Wise or wide (WoW) study protocol: a national, multicentre, prospective, randomised and controlled, parallel group, non-inferiority study to compare single-staged versus two-staged excisions of thin invasive (≤1.0 mm) melanoma.

BACKGROUND: Sweden has one of the highest incidence rates of cutaneous melanoma globally, and the incidence is rapidly increasing. Melanoma mortality is linked to the thickness of the primary tumour, with thicker melanomas having a poorer prognosis. Thin invasive melanomas (≤1.0 mm Breslow thickness) have excellent prognosis. Traditionally, the surgical approach for melanoma involves a two-step procedure of a diagnostic excision followed by a wide local excision (WLE) with 10 mm clinical margins. The WLE aims to remove potential microsatellites and residual melanoma, which in theory would prev

Efficacy of the SUPERvised PREhabilitation with Personalised, Aerobic training, Respiratory muscle training and EDucation (SUPER-PREPARED) multi-model in patients with abdominal surgery: protocol for a single-blind, randomised controlled trial.

INTRODUCTION: Globally, millions of abdominal surgeries are performed annually, placing a substantial burden on public health systems worldwide. Postoperative pulmonary complications (PPCs) are common negative consequences after major abdominal surgery that can worsen recovery outcomes. Although the effect of prehabilitation has been frequently investigated in this population, the feasibility and efficacy of delivering a supervised, intensive exercise programme within a limited preoperative period remain underexplored. This randomised controlled trial (RCT) aims to evaluate the effectiveness of

A Group Motivational Interviewing intervention in schools to reduce sugar sweetened beverages intake among young people (DISS): a feasibility study.

INTRODUCTION: Child obesity is a global public health issue affecting high, middle and low-income countries. The causes of obesity are complex and influenced by environmental and behavioural factors including diet and sedentary lifestyles. Young people consume sugar sweetened drinks (SSB) regularly, providing the largest single source of free sugars in the UK. This paper presents the protocol for a cluster feasibility study that aims to assess feasibility of a school obesity intervention delivered by teachers to reduce SSB consumption among 12-13-year-olds in secondary schools to prevent weigh

Social support facilitates threat extinction retention in humans: an fMRI study.

Extinction is a recognized process for extinguishing threat memories; however, the process of extinction can be emotionally challenging, and the effect is usually short-lived. Animal studies have shown that peer presentation can inhibit freezing behavior and prevent return of the threat memories, suggesting that social support may enhance threat extinction. However, the role of social support in maintaining human threat extinction remains unclear. The present study examined the effect of social support on threat memory extinction in humans, and its underlying neural substrates. Succeeding thre

Effect of intraoperative oesophageal pressure-guided PEEP on postoperative pulmonary complications in elderly patients undergoing major laparoscopic surgery: study protocol for a multicentre randomised controlled clinical trial in China.

INTRODUCTION: Postoperative pulmonary complications (PPCs) induced by mechanical ventilation in general anaesthesia are related to poor clinical outcomes. Positive end-expiratory pressure (PEEP) is a key component of the lung protective ventilation strategy. Individualised PEEP guided by end-expiratory oesophageal pressure (Pes) to maintain a positive end-expiratory transpulmonary pressure can avoid alveolar collapse and improve the respiratory mechanics. However, there is no research studying the relationship between the Pes-guided PEEP titration and PPCs. The aim of our study is to investiga

Effects of combined haemodialysis-haemoadsorption therapy in maintenance haemodialysis patients: study protocol for a multicentre, open-label, randomised, parallel controlled trial.

INTRODUCTION: Haemodialysis (HD) is an essential treatment for end-stage renal disease patients to improve their quality of life. However, conventional HD may not effectively remove medium and large molecules and protein-bound toxins, leading to the occurrence of various complications. Haemoadsorption (HA), on the other hand, can address this limitation. Therefore, a multicentre, open-label, randomised, parallel controlled study will be conducted to compare HA combined with HD (HAHD) with HD alone in maintenance haemodialysis (MHD) patients. The primary endpoint is the change in serum β2-MG, P

Effect of time-restricted feeding on cognitive dysfunction in Alzheimer's disease and the role of ApoE polymorphism: protocol for a randomised controlled trial.

INTRODUCTION: With the global ageing population accelerating, the prevalence of Alzheimer's disease (AD) continues to rise annually. However, the underlying mechanisms of AD remain unclear, and effective treatments are still lacking. Apolipoprotein E (ApoE) gene polymorphism and dietary habits are critical risk factors for AD. Time-restricted feeding (TRF), an intermittent fasting strategy that limits the daily window of food intake while maintaining nutritional balance, has garnered significant attention in recent years for its potential to improve cognitive dysfunction. This study aims to in

Effect of ligation sequence of the inferior mesenteric artery and vein on circulating tumour cells and survival in minimally invasive rectal cancer surgery: study protocol for a randomised controlled trial (ARVECTS study).

