen biopsies from epiglottis, supraglottis, glottis, and subglottis were prepared for conventional histology, immunohistochemistry (CD45), and single and two-color immunofluorescence (CD3, MHC-II, CD45). RESULTS: The anatomy of the laryngeal skeleton…

Galeal pedicle flap of the occipital region for pharynx reconstruction: anatomic and clinical considerations.

Surgeons are still searching for the ideal method for pharynx reconstruction after tumor ablation. The objective of this study was to prove the anatomic and clinical viability of an occipital galeal pedicle flap for hypopharynx reconstruction. We studied anatomic details in 50 fresh adult cadavers. The dissections were performed after posterior galea exposure, silicone injection in occipital vessels, and mobilization of the galeal flap with an 8 x 8 cm square of galea. We also used the proposed flap in three clinical cases after laryngopharyngectomy. The surgical technique and its problems are

Supraclavicular nerve block: anatomic analysis of a method to prevent pneumothorax.

Supraclavicular nerve blocks are technically easy to perform, but may be associated with pneumothorax. The objective of this study is to define the parasagittal anatomy important to our modified technique of supraclavicular nerve block designed to decrease the incidence of pneumothorax and to determine whether this technique is anatomically sound. Two cadaver specimens were studied. One embalmed specimen was dissected to establish the relationship of the brachial plexus to our modified needle entry site. The neck and upper thorax of an unembalmed cadaver were frozen, and parasagittal serial se

Cervical lymph node metastases of oral carcinoma related to the depth of invasion of the primary lesion.

Squamous cell carcinoma of the tongue has been found to have a greater frequency of cervical metastases on initial examination than lesions affecting other oral sites. This study analyzes a sample of 21 pathological specimens of oral carcinoma and block neck dissections relating the confirmed histological presence of cervical metastasis to the depth of invasion of the primary lesion. Tongue carcinomas were compared with lesions affecting other oral sites. An arbitrary anatomical levelling system was devised to assess the depth of invasion. Tongue carcinomas show a propensity to infiltrate more…

[Spontaneous dissecting aneurysms of the cerebral arteries].

A review of the literature on Spontaneous Dissecting Aneurysms (DA's) of cerebral arteries is presented with 3 personal cases of DA's of the Internal Carotid Artery (ICA). Patients with spontaneous DA's of the extracranial ICA are of middle age (30 to 60 year old) and present with an ipsilateral pain in the neck or face and/or with TIAs (45 p. 100 and 50 p. 100 of the cases respectively). Claude Bernard-Horner's sign is frequent. The clinical, radiological and pathological data suggest that in most cases, if not all, neurological deficits are due to embolism and/or anterograde thrombus. This h

Sentinel node biopsy in early oral squamous cell carcinomas: a 10-year experience.

OBJECTIVES/HYPOTHESIS: To evaluate the reliability of the sentinel node (SN) biopsy in early oral squamous cell carcinomas. STUDY DESIGN: Prospective cohort study. METHODS: We conducted a primary prospective study on 53 consecutive patients presenting T1, T2 N0 squamous cell carcinomas of the oral cavity between January 2000 and June 2003. Primary results demonstrated a negative predictive value of 100%. The series was then extended until June 2010, with a total number of 166 successful procedures. RESULTS: The cohort accounted for 118 males and 48 females with a mean age of 56 years. The m

Preferential impairment of nitric oxide-mediated endothelium-dependent relaxation in human cervical arteries after irradiation.

BACKGROUND: Vascular abnormalities are a major cause of postoperative complications in irradiated tissues. Endothelial cell dysfunction characterized by diminished endothelium-dependent relaxation may be involved. We examined the endothelium-dependent relaxation and morphology of the endothelium in irradiated human cervical arteries. METHODS AND RESULTS: Irradiated arteries were taken from the neck region of patients who had radiation therapy. Arteries from patients who did not receive radiation therapy were used as controls. Endothelium-dependent relaxation to acetylcholine and A23187 was im

Papillary thyroid carcinoma in thyroglossal duct cyst: a Peruvian case series.

SUMMARY: Papillary thyroid carcinoma (PTC) is the most frequent type of differentiated thyroid cancer, while thyroglossal duct cysts (TGDCs) are common congenital anomalies of the neck. The coexistence of PTC within a TGDC is exceptionally rare, with a reported incidence of less than 1.5%. We present three Peruvian cases of PTC arising in TGDCs. The patients (two females and one male; age range: 34-47 years) presented with progressive midline cervical masses of two to four years' duration. All underwent cervical ultrasound, contrast-enhanced computed tomography, and fine-needle aspiration biop

Malignant melanoma presenting as a thyroid mass with synchronous retroperitoneal metastases in a 55-year-old male: a case report.

BACKGROUND: Malignant melanoma (MM) is an aggressive tumor characterized by a high tendency for early metastasis. Although most cases have identifiable primary sites, melanoma of unknown primary (MUP) presents as metastatic disease without detectable primary lesions. MM presenting as a thyroid mass with concurrent metastasis to the spleen-kidney interspace is extremely rare, and the occult nature of the primary lesion also poses a significant diagnostic challenge, often leading to confusion with aggressive primary thyroid malignancies. CASE DESCRIPTION: A 55-year-old male with an unremarkable

Optimization strategies for ultrasound zoning of cervical lymph nodes in esophageal cancer: a precision medicine approach.

Esophageal cancer is a common malignancy with high incidence and mortality rates. Its pathological types vary by region, with squamous cell carcinoma predominant in Asia and adenocarcinoma in Western countries. Accurate staging before treatment is crucial for selecting appropriate therapeutic strategies. The existing international staging systems primarily include the American Joint Committee on Cancer and Union for International Cancer Control (AJCC/UICC) system and the Japanese Esophageal Society (JES) system. However, these systems differ in lymph node definitions and zoning, particularly r

Clinical criteria to exclude acute vascular pathology on CT angiogram in patients with dizziness.

BACKGROUND: Patients presenting to the emergency department (ED) with dizziness may be imaged via CTA head and neck to detect acute vascular pathology including large vessel occlusion. We identify commonly documented clinical variables which could delineate dizzy patients with near zero risk of acute vascular abnormality on CTA. METHODS: We performed a cross-sectional analysis of adult ED encounters with chief complaint of dizziness and CTA head and neck imaging at three EDs between 1/1/2014-12/31/2017. A decision rule was derived to exclude acute vascular pathology tested on a separate valid
