The effect of shear wave elastography in the diagnosis of delaminated partial-thickness rotator cuff tears.

AIM: To assess the effectiveness of shear wave elastography (SWE) in diagnosing delaminated partial-thickness rotator cuff tears (DPT-RCT). MATERIAL AND METHODS: A retrospective study was carried out on 137 patients with DPT-RCT. The study included complete clinical data, including the images of conventional ultrasound (US), SWE, Magnetic Resonance Imaging (MRI) and shoulder arthroscopic surgery. The features of US, SWE, and MRI were evaluated. The study analysed the Shear-Wave Velocity (SWV) among three types of DPT-RCT, and between the regions of tears, normal contralateral, and affected un

Assessment of approaches to estimate scapular orientation in children with brachial plexus birth injury.

BACKGROUND: Challenges in measuring dynamic scapular orientation limit assessment of scapulothoracic and glenohumeral contributions to shoulder function in children with brachial plexus birth injury (BPBI). Double calibration acromion marker cluster (D-AMC) and linear model approaches have been validated to estimate scapular motion in healthy adults, but neither has been evaluated in BPBI. RESEARCH QUESTION: Are the linear model and D-AMC approaches able to accurately estimate scapular orientation in children with BPBI at functional arm postures? METHODS: Seventeen children with BPBI positio

Pneumothorax, an Uncommon but Devastating Complication following Shoulder Arthroscopy: Case Reports.

Shoulder arthroscopy is a mature and widely used treatment to deal with various shoulder disorders. It enables faster recovery and decreases postoperative complications. However, some complications related to shoulder arthroscopy cannot be neglected because they could be life threatening. We presented three cases of various clinical manifestations of pneumothorax after shoulder arthroscopy. The first case was a 65-year-old female who underwent arthroscopic rotator cuff repair under general anesthesia and interscalene nerve block in the beach-chair position. The second case was a 58-year-old ma

A multicenter randomized controlled trial comparing gamification with remote monitoring against standard rehabilitation for patients after arthroscopic shoulder surgery.

BACKGROUND: Gamification has become increasingly popular in rehabilitation and is viewed as a tool to improve patient activation, motivation, and engagement. The aim of this study was to compare the efficacy of validated exergames played through a system using "depth sensor" and bespoke software against standard physiotherapy in patients treated with arthroscopic shoulder surgery. This included the following common conditions: subacromial impingement syndrome, calcific tendinopathy, and rotator cuff tear. METHODS: Following arthroscopic shoulder surgery, patients were randomized into 1 of 2 g

Isolated Anterior Interosseous Neuropathy Affecting Only the Flexor Digitorum Profundus to the Index Finger After Shoulder Arthroscopy: A Case Report and Review of the Literature.

Anterior interosseous nerve neuropathy is an uncommon neuropathy with multiple potential etiologies. We present a rare case of anterior interosseous nerve neuropathy affecting only the flexor digitorum profundus to the index finger and occurring after shoulder arthroscopy. This unique presentation used a combination of both electrodiagnostic testing and neuromuscular ultrasound to obtain an accurate diagnosis and highlights the importance of these complementary tests in the evaluation of nerve disorders. To our knowledge, anterior interosseous nerve neuropathy after shoulder arthroscopy affect

Palliative shoulder and elbow surgery in obstetrical brachial plexus birth palsy.

Palliative surgery in a child with incomplete recovery following obstetric brachial plexus birth palsy (BPBP) is common. Surgical management strategies for BPBP sequelae have the common objectives of decreasing the risk of functional limitations in the long term and improving function. There is no single treatment to deal with the sequelae of BPBP. While there is a myriad of possible clinical presentations, the ages for surgery extend from a 6- to 12-month-old infant to the mature adolescent. Numerous procedures have been described in the literature, ranging from simple soft tissue release to

Patient gender and rotator cuff surgery: are there differences in outcome?

BACKGROUND: Although rotator cuff syndrome is common and extensively studied from the perspective of producing healed tendons, influence of gender on patient-reported outcomes is less well examined. As activity and role demands may vary widely between men and women, clarity on whether gender is an important factor in outcome would enhance patient education and expectation management. Our purpose was to determine if differences exist in patient-reported outcomes between men and women undergoing rotator cuff surgery. METHODS: One hundred forty-eight participants (76 W:72 M) aged 35-75 undergoin

Editorial Commentary: Mental Health Comorbidities Are Associated With Increased Cost, Opioid Use, and Inferior Outcomes After Shoulder Rotator Cuff Repair.

In patients undergoing rotator cuff repair (RCR), patient comorbidities including mental health diagnoses of anxiety and depression are very common and are known to adversely affect surgical outcomes. This study highlights the complex interplay of mental health with other important considerations in outcomes following RCR, namely opioid use and health care costs. The extent to which we can mitigate opioid use and health care costs in patients with comorbid mental health disorders undergoing RCR remains unclear but worthy of concerted efforts to improve patient outcomes. Improving recognition o

Outcomes after rotator cuff repair in the elderly as assessed by the American Shoulder and Elbow Surgeons shoulder score.

Rotator cuff tears are common among the elderly, and studies on the outcomes after rotator cuff repair in the elderly are limited. We carried out this meta-analysis with systematic literature search, aiming to clarify the outcomes after rotator cuff repair in the elderly as assessed by the American Shoulder and Elbow Surgeons (ASES) shoulder score. We conducted a literature search through October 2019 in PubMed and EMBASE databases and performed meta-analysis to calculate the summary mean difference comparing the post- and pre-operation ASES scores under both fixed-effect and random-effect mod

The Rotator Cuff and the Superior Capsule: Why We Need Both.

Tears of the rotator cuff are frequent. An estimated 250,000 to 500,000 repairs are performed annually in the United States. Rotator cuff repairs have been successful despite fatty infiltration and atrophy of the rotator cuff muscles. Although the emphasis in rotator cuff repair has historically focused on re-establishing the tendon attachment, there is growing interest in and understanding of the role of the superior capsule. The superior capsule is attached to the undersurface of the supraspinatus and infraspinatus muscle-tendon units, and it resists superior translation of the humeral head.

Fracture of the scapular neck sustained in an arm wrestling match.

The sport of arm wrestling requires very little equipment, and can take place anywhere a flat surface is available. As such, undertrained participants often compete, with inevitable injury. Humeral fractures, and elbow injuries are well described, but scapular fractures have not previously been reported in the literature.

Proximal humerus shaft fracture after pectoralis major tendon rupture repair.

Surgical repair of a complete pectoralis major tendon rupture at the humeral insertion has superior results compared to nonoperative treatment. To our knowledge, a proximal humerus shaft fracture occurring at the site of the bone trough and cortical drill holes after a pectoralis major tendon rupture repair has not been reported in the literature.A 45-year-old man sustained an acute left pectoralis major tendon rupture at the humeral insertion while performing a bench press maneuver. He underwent acute surgical repair. Approximately 8 weeks postoperatively, the patient fell from a standing hei

Fluid temperatures during arthroscopic subacromial decompression using a radiofrequency probe.

Our aim was to investigate the temperatures reached in the subacromial space during radiofrequency ablation of the subacromial bursa, in order to see if the probes generate sufficiently high temperatures to cause chondrolysis of the articular surfaces in the glenohumeral joint. We recorded the maximum temperatures in the subacromial space during arthroscopic subacromial decompression on 30 consecutive patients using a sterile digital temperature probe (Series 400, DeRoyal, TN, USA). Both the mean (27.8 degrees C) and maximum (41.8 degrees C) temperatures reached were below the experimental thr
