[A case of Wernicke's encephalopathy with detrusor-sphincter dyssynergia].

We report a case of a 54-year-old man with alcoholic Wernicke's encephalopathy. Neurological examination showed unconsciousness, absence of the oculocephalic reflex, generalized hyporeflexia, and urinary retention. The patient immediately regained consciousness after the administration of thiamine, but amnesia and cerebellar ataxia became apparent. The urinary retention persisted, and an urodynamic study showed detrusor-sphincter dyssynergia. Three months after the treatment, the urinary retention resolved, and a second urodynamic study showed disappearance of the detrusor-sphincter dyssynergi…

Detrusor Underactivity: The Current Concept of the Pathophysiology.

Based on evidence from available literature, we review the pathophysiology of detrusor underactivity (DU). DU is likely to be multifactorial. Aging reduces detrusor activity, but other concomitant causes may aggravate this condition, resulting in decrease of detrusor contractility. Impaired detrusor contractility has been regarded as a major etiologic factor of DU. However, a more complex pathology has been proposed. As contributing factors to DU, we discuss disturbances of the sensory afferent side of the micturition reflex, the central nervous system (CNS) and the efferent side of the reflex…

Evaluation of in vivo and in vitro biological activities of different extracts of Cuscuta arvensis.

In the present study, the potential effects of extracts from the whole plant of Cuscuta arvensis were studied in mice using the carrageenan-induced hind paw edema model for antiinflammatory activity and the p-benzoquinone-induced writhing reflex for the assessment of antinociceptive activity. In order to obtain the extracts, the whole plant of C. arvensis was extracted with different solvents such as n-hexane, dichloromethane, ethyl acetate, methanol and distilled water. Antioxidant activity was evaluated by the 2,2-diphenyl-1-picrylhydrazyl (DPPH) radical scavenging assay. The methanolic and …

TRP channel antagonists as potential antitussives.

Cough is a troublesome symptom associated with many respiratory diseases. In some instances cough can become prolonged and excessive, and chronic cough of various aetiologies is a common presentation to specialist respiratory clinics. However, current treatment options are limited. Despite its importance, our understanding of the mechanisms that provoke cough is poor. Recent investigation has focused on the interaction between G-protein-coupled receptors and ion channels expressed on airway sensory nerves that are responsible for driving the cough reflex. In particular, the Transient Receptor …

Personal experiences with vestibular evoked myogenic potentials as a modern method of diagnosing vestibular organ lesion and monitoring treatment.

Vestibular evoked myogenic potentials (VEMPs) appear to represent a new and promising technique for the assessment of vestibulospinal reflex function. The primary aims of the study described in this article were (1) to record VEMPs in normal volunteers using available equipment and to establish a range of norms of VEMP parameter values (latency, amplitude); (2) to confirm the saccular origin of VEMPs; (3) to assess the diagnostic significance of VEMPs; and (4) to evaluate the usefulness of VEMPs in monitoring therapeutic results. The study population consisted of 252 patients representing vari…

Spasticity: the misunderstood part of the upper motor neuron syndrome.

Spasticity is a sensorimotor phenomenon related to the integration of the nervous system motor responses to sensory input. Although most commonly considered a velocity-dependent increase to tonic stretch, it is related to hypersensitivity of the reflex arc and changes that occur within the central nervous system, most notably, the spinal cord. Injury to the central nervous system results in loss of descending inhibition, allowing for the clinical manifestation of abnormal impulses. Muscle activity becomes overactive. This is mediated at several areas of the stretch-reflex pathway. Although spa…

[Pathophysiology of spasticity].

Lance's definition of spasticity focuses on the exaggeration of the tonic stretch reflex as one component of the upper motor neuron syndrome. In daily practice, many different symptoms are referred to as spasticity. Experimental studies stress the particular role of the premotor cortex and the medial reticular formation for the genesis of spasticity. Physiological studies clearly demonstrate the two components, phasic and tonic, of the stretch reflex. Whatever the pathology, the clinical picture of spasticity seems to depend less upon the etiology of the lesion and more upon its location in th…

Electrophysiological evidence for trigeminal neuron sensitization in patients with migraine.

The electrically elicited corneal reflex is a useful tool for exploring the trigeminal system in humans and it may provide additional evidence pointing to a dysfunction of this system in migrainous patients. Tactile perception, corneal reflex and pain thresholds were studied in 48 migraine without aura patients during pain-free periods and compared with those observed in 24 controls. Twenty-eight of the patients had strictly unilateral headache, while the other 20 had bilateral or side-shifting pain during attacks. Both migraine subgroups (bilateral and unilateral) showed significantly lower t…

Trigeminal autonomic cephalgias (TACs).

The concept of a group of headaches whose pathophysiological focus revolves around the trigeminal-autonomic reflex fills a useful gap in characterising a number of primary headache syndromes. Broadly, these syndromes involve activation of trigeminovascular nociceptive pathways with reflex cranial autonomic activation. Clinically, this physiology predicts pain with some combination of lacrimation, conjunctival injection, nasal congestion, or eyelid oedema. Several of the primary neurovascular headaches, notably cluster headache, paroxysmal hemicrania and short-lasting neuralgiform pain with con…

Isolated cranial nerve palsies due to brainstem lesions.