INTRODUCTION: Total mesorectal excision has been adopted as standard procedure for resectable rectal cancer. However, there is no regulation in the current guidelines on the sequence of ligation of the inferior mesenteric artery and vein during rectal cancer surgery owing to a lack of sufficient evidence. Circulating tumour cells (CTCs) in peripheral blood can be used as potential indicators for predicting postoperative recurrence and prognosis in patients with colorectal cancer. The aim of the study is to investigate whether vascular ligation sequence affects the dissemination of CTCs into th

Neurophysiological alterations during sensory processing in autism - a meta-analysis.

While sensory processing alterations in autism are well-documented, the neurophysiological correlates remain unclear. This meta-analysis examined differences in early event-related potentials (ERP) and event-related fields (ERF) between autistic and non-autistic individuals using electroencephalography and magnetoencephalography to identify neurophysiological alterations that may underlie variations in sensory perception, communication, and social interaction in autism. Following PRISMA guidelines, a database search was conducted for peer-reviewed studies published from January 1980 onwards. Efficacy of soothing cream gel in the range of motion and chronic pain at the shoulder and elbow: protocol of a double-blinded, randomised, placebo-controlled trial.

INTRODUCTION: Upper limb problems have a significant impact on the global population leading to pain and restricted joint mobility, ultimately impacting their quality of life. Traditional treatments, such as non-steroidal anti-inflammatory drugs and corticosteroids, often come with undesirable side effects, prompting patients to seek alternative therapies. In this trial, we hypothesise that soothing cream gel (SCG) will improve range of motion and chronic pain in the shoulder and elbow. The objective of this trial is to evaluate the efficacy of SCG in improving the range of motion and chronic

Impact of low-dose sevoflurane with propofol-based anaesthesia on motor-evoked potentials in infants: protocol for a single-centre randomised controlled study.

INTRODUCTION: Motor-evoked potentials (MEP) are widely used to mitigate the risk of nerve injury resulting from surgical manipulation. Infants are more susceptible to anaesthetics that suppress MEP because of the immaturity of their nervous structures. Current evidence of the impact of the interaction between a small dose of sevoflurane and propofol-based total intravenous anaesthesia (TIVA) on MEP in infants is controversial. This current study aims to evaluate the impact of the coadministration of low-dose sevoflurane with propofol-based TIVA on MEP in infants. METHODS AND ANALYSIS: A rando

Efficacy of reminiscence therapy with different media on cognitive function and negative moods for older adult patients who had a stroke: protocol of a network meta-analysis.

INTRODUCTION: Stroke is a common cause of death and disability in the older adult and increases the risk and severity of cognitive impairment, which is a factor for long-term death among stroke survivors. Some studies have focused on the effects of reminiscence therapy with different media on stroke survivors. It is currently unclear which is the best medium. This protocol aims to deal with this problem by using a network meta-analysis. METHODS AND ANALYSIS: Published randomised controlled trials will be included if reminiscence therapy plus usual care was applied in older adult patients who

Correlation between bilateral TMJ MRI findings: A systematic review of the literature.

The correlation between magnetic resonance imaging (MRI) signs and clinical findings has been highlighted in multiple studies. However, very little information is available on the correlation between the bilateral temporomandibular joints (TMJs) of the same individual. The majority of efforts in the clinical research setting have focused on the correlation between ipsilateral imaging and clinical findings, while less attention has been paid to the contralateral imaging findings of the anatomical structures. The objective of this paper was to review the existing literature that compares temporom

Comparing the antireflux effect of laparoscopic proximal gastrectomy with double-flap technique reconstruction versus laparoscopic total gastrectomy with Roux-en-Y reconstruction for proximal early gastric cancer: study protocol for a multicentre, prospective, open-label, randomised controlled trial.

INTRODUCTION: Laparoscopic proximal gastrectomy with double flap technique (LPG-DFT) reconstruction has been used for proximal early gastric cancer in recent years. However, its feasibility and safety remain uncertain, as only a few retrospective studies have contained postoperative complications and long-term survival data. LPG-DFT for proximal early gastric cancer is still in the early stages of research. Large-scale, prospective randomised controlled trials (RCTs) are necessary to assess the value of LPG-DFT for proximal early gastric cancer. METHODS AND ANALYSIS: This study is a multicent
