Frailty is associated with increased complication, readmission, and hospitalization costs following primary total shoulder arthroplasty.

BACKGROUND: Frailty is an age-related state of multisystem decline that has been associated with negative outcomes after surgery. Numerous methods have been utilized to quantify frailty and predict postoperative outcomes with variable results. The purpose of this study is to determine if the Hospital Frailty Risk Score (HFRS) is an independent predictor of postoperative complications and increased healthcare costs following elective primary total shoulder arthroplasty (TSA). METHODS: The Nationwide Readmissions Database was queried to select patients undergoing elective primary TSA from 2016

A preoperative risk assessment tool for predicting adverse outcomes among total shoulder arthroplasty patients.

BACKGROUND: With the increased utilization of Total Shoulder Arthroplasty (TSA) in the outpatient setting, understanding the risk factors associated with complications and hospital readmissions becomes a more significant consideration. Prior developed assessment metrics in the literature either consisted of hard-to-implement tools or relied on postoperative data to guide decision-making. This study aimed to develop a preoperative risk assessment tool to help predict the risk of hospital readmission and other postoperative adverse outcomes. METHODS: We retrospectively evaluated the 2019-2022(Q

Impact of antiplatelet and anticoagulant drugs on postoperative bleeding in reverse total shoulder arthroplasty.

BACKGROUND: The decision to withdraw or continue antiplatelet and anticoagulant drugs would be balanced on the bleeding risk and the cardiovascular risk. The purpose of this study was to investigate perioperative bleeding in reverse shoulder arthroplasty (RSA) to determine the impact of oral antiplatelet and anticoagulant medications in the absence of drug withdrawal. The hypothesis was that the continuation of antiplatelet and anticoagulant drugs would increase postoperative bleeding but to a limited extent. METHODS: During the study period, RSA cases were prospectively included and retrospe

Randomized clinical trial of zirconia and polyetheretherketone implant abutments for single-tooth implant restorations: A 5-year evaluation.

STATEMENT OF PROBLEM: Titanium-supported polyetheretherketone (PEEK) abutments provide an economic alternative to zirconia abutments in esthetically important areas. Research comparing the performance regarding esthetics, longevity, and biologic parameters of PEEK abutments is lacking. PURPOSE: The purpose of this clinical study was to determine whether PEEK implant abutments provide similar esthetic and biologic parameters and survival rates as zirconia implant abutments. MATERIAL AND METHODS: Forty participants (age 20 to 50 years) receiving maxillary anterior and premolar implants were en

Age, participation in competitive sports, bony lesions, ALPSA lesions, > 1 preoperative dislocations, surgical delay and ISIS score > 3 are risk factors for recurrence following arthroscopic Bankart repair: a systematic review and meta-analysis of 4584 shoulders.

PURPOSE: Determining the risk of recurrent instability following an arthroscopic Bankart repair can be challenging, as numerous risk factors have been identified that might predispose recurrent instability. However, an overview with quantitative analysis of all available risk factors is lacking. Therefore, the aim of this systematic review is to identify risk factors that are associated with recurrence following an arthroscopic Bankart repair. METHODS: Relevant studies were identified by searching PubMed, Embase/Ovid, Cochrane Database of Systematic Reviews/Wiley, Cochrane Central Register of

[Biomechanical comparative analysis of screw fixation and cortical endo-button fixation in Latarjet procedure for anterior dislocation of shoulder joint].

OBJECTIVE: To clarify the value of the cortical endo-button as an internal fixator in Latarjet procedure through biomechanical analysis. METHODS: Ten pairs of shoulder joints from 6-7 months old male pigs were selected. Each pair was randomly divided into screw group and endo-button group. A 25% glenoid defect model was created, and the porcine infraspinatus tendon and its associated bone were used to simulate conjoint tendon and coracoid process in human body. The bone grafts were fixed with two 3.5 mm screws and double cortical endo-buttons with high-strength sutures in screw group and endo

Acromioclavicular Joint Stabilization: A Biomechanical Study of Bidirectional Stability and Strength.

