Enhancing evidence-based care using trial emulation in electronic health records: real-world effects of empagliflozin in people with type 2 diabetes.

BACKGROUND: There is growing interest in widening the use of sodium-glucose co-transporter 2 inhibitors (SGLT2i) to all people with type 2 diabetes mellitus (T2DM). However, pivotal randomized controlled trials (RCTs) evaluated these drugs only in highly selected populations, often lacking generalizability to real-world populations. Understanding the effects of SGLT2i in populations where RCT evidence may be lacking is essential to help inform guideline development. To address this, we estimated the effect of empagliflozin in real-world users, many of whom would not have been eligible for the

To investigate the effect of a high-fat diet on pharmacokinetics/renal function/RAAS-related parameters after a single dose of empagliflozin in healthy Chinese adults.

OBJECTIVES: Food intake affects pharmacokinetic (PK) parameters of empagliflozin (EMPA). In the treatment of chronic kidney disease, EMPA in combination with renin-angiotensin-aldosterone system (RAAS) inhibitors is widely used. This study aims to investigate how a high-fat diet (HFD) affects the PK parameters, pharmacodynamic (PD) parameters related to renal function, and PD parameters related to RAAS of EMPA. METHODS: Based on a bioequivalence study of healthy Chinese adults, twenty blood sampling points were set up for each participant before EMPA and within 48 hours after 10 mg EMPA admini

eGFR slope improvement with SGLT2 inhibitors is not statistically associated with changes in urinary protein in Japanese CKD patients: a single-center real-world analysis.

BACKGROUND: SGLT2 inhibitors reduce urinary protein and slow eGFR decline in clinical trials. However, real-world evidence on within-patient changes in eGFR slope before and after treatment initiation, and their relationship to changes in urinary protein, remains limited. This study was designed to address these concerns. METHODS: This retrospective study included 90 Japanese CKD patients who initiated dapagliflozin and had ≥ 5 eGFR measurements during both the 12-month periods before and after initiation. The treatment effect was defined as the ΔeGFR slope (post-minus pre-initiation slope).

Modulation of Amyloid and Tau Pathology by Empagliflozin in a High-Fat Diet and Streptozotocin-Induced Type 2 Diabetes-Associated Alzheimer's Model.

Type 2 diabetes mellitus (T2DM) contributes notably to the development and progression of Alzheimer's disease (AD) through overlapping pathological mechanisms such as insulin resistance, amyloid-β (Aβ) accumulation and tau hyperphosphorylation. Sodium-glucose cotransporter-2 (SGLT2) inhibitors have recently emerged as promising candidates for neuroprotection in metabolic disorders. The present work explored the potential therapeutic impact of the SGLT2 inhibitor empagliflozin (EMPA) compared with the acetylcholinesterase inhibitor rivastigmine (RIV) in a T2DM-induced AD rat model. The T2DM-AD

Dapagliflozin Attenuates Pericardial Adipose Tissue-Derived Leptin-Mediated Myocardial Remodeling in Obese Rats.

BACKGROUND: The favorable cardiovascular effects of sodium-glucose co-transporter 2 inhibitors in diabetes are well established, but their potential to ameliorate obesity-related cardiac dysfunction remains unclear. Pericardial adipose tissue (PAT)-derived adipocytokines, particularly leptin, contribute to obesity-associated cardiac remodeling. We investigated the detrimental effects of PAT-derived leptin on cardiac remodeling and whether dapagliflozin confers cardioprotection by reducing and counteracting PAT-derived leptin in obese rodents. METHODS: Male Wistar rats, C57BL/6J mice, and ob/o

Comparative effectiveness of alternative second-line oral glucose-lowering therapies for type 2 diabetes: a precision medicine approach applied to routine data.

AIMS/HYPOTHESIS: National clinical guidelines recommend that second-line treatment for type 2 diabetes mellitus is chosen according to individuals' characteristics but there is limited evidence available to inform this choice. This paper's aim is to compare the effects on HbA1c of sulfonylureas (SU), dipeptidyl peptidase-4 inhibitors (DPP4i) or sodium-glucose cotransporter-2 inhibitors (SGLT2i) added to metformin as second-line oral glucose-lowering treatments according to an individual's age, baseline HbA1c and presence of multiple long-term conditions (MLTCs). Selective sodium-glucose cotransporter two inhibitor empagliflozin ameliorates diabetic cardiomyopathy by activating the AMPK/TFEB signaling pathway.

The highly selective SGLT2 inhibitor (SGLT2i) is reported to have beneficial effects on diabetic cardiac hypertrophy; however, the molecular mechanisms underlying the cardioprotection of SGLT2i are not fully understood. In this study, we investigated the impact of the SGLT2 Inhibitor empagliflozin (EMPA) on diabetic hearts and its regulatory mechanisms in high-fat-diet (HFD)- and streptozotocin (STZ)-treated rats. Male rats orally administered HFD/STZ treatment for eight weeks, with or without EMPA (10 mg/kg), were used as our in vivo model. Hematoxylin and eosin (H&E) staining was used for hi
