Maxillary malignant mesenchymoma and massive fibrous dysplasia.

This is the first report (to our knowledge) that describes a patient with massive polyostotic fibrous dysplasia involving the calvaria and facial skeleton that subsequently underwent transformation to a malignant mesenchymoma with elements of chondrosarcoma, osteosarcoma, and rhabdomyosarcoma arising in the maxilla. Malignant transformation occurred in the absence of prior radiation exposure, osteomyelitis, or known bony infarction. A review of the literature did not reveal any similar cases of massive fibrous dysplasia of the maxilla degenerating to multiple simultaneous malignant histotypes.

CT/MRI of musculoskeletal complications of AIDS.

While uncommon, many musculoskeletal disorders may be seen in association with the acquired immune deficiency syndrome (AIDS). Infections such as osteomyelitis, bacterial myositis and septic arthritis, neoplasms such as non-Hodgkin lymphoma and Kaposi sarcoma, and myopathies and polymyositis have been reported in this patient population. Computed tomography and magnetic resonance imaging frequently detect unanticipated musculoskeletal disease in a patient with AIDS, and may further help to distinguish infections from neoplastic disorders.

Spinal vacuum phenomena: anatomical study and review.

"Vacuum" phenomena relate to the accumulation of gas, principally nitrogen, in crevices within the intervertebral disk or vertebra. Their appearance does not uniformly indicate "degenerative" disk disease (primary intervertebral osteochondrosis), as gaseous collections may accompany other processes (vertebral osteomyelitis, Schmorl node formation, spondylosis deformans, vertebral collapse with osteonecrosis) affecting the disk and adjacent vertebral bodies. The location and appearance of the "vacuum" phenomena are helpful indicators as to the precise nature of the spinal disorder.

[Radiological findings in periosteal reaction due to inflammation (author's transl)].

Presentation of radiographically apparent periosteal changes due to inflammation. Solid periosteal reaction is being distinguished from an interrupted-type reaction. Some cases of acute and chronic osteomyelitis, congenital and acquired lues, bony tuberculosis, leprosy and fungal disease serve to demonstrate the different types of periosteal reaction; and a differential diagnosis is being discussed. The diagnostic evaluation should include bone scanning in addition to routine radiography with special views and follow up examinations.

When Rewiring Fails-The Enduring Role of the Pectoralis Major Flap in Sternal Wound Reconstruction.

Background: Deep sternal wound infections (DSWIs) remain a serious complication after median sternotomy, often requiring complex wound management strategies. While modern approaches include vacuum-assisted closure (VAC) and plating techniques, the pedicled pectoralis major muscle flap (PMF) continues to play a pivotal role in surgical reconstruction, especially in cases with sternal destruction or osteomyelitis. Methods: In this retrospective single-centre analysis, 166 patients with DSWI following cardiac surgery were reviewed. Clinical data, comorbidities, laboratory parameters, and surgical…

Successful treatment of Rhodococcus equi P-type infection with subcutaneous abscessation in a foal.

Rhodococcus (R.) equi is a common causative agent of the septic arthritis, physitis, and osteomyelitis (SAPO) complex in foals, often resulting from hematogenous dissemination or perforating trauma. The early detection of osteomyelitis is challenging, and treatment has a guarded prognosis.A 3-month-old warmblood filly was presented with severe lameness, swelling, and palpable heat in the right cubital region. Ultrasonographic examination revealed signs suggestive of septic arthritis. Joint lavage and intra-articular injection of antibiotic were performed, but synovial swab cultures were steril…

Frequency of Dosing of Cephalexin for Oral Step-Down Therapy of Pediatric Osteoarticular Infections Caused by Methicillin-Sensitive Staphylococcus Aureus.

Osteoarticular infections are one of the more common invasive bacterial infections encountered in children. There exist significant practice variations in both the diagnosis and treatment of such infections. However, the practice of transitioning from parenteral therapy to oral antibiotics has been well validated by several studies. For methicillin-sensitive Staphylococcus aureus (MSSA), cephalexin is often recommended. Prospective, controlled data regarding optimal dosing of cephalexin in pediatric osteomyelitis are not available. We sought to review our retrospective, uncontrolled data on fo…

Identifying domains for CNO and SAPHO: A scoping review to create domains from existing outcomes by the OMERACT CNO and SAPHO working group.