BACKGROUND: It is important to restore horizontal and vertical stability to the acromioclavicular (AC) joint when treating dislocations of this joint. Most surgical stabilization techniques of the AC joint have primarily addressed the coracoclavicular ligament complex; however, these techniques may not satisfactorily restore horizontal stability to the AC joint. PURPOSE: To evaluate the strength and bidirectional stability of 3 AC joint stabilizing techniques in a cadaveric model. STUDY DESIGN: Controlled laboratory study. METHODS: A total of 24 cadaveric shoulders were randomly allocated t

Sternoclavicular Joint Graft in Temporomandibular Joint Reconstruction for Ankylosis.

BACKGROUND: Temporomandibular joint ankylosis is one of the most distressing clinical condition resulting in loss of jaw function and impairment of growth. Ankylosis is a greek word meaning "Stiff Joint". It is observed that in humans the Sternoclavicular joint and Temporomandibular joint are similar morphologically and histologically as they are the only two synovial joints covered with fibrocartilage. This similarity of the joints have encouraged the surgeons to use SCG as an alternative to costochondral grafts. AIMS: The purpose of this article was to evaluate the feasibility of using ster

Orthopaedic issues in the musculoskeletal care of adults with cerebral palsy.

AIMS: Orthopaedic care of adults with cerebral palsy (CP) has not been well documented in orthopaedic literature. This paper focuses on some of the common problems which present themselves when adults with CP seek orthopaedic intervention. In particular, we review the most common orthopaedic issues which present to the Penn Neuro-Orthopaedics Program. METHOD: A formal review of consecutive surgeries performed by the senior author on adults with CP was previously conducted. This paper focuses on the health delivery care for the adult with orthopaedic problems related to cerebral palsy. Ninety-

Seat belt associated central line fracture--a previously unreported complication in cystic fibrosis.

Totally implantable venous access devices (TIVAD) are used widely in the management of cystic fibrosis (CF) [Standards of Care: Standards for the clinical care of children and adults with cystic fibrosis in the UK. The Cystic Fibrosis Trust's Clinical Standards and Accreditation Group. Cystic Fibrosis Trust, London, UK, 2001]. They have been shown to be safe and advantageous in the long term administration of intravenous antibiotics [A-Rahman A, Spencer D. Totally implantable vascular access devices for cystic fibrosis. Cochrane Database Syst Rev 2003(3)]. However, TIVADs are not without short

Superior Baseplate Inclination Is Associated With Instability After Reverse Total Shoulder Arthroplasty.

BACKGROUND: Instability is the most common complication after reverse total shoulder arthroplasty (rTSA). In the native glenohumeral joint, in addition to full dislocations, more subtle forms of instability exist. However, the incidence of more subtle forms of instability, the factors associated with instability, and the effect of instability on validated outcome scores after rTSA remain poorly understood. QUESTIONS/PURPOSES: (1) After rTSA, what is the risk of instability, including more subtle forms of subjective instability? (2) What are the factors associated with instability? (3) Are mor

Is early mobilization after volar locking plate fixation in distal radius fractures really beneficial? A meta-analysis of prospective randomized studies.

STUDY DESIGN: This was a systematic review with a meta-analysis. INTRODUCTION: Despite rising trends toward surgical treatment of distal radius fractures (DRF) with volar locking plate (VLP) fixation, there is a lack of consensus on when to start vigorous wrist range of motion (ROM) exercises after surgery. PURPOSE: We performed a meta-analysis to compare early and late mobilization after VLP fixation in patients with DRF. METHODS: Four prospective randomized controlled trials with a minimum of 6 months of follow-up were retrieved through MEDLINE (PubMed), EMBASE, Web of Science, the Cochra

Case studies in the diagnosis of upper extremity pain using magnetic resonance imaging.

The diagnosis of upper extremity pain can be a daunting task for the examining physician. The differential diagnosis for pain at each level in the upper extremity is often extensive and diverse. Quite frequently, the clinical signs and symptoms of multiple pathologic conditions are overlapping and nonspecific. The decision to treat conservatively or with surgery often depends on the magnetic resonance imaging (MRI) findings. MRI's ability to produce multiplanar images with excellent soft tissue contrast has made it an indispensable tool for diagnosis and treatment planning in patients with upp