Isolated cranial nerve palsies are often attributed to lesions of the respective nerves along their extraaxial courses. There are a significant number of reports of individual patients with cranial nerve palsies, mostly of the 3rd and 6th nerves, as the sole manifestation of brainstem lesions proven by magnetic resonance imaging (MRI) or computer-assisted tomography (CT). An intraaxial basis may still be underestimated if based on MRI only, as electrophysiological abnormalities indicating brainstem lesions (masseter reflex, blink reflex, DC electrooculography) may be independent from MRI-docum…

Eyelid changes in long-standing leprosy.

To describe eyelid changes in ocular leprosy, 74 patients (148 eyes or 296 eyelids) were examined, focusing on eyelid abnormalities. The adnexal examination included evaluation of the upper eyelid crease pattern, qualitative assessment of the orbicularis oculi muscle function, measurement of the distance between the corneal reflex and the upper eyelid margin (margin reflex distance), and slit-lamp biomicroscopy of the eyelashes and tarsal conjunctiva. Eyelash ptosis was a common finding associated with a multiple upper eyelid crease pattern and trichiasis. In the past, eyelash ptosis has proba…

A new treatment for premature ejaculation: the rehabilitation of the pelvic floor.

This study evaluated pelvic floor rehabilitation as a possible treatment for premature ejaculation. In this treatment it is assumed that the pelvic muscles are involved in the control of the ejaculatory reflex. The treatment avails itself of a method already used for fecal and urinary incontinence. Eighteen patients with premature ejaculation were recruited. Fifteen (83%) of them had suffered from this disturbance for at least five years. Most of them had experienced other therapies without success. After 15-20 sessions of pelvic floor rehabilitation, 11 (61%) patients were cured and are able …

Pharmacological profile of the novel putative anxiolytic agent 1-amino-5-bromouracil.

The newly synthesized compound, 1-amino-5-bromouracil (ABU, CAS 127984-93-4), showed unique anxiolytic activity in rats and mice. Its minimum effective dose was 10 mg/kg p.o. in the Geller type conflict test in rats, and it showed anxiolytic activity at a dose of 20 mg/kg i.p. in the Vogel type conflict test in mice. ABU also induced loss of the righting reflex in mice. The ED50, a dose level that induces loss of the righting reflex in 50% of mice, was 86.4 mg/kg p.o. On the other hand, ABU showed only weak activities both potentiating the drug-induced anesthesia and myorelaxing in comparison …

Effects of contralateral acoustic stimulation on otoacoustic emissions following vestibular neurectomy.

This study demonstrates that, following unilateral vestibular neurectomy, the inhibitory effect of contralateral acoustic stimulation on evoked otoacoustic emissions is absent. The patient acts as her own control in that the unoperated side shows normal suppression of otoacoustic emission amplitude with contralateral acoustic stimulation. The lack of interaural suppression of otoacoustic emissions on the sectioned side, in the presence of normal acoustic reflex threshold levels, provides evidence that observed phenomena are not merely a function of middle ear reflex activity. It is concluded t…

Resinferatoxin, an ultrapotent capsaicin analogue, has anti-emetic properties in the ferret.

Resinferatoxin (100 micrograms/kg, s.c.), the ultrapotent analogue of capsaicin, when given acutely blocked radiation-(200 rads) and copper sulphate (40 mg% 30 ml, p.o.)-induced emesis in ferrets and substantially decreased loperamide (0.5 mg/kg, s.c.)-induced vomiting, without significantly affecting the von Bezold-Jarisch reflex or gag reflex. It also produced a decrease in core temperature as has been reported for capsaicin. The observation that resinferatoxin reduced or blocked emesis induced by both centrally (loperamide) and peripherally (CuSO4, radiation) acting stimuli suggests a novel…

Autonomic hyperreflexia with spinal cord injury.

Autonomic hyperreflexia occurs in up to 85 percent of individuals with spinal cord injuries above the major splanchnic sympathetic outflow. In such cases, paroxysmal reflex sympathetic activity develops in response to noxious stimuli below the level of the neurologic lesion. The clinical features of autonomic hyperreflexia are due largely to reflex sympathetic adrenergic and cholinergic discharges with dysfunctional supraspinal regulatory control. Cephalgia, diaphoresis, flushing, tachycardia or bradycardia, and paroxysmal hypertension are most commonly observed. Although a variety of stimuli …

Complications of ankle arthroscopy.

Of over 4500 ankle arthroscopies recently surveyed by the Arthroscopy Association of North America, only one complication (an infection) was reported. Nerve injury, vascular injury, or reflex sympathetic dystrophy were not reported. The ankle's anatomy is complex and there are many structures that can be damaged by imprecise portal placement. A review of our experience with 53 consecutive ankle arthroscopies found nine complications for a rate of 17%. There were three permanent dorsal sensory nerve injuries, two synovial fistulas, three wound infections, and one reflex sympathetic dystrophy. A…