INTRODUCTION: Chronic nonbacterial osteomyelitis (CNO) and synovitis, acne, pustulosis, hyperostosis, and osteitis (SAPHO) syndrome are autoinflammatory bone diseases of unknown etiology that present with bone pain and varying degrees of extraosseous manifestations such as skin, intestinal and joint involvement. Currently, there are no validated outcome measurement sets that represent the input from all collaborating groups. METHODS: The OMERACT CNO & SAPHO working group performed a scoping review to identify domains previously used in CNO and SAPHO clinical studies. The list of potential dom…

Masquelet Technique Combined with Platelet-Rich Plasma for Large Infectious Bone Defects: A Narrative Review.

Large segmental infected bone defects represent a serious complication frequently encountered following high-energy trauma, bone tumor resection, and osteomyelitis surgery, presenting substantial challenges in orthopedic management. Current clinical approaches such as autologous bone grafting, allogeneic bone grafting, the Ilizarov technique, and artificial bone substitutes each come with inherent limitations. In recent years, the Masquelet technique (induced membrane technique) has become increasingly refined in clinical practice. This method works by establishing a bioactive local microenvir…

Innovative Management of Brodie's Abscess: Continuous Local Antibiotic Perfusion in a 14-Year-Old Patient.

BACKGROUND Brodie's abscess is a rare form of subacute osteomyelitis that primarily affects the metaphyseal regions of long bones. Conventional treatment involves thorough curettage and systemic antibiotic therapy for ≥6 weeks, with bone grafting required for abscess sizes >3 cm. Continuous local antibiotic perfusion (CLAP) delivers high concentrations of antibiotics directly to the infection site and facilitates preservation of biologically active bone tissue. Here, we report a case of an extensive Brodie's abscess that was successfully treated with CLAP, highlighting its potential as an alte…

Orthopedic complications of sickle-cell disease in children.

Sickle cell disease is the most common serious genetic disease in the world. It is a systemic disease, characterized by vaso-occlusive phenomena, especially in the bone capillary network. Orthopedic complications are thus the most common, with a strong impact on quality of life. In children, these complications mainly comprise bone infarction, epiphyseal aseptic osteonecrosis, and bone and joint infection: acute or chronic osteomyelitis, osteoarthritis and septic arthritis. Aseptic osteonecrosis mainly involves the proximal extremities of the femur and humerus. It may be completely asymptomati…

Dilemma with implant placement in patients with florid cemento-osseous dysplasia: A literature review.

METHODOLOGY: An electronic search was done in PUBMED, SCOPUS, and a hand search was done in radiology, periodontology, and oral surgery journals. The search yielded 428 results, from which only 6 articles were selected for this literature review. Both prospective and retrospective studies were included. Clinical studies with information on the pre-implant condition of the site, detailed implant procedure, and follow-up after implant placement of more than 6 months were only considered for this review. RESULTS AND CONCLUSION: Limited clinical studies, shorter follow-up periods were the shortco

Computed tomographic features of severe horn infection in a male Scimitar-horned oryx (Oryx dammah).

A 3-year-old male Scimitar-horned oryx presented for evaluation of an acutely deformed right horn with right head tilt and right facial pain. Computed tomographic evaluation revealed an increased volume of central fluid/soft tissue attenuation with gas-attenuating foci within the right horn. The right horn was amputated at the right horn base. Imaging and histopathologic features were consistent with emphysematous osteomyelitis. Following treatment, the patient returned to normal behavior. This is the first veterinary report describing the computed tomographic features of a horn infection in a…

Papineau-irrigation technique: an alternative treatment of fracture-related infectious soft tissue defects.

The original Papineau technique described satisfactory results in treating infection-related pseudarthrosis and chronic osteomyelitis with chronic draining wounds. We described our experience in treating these soft tissue defects using the Papineau-irrigation technique. We retrospectively reviewed the records of patients that were treated with the Papineau-irrigation technique at UZ Leuven, Belgium, between January 2006 and January 2023. All surgical procedures were performed by one senior orthopedic surgeon. There were no exclusion criteria. There was successful healing of the soft tissue def…

Sequestration and Involucrum: Understanding Bone Necrosis and Revascularization in Pediatric Orthopedics.

Sequestration, a condition where a section of bone becomes necrotic due to a loss of vascularity or thrombosis, can be a challenging complication of osteomyelitis. This review explores the pathophysiology of sequestration, highlighting the role of the periosteum in forming involucrum and creeping substitution which facilitate revascularization and bone formation. The authors also discuss the induced membrane technique, a two-stage surgical procedure for cases of failed healing of sequestration. Future directions include the potential use of prophylactic anticoagulation and novel drugs targetin…
