Secondary digital nerve repair in the foot with resorbable p(DLLA-epsilon-CL) nerve conduits.

Nerve guides are increasingly being used in peripheral nerve repair. In the last decade, much preclinical research has been undertaken into a resorbable nerve guide composed of p(DLLA-epsilon-CL). This report describes the results of secondary digital nerve reconstruction in the foot in a patient with post-traumatic neuromas of the common plantar digital nerves II-III and III-IV. The neuromas were resected and reconstruction of the nerves was carried out with resorbable Neurolac nerve guides. The Pressure Specified Sensory Device was used to measure the static (s) and moving (m) 1- and 2-point

Delayed motor and sensory neuropathy in a patient with brainstem encephalitis.

Bickerstaff's brainstem encephalitis (BBE), Miller Fisher syndrome (MFS) and Guillain-Barré syndrome (GBS) are thought to be closely related and to form a continuous spectrum. However, chronic polyneuropathy in BBE has not been reported. We report the temporal profile of anti-ganglioside antibody titer in a case of BBE-like brainstem encephalitis complicated with chronic polyneuropathy. A 71-year-old Japanese woman presented with drowsiness and cerebellar ataxia in addition to mild weakness in distal limb muscles. Anti-GalNAc-GD1a IgG and anti-GalNAc-GM1b IgG antibodies were positive in her se

Wrist median nerve motor conduction after end range repeated flexion and extension passive movements in Carpal Tunnel Syndrome. Pilot study.

Carpal Tunnel Syndrome (CTS) can be due to a variety of different pathological conditions. These etiological and epidemiological differences may explain the non-homogeneous response to ordinary conservative therapeutical options observed in this syndrome. The aim of our study was to investigate on the possibility of identifying different sub-groups of patients among conservatively treatable CTS with different susceptibility to physiotherapeutic treatments. We decided to utilize an objective approach measuring some median motor nerve function parameters. Short term variations of Compound Motor

Fracture callus engulfing a peripheral nerve does not affect its function: an experimental study in rabbits.

The fate of a peripheral nerve engulfed in fracture callus is not known. We investigated the impact of envelopment of the sciatic nerve by fracture callus using a New Zealand rabbit femoral fracture model. The sciatic nerve was mobilized and coiled around the ipsilateral femur, which was surgically fractured, shortened, and osteosynthesized. Bony union was achieved, and callus engulfed the sciatic nerve in all animals. Nerve function was evaluated clinically and by conduction studies preoperatively and postoperatively. Although the nerve function in terms of clinical evaluation, amplitude, mot

Cholinergic regulation of the evoked quantal release at frog neuromuscular junction.

The effects of cholinergic drugs on the quantal contents of the nerve-evoked endplate currents (EPCs) and the parameters of the time course of quantal release (minimal synaptic latency, main modal value of latency histogram and variability of synaptic latencies) were studied at proximal, central and distal regions of the frog neuromuscular synapse. Acetylcholine (ACh, 5 x 10(-4) M), carbachol (CCh, 1 x 10(-5) M) or nicotine (5 x 10(-6) M) increased the numbers of EPCs with long release latencies mainly in the distal region of the endplate (90-120 microm from the last node of Ranvier), where th

Preserved cutaneous silent periods in severe entrapment neuropathies.

Noxious digital nerve stimulation during isometric contraction of hand muscles leads to transient suppression of the electromyographic activity, the so-called cutaneous silent period (CSP), which is mostly due to a spinal reflex mediated by A-delta fibers. We investigated two patients with carpal tunnel syndrome (CTS) and two patients with ulnar entrapment at the elbow (UNE), in whom routine sensory conduction studies failed to document afferent fiber continuity across the lesion site. In three patients, motor nerve conduction studies and electromyography failed to demonstrate intact efferent

A suspected case of proximal diabetic neuropathy predominantly presenting with scapulohumeral muscle weakness and deep aching pain.

A 48-year-old man with a 14-year history of type 2 diabetes with proliferative diabetic retinopathy and distal symmetrical diabetic polyneuropathy visited our hospital. Eight months later, he subacutely developed difficulty in both shoulder movement and trouble standing up from a squatting position. This was accompanied by severe bilateral shoulder and thigh pain. Magnetic resonance imaging of the brain, cervical and lumbar spine, computed tomography of the shoulder and X-ray films of the cervical spine and shoulder revealed no abnormality. Cerebrospinal fluid showed a mild elevation of protei

Proton magnetic resonance spectroscopy and transcranial magnetic stimulation for the detection of upper motor neuron degeneration in ALS patients.

Transcranial magnetic stimulation (TMS) was compared to proton magnetic resonance spectroscopy (1H-MRS) for the detection of upper motor neuron loss or dysfunction in 49 ALS patients classified according to the El Escorial criteria. Abnormal NAA/Cho ratios were detected in 53% of ALS patients. Abnormal TMS results (i.e. cortical inexcitability or prolonged CMCT's) were obtained in 63% of ALS patients. If one or both methods were considered for diagnosis of upper motor neuron degeneration/dysfunction, the percentage of abnormal findings was 77%, whilst in 39% of all patients both methods produc

An in vitro temporomandibular joint-nerve preparation for pain study in rats.

