Characterization of Intraoperative Motor Evoked Potential Monitoring for Surgery of the Pediatric Population with Brain Tumors.

OBJECTIVE: We investigated the relationship between the reliability of the transcranial or transcortical motor evoked potential (MEP) response and age in pediatric patients aged ≤15 years with brain tumor. METHODS: We retrospectively analyzed the data from 60 consecutive patients aged ≤15 years who had undergone brain tumor surgery that involved intraoperative MEP monitoring from October 2009 to May 2016. RESULTS: A total of 41 patients with reliable signals (MEP response group) and 19 patients without reliable signals (MEP nonresponse group) were included in the present study. The mean age

Electroencephalographic markers of brain development during sevoflurane anaesthesia in children up to 3 years old.

BACKGROUND: General anaesthetics generate spatially defined brain oscillations in the EEG that relate fundamentally to neural-circuit architecture. Few studies detailing the neural-circuit activity of general anaesthesia in children have been described. The study aim was to identify age-related changes in EEG characteristics that mirror different stages of early human brain development during sevoflurane anaesthesia. METHODS: Multichannel EEG recordings were performed in 91 children aged 0-3 yr undergoing elective surgery. We mapped spatial power and coherence over the frontal, parietal, temp

Feasibility of Continuous Intraoperative Neural Monitoring During Transoral Endoscopic Thyroidectomy Vestibular Approach in a Porcine Model.

Background: Experimental study to evaluate feasibility of continuous intraoperative neural monitoring (C-IONM) in transoral endoscopic thyroidectomy vestibular approach (TOETVA). Methods: Duroc-Landrace pigs were orally intubated with electromyogram endotracheal tube. Automatic periodic stimulation (APS) electrode was allocated in the operative field through the 5-mm ports. APS was then repose on vagal nerve (VN) with different approaches: (1) median, that is, between sternothyroid and thyroid gland; and (2) lateral, that is, between sternocleidomastoid and sternothyroid. VN was stimulated pro

Simultaneous lateral interbody fusion and pedicle screws (SLIPS) with CT-guided navigation.

OBJECTIVES: The advent of minimally invasive, percutaneous techniques for the placement of pedicle screws has led to the evolution of a popular treatment paradigm: anterior or lateral interbody fusions followed by posterior percutaneous pedicle screw placement. We present the operative technique for anterior-to-psoas lateral interbody fusion (ATP-LIF) with simultaneous posterior lumbar percutaneous pedicle screw fixation using intraoperative CT-guided navigation. PATIENTS AND METHODS: This technique capitalizes both on the more oblique approach used in the ATP-LIF procedure, as well as the an

Theory-of-mind-related neural activity for one's romantic partner predicts partner well-being.

Healthy social relationships are linked to myriad positive physical and mental health outcomes, raising the question of how to enhance relationship formation and quality. Behavioral data suggest that theory of mind (ToM) may be one such process. ToM is supported by a network of brain regions including the temporo-parietal junction (TPJ), medial prefrontal cortex and precuneus (PC). However, little research has investigated how the ToM network supports healthy social relationships. Here, we investigate whether recruitment of the ToM network when thinking about the mental states of one's romanti

The anatomy of the external branch of the superior laryngeal nerve in Koreans.

BACKGROUND: Preservation of the external branch of the superior laryngeal nerve (EBSLN) during thyroidectomy is important because its injury may lead to frequent occurrence of vocal fatigue and the inability to perform superenergetic phonation. Most studies on the anatomy of EBSLN have been performed in Western countries and, thus, have some limitations in their application to Koreans. We explored the anatomy of EBSLNs using intraoperative neuromonitoring (IONM) in Korean adults. METHODS: Between August 2011 and December 2011, 50 patients underwent thyroidectomy at Korea University, Anam Hosp

EFNS guideline on diagnosis and management of post-polio syndrome. Report of an EFNS task force.

Post-polio syndrome (PPS) is characterized by new or increased muscular weakness, atrophy, muscle pain and fatigue several years after acute polio. The aim of the article is to prepare diagnostic criteria for PPS, and to evaluate the existing evidence for therapeutic interventions. The Medline, EMBASE and ISI databases were searched. Consensus in the group was reached after discussion by e-mail. We recommend Halstead's definition of PPS from 1991 as diagnostic criteria. Supervised, aerobic muscular training, both isokinetic and isometric, is a safe and effective way to prevent further decline

Arthrogenic muscle inhibition in the leg muscles of subjects exhibiting functional ankle instability.

BACKGROUND: Functional ankle instability or a subjective report of ;;giving way'' at the ankle may be present in up to 40% of patients after a lateral ankle sprain. Damage to mechanoreceptors within the lateral ankle ligaments after injury is hypothesized to interrupt neurologic feedback mechanisms resulting in functional ankle instability. The altered input can lead to weakness of muscles surrounding a joint, or arthrogenic muscle inhibition. Arthrogenic muscle inhibition may be the underlying cause of functional ankle instability. Establishing the involvement of arthrogenic muscle inhibition

[Results of surgical treatment of radio-humeral epicondylopathy].

We examined extent and affection of an assumed neuromuscular transmission disorder by performing a prospective clinical study on 75 patients with therapy-resistant radiohumeral epicondylopathy. Before operation, we electromyographically diagnosed an increased rate of polyphasic potentials of the long wrist extensors as well as a prolonged motor latency of the respective muscles. Corresponding to a hereby implied damage to the distal part of the motor neuron, disordered neuromuscular recruitment combined with a reduced maximum strength and -elasticity could be proven. Both effects were signific

Comparative study between the normally hearing child and the hard of hearing child, by acoustic impedance measurements of the ear.

In the first part, the author makes a comparative study of the tympanometric results obtained from the normally hearing child and the hard of hearing child. The frequency of serous otitis media is statistically higher in the deaf child. The author compares such results of tympanometry as obtained from the child, considers the contribution of this technique to the field of pediatric audiology and suggests a classification of tympanograms in terms of the probe-tone frequency of the impedancemeter. The second part concerns the acoustico-facial reflex. This study compares the recording of the stap

Functional Somatotopy of Lumbar Dorsal Rootlets and its Role in Selective Recruitment via Lateral Spinal Cord Stimulation.

OBJECTIVE: Lateral spinal cord stimulation (LSCS) is a promising approach for restoring somatosensory feedback in lower-limb amputees, but its spatial selectivity remains limited. Percepts often spread to unintended regions of the residual limb, and reducing electrode contact size may not improve focality. This study investigated whether the anatomical organization of lumbar dorsal rootlets (DR) imposes fundamental constraints on LSCS selectivity. APPROACH: Acute neurophysiology experiments were performed in six adult cats. Both LSCS and individual DR stimulation were conducted in the same an…
