Complications of tension-band wiring of olecranon fractures.

We encountered a high incidence of complications related to the technique of tension-band wire fixation of displaced fractures of the olecranon in a five-year retrospective study of twenty patients (twenty fractures). All had been treated with primary open reduction using the AO technique of tension-band wiring. Twenty patients were followed at least to union as determined radiographically. The most frequent complication was symptomatic prominence of the Kirschner wires at the elbow in sixteen patients. There was skin breakdown in four patients, and infection developed in one. Measurable proxi

Failure of the Noncontact Bridging Periprosthetic Plating System: A Single-Institution Experience.

BACKGROUND: Periprosthetic femur fractures (PFFs) are challenging complications following total joint arthroplasty. Locking compression plates have emerged as a preferred option because of their stability and ability to be placed in a minimally invasive fashion. Surgical techniques for their application have been well-described; however, there are limited reports of failure. Here, we present 6 unique cases of implant failure using the Noncontact Bridging (NCB) Periprosthetic Plating System in the management of PFF. METHODS: A retrospective chart review was conducted of all open reduction inte

Selective facet joint and interlaminar fusion using unilateral biportal endoscopy for thoracolumbar compression fractures: a comparative study.

To compare the clinical outcomes and radiological findings of selective facet joints and interlaminar fusion using UBE technique with percutaneous pedicle screw fixation (observation group, OG) versus open reduction fusion fixation (control group, CG) in patients with thoracolumbar compression fractures. 110 patients were included, including 56 in the OG and 54 in the CG. The clinical results were evaluated by visual analogue scale score (VAS) and the oswestry disability index score(ODI). Radiographic findings were evaluated by the sagittal cobb angle(CA), the anterior vertebral height(AVH) an

Ulnar Nerve Neuropathy After Surgery for Intraarticular Distal Humerus Fractures: An Analysis of 116 Patients.

PURPOSE: To identify the incidence and the factors associated with a postoperative ulnar nerve neuropathy in patients who had undergone open reduction and internal fixation for intraarticular distal humerus fractures. METHODS: We retrospectively reviewed 116 patients who had undergone surgery between January 2011 and December 2020. Age, sex, BMI, mechanism of injury, open or closed fracture, operation time, tourniquet time, and nerve injury at the final examination were collected from medical charts. We essentially used the paratricipital approach. In cases in which the reduction of intraarti

Nonmetallic posterior monosegmental cervical fusion of a dislocated C6/7 fracture in a 4-year-old girl : A case report.

PURPOSE: Pediatric cervical spine injuries constitute approximately 1-2% of all pediatric trauma cases. Usually pediatric vertebral injuries appear as stable A type fractures, whereas B and C type injuries are relatively uncommon. In contrast to adults, the appropriate treatment strategy in children is still controversial and places spine surgeons in complex situations. This article reports the case of a 4-year-old girl with an unstable B type injury at the C6/7 level (AOSpine C6-7: B2 [F4 BL, C7:A1]) with bilateral locked fractures of the facet joints after falling down a flight of stairs. Acute on Chronic Distal Radius Fracture: A Case Series and Technique Description.

PURPOSE: Distal radius fractures are the most common fractures in adults. Because of the prevalence of these injuries, patients may present with a repeat distal radius fracture on the same wrist through the site of a malunion. We clinically refer to this as an acute on chronic distal radius fracture. In this setting, the restoration of acceptable alignment can be challenging. There is little guidance in the literature for the management of these fractures. We report our experience with acute on chronic distal radius fractures. The secondary fracture plane was used to correct the prior deformit

Modified Mason type III and IV radial head fractures: results of different surgical treatments.

BACKGROUND AND AIM: Fractures of the radial head account for 4% of all fractures and 33 % of all elbow fractures. Their treatment is somewhat challenging and diversified, especially in more complex fractures (type III and IV of modified Mason's classification). The aim of this study was to identify the best surgical treatment for patients having sustained these latter lesions and outline possible predictive factors of worse outcomes. MATERIAL AND METHODS: Data were retrospectively collected for 63 patients affected by radial head fracture and operated between 2006 and 2014 at the University H

Blow-in fractures of the orbit.

A blow-in fracture is an inwardly displaced fracture of the orbital rim or wall resulting in decreased orbital volume. The purpose of this study is to classify orbital blow-in fractures, describe the distinguishing clinical and radiologic features, and review the result of treatment. The series consists of 41 patients with blow-in fractures (34 males and 7 females). The mean age of the patients was 36 years. All were treated between 1979 and December of 1986 at Sunnybrook Medical Centre in Toronto. Clinical features of blow-in fractures were primarily related to the decrease in volume of the o

Retrospective analysis of 29 fractures of the olecranon process of the equine ulna.