A novel in vitro TMJ-nerve preparation was developed to quantitatively study peripheral sensory mechanisms of temporomandibular joint (TMJ). The TMJ region on one side (including mandibular head, disc, retrodiscal tissue and mandibular fossa) of adult Wistar albino rats was excised together with the auriculo-temporal nerve. The block was preserved in a modified Krebs-Henseleit solution saturated with O(2)/CO(2) (95/5%) gas mixture. Using a calibrated von Frey type apparatus, mechanical noxious stimulation was applied directly to various sites within the TMJ region. In addition, thermal and che

Multiple agents potentiate alpha1-adrenoceptor-induced conduction depression in canine cardiac purkinje fibers.

BACKGROUND: Halothane more so than isoflurane potentiates an alpha1-adrenoceptor (alpha1-AR)-mediated action of epinephrine that abnormally slows conduction in Purkinje fibers and may facilitate reentrant arrhythmias. This adverse drug interaction was further evaluated by examining conduction responses to epinephrine in combination with thiopental and propofol, which "sensitize" or reduce the dose of epinephrine required to induce arrhythmias in the heart, and with etomidate, which does not, and responses to epinephrine with verapamil, lidocaine, and l-palmitoyl carnitine, a potential ischemic

Peroneal motoneuron excitability increases immediately following application of a semirigid ankle brace.

STUDY DESIGN: Within-session, within-subject comparison of 2 conditions. OBJECTIVES: To determine the influence of application of a semirigid ankle brace on the excitability of the peroneus longus muscle motoneuron pool as measured by the H-reflex. BACKGROUND: The literature suggests that cutaneous mechanoreceptors can contribute to proprioception, especially during conditions in which skin experiences displacement. Further, skin displacement and stimulation of cutaneous mechanoreptors have been shown to increase motoneuron excitability. METHODS AND MEASURES: H-reflexes and M-waves of the p

A clinical and neurophysiological study of scorpion envenomation in Assiut, Upper Egypt.

Scorpion envenomation (SE) represents an agonizing problem in many countries, especially in rural areas. This clinical and neurophysiological study aimed to determine the relative frequency of scorpion envenomation in the Assiut area, in Upper Egypt. Full clinical evaluation was carried out for all children < or =18 y of age included in the study. Electroencephalography (EEG), electromyography (EMG) and motor conduction velocity measurements were carried out for a variable number of children. SE was recorded in 302 cases per year in this area. Of these, 78.5% were < or =18 y of age. SE occurre

Interfacing the body's own sensing receptors into neural prosthesis devices.

Functional Electric Stimulation (FES) is today available as a tool in muscle activation used in picking up objects, in standing and walking, in controlling bladder emptying, and for breathing. Despite substantial progress over nearly three decades of development, many challenges remain to provide a more efficient functionality of FES systems. The most important of these is an improved control of the activated muscles. Instead of artificial sensors for feedback, new developments in electrodes to do long-term and reliable recordings from peripheral nerves emphasize the use of the body's own sens

Pelvic floor function/dysfunction and electrodiagnostic evaluation.

The pelvic floor provides support for the bladder, rectum, and genital systems, as well as proper positioning and orientation of the urethral and anal sphincters. Impairment may result in prolapse, urinary and fecal incontinence, and sexual dysfunction. The impact is enormous in terms of personal, social, and financial burden. Pertinent anatomy is presented, followed by an overview of available electrodiagnostic techniques and a description of pudendal nerve conduction studies, sacral reflex testing and selected electromyographic techniques. Clinical applications are discussed throughout the t

Reproducibility of different methods for diagnosing and monitoring diabetic neuropathy.

Conflicting results have been published concerning the reliability of methods to assess diabetic polyneuropathy. Therefore we compared the reproducibility of nerve conduction studies and quantitative measurements of vibratory sensation in 23 diabetic patients with clinically and/or neurophysiologically established neuropathy. Motor nerve conduction of peroneal and median nerves, sensory nerve conduction of sural and median nerves and vibratory perception thresholds of index fingers and big toes (forced-choice method) were assessed on three occasions with an interval of one to fourteen days, wi

Surgical treatment of carpal tunnel syndrome and trigger digits in children with mucopolysaccharide storage disorders.

The role of surgical intervention for carpal tunnel syndrome (CTS) and trigger digits in children with mucopolysaccharide storage disorders (MPSDs) has not been clearly defined, particularly as the treatment of the underlying disease has advanced to include bone marrow transplantation. This study reviews our experience in the treatment of CTS and trigger digits in 22 children with MPSDs who were evaluated for CTS by electromyographic (EMG)/nerve conduction velocity (NCV) testing. Seventeen children were diagnosed with CTS by EMG/NCV testing and were treated with bilateral open surgical release