Twenty-nine fractures of the olecranon process of the ulna in horses treated at The Ohio State University College of Veterinary Medicine from 1973 through 1983 were evaluated. The objectives of the study were to determine the prognosis for various configurations of fractures of the olecranon process of the ulna, to determine what preoperative factors influenced the prognosis, and to evaluate the efficacy of the treatment methods used. The fractures were placed in one of 4 categories on the basis of severity of the injury: type-1 fractures involved the physeal plate; type-2 fractures involved t

[Postoperative outcome of 30 patients with navicular bone fractures-In connection with AOFAS-Score, SF-12 and fracture classifications].

BACKGROUND: Fractures of the navicular bone are rare and the number of those treated surgically is even smaller. Moreover, scientific analyses on this topic are only sporadically present in the literature, therefore this retrospective and monocentric study was initiated. METHODS: A total of 30 patients with 30 fractures were included. With the exception of one primary fusion, all patients underwent open reduction with internal osteosynthesis. Clinical and radiological follow-up was performed at least 2 years postoperatively using AOFAS-Score, SF-12 and a radiological examination. The primary

Distal Radius Plates With Coracoid Suture Anchors for the Fixation of Displaced Lateral End Clavicle Fractures.

Introduction Management of displaced fractures of the lateral end clavicle has always challenged orthopaedic surgeons, due to a high non-union rate, and difficulty achieving stable fixation allowing early mobilisation. Different methods of fixation have been utilised to provide stability and improve healing and functional outcomes. In this series, we evaluate the results of internal fixation using dorsal distal radius locking plates and coracoid suture anchors. Patients and methods We retrospectively reviewed the clinical notes and radiographs of eight consecutive patients with fractures of th

Neuropathic ankle fractures.

A 65-year-old diabetic female presented with a 3-week history of a left swollen foot after a minor inversion injury and was found to have a minimally displaced fibular fracture. Despite casting and strict instructions to remain non-weight bearing, the patient continued to bear weight and later developed a significantly more displaced fracture with a draining ulcer. This injury eventually required a tibiotalocalcaneal arthrodesis using a retrograde hindfoot nail. Neuropathy and neuropathic fractures can be devastating complications of diabetes and thus require early diagnosis and intervention b

Closed Pantalar Dislocation With Checkrein Deformity: A Unique Case Report and Literature Review.

Closed pantalar dislocations are a rare variant of an uncommon injury. Pantalar dislocations are typically caused by high-energy trauma resulting in an open injury with associated fracture of the articulating bones. Given its obscurity, the literature on closed pantalar dislocations is scarce, and no standard treatment protocol has been accepted. This case report chronicles the treatment and outcome of a 29-year-old man who presented with a checkrein deformity of all digits after a closed pantalar dislocation with 6-month follow-up. A comprehensive literature review found 28 articles represent

External Fixation versus Unreamed Tibial Intramedullary Nailing for Open Tibial Fractures: A Meta-analysis of Randomized Controlled Trials.

Controversy exists over whether the use of external fixation (EF) or unreamed tibial intramedullary nailing (UTN) is optimal for the treatment of open tibial fractures. The aim of this study was to compare clinical outcomes in terms of postoperative superficial and deep infection, malunion, delayed union, nonunion and hardware failure between these two treatment methods. So a systematic review and meta-analysis was performed. All available randomized controlled trials that compared the clinical results of EF to those of UTN were obtained and the reported numbers of citations for each observed

Pediatric supracondylar humeral fractures: Does pin and splint removal 3 weeks after surgery increase the risk of secondary displacement?

PURPOSE: Supracondylar fractures of the distal humerus are among the most common pediatric fractures. For unstable fractures, reduction with pin fixation and postoperative immobilization is standard, but the optimal duration of pin fixation and splinting remains uncertain. This study evaluated whether pin and splint removal approximately 3 weeks after surgery was associated with secondary fracture displacement in children treated for displaced supracondylar humeral fractures. METHODS: We retrospectively reviewed pediatric patients surgically treated for displaced supracondylar humeral fractur

Talar Arthroscopic Reduction and Internal Fixation (TARIF): A Novel All-Inside Soft-Tissue-Preserving Technique.

BACKGROUND: Talar arthroscopic reduction and internal fixation (TARIF) is an alternative approach for the operative fixation of talar fractures that may be utilized instead of more traditional open approaches such as medial, lateral, or even dual anterolateral. The TARIF approach allows for nearly anatomic fracture reduction and fixation of talar neck, body, and posterior dome fractures while minimizing the soft-tissue stripping and vascular injury associated with the standard anterolateral approach. DESCRIPTION: Following initial closed fracture reduction and any associated procedures, we re
