Superior labrum-biceps tendon complex lesions of the shoulder.

The detachment of the superior labrum from anterior to posterior has previously been reported. This lesion has been classified into four types. It was our impression that not all superior labrum abnormalities fit into such a classification system and that the mechanism of injury was distinctly different. During a 5-year period, 84 of 712 (11.8%) patients had significant labral abnormalities; 52 of 84 patients (6.2%) had lesions that fit within the classification system (Type II, 55%; III 4%; IV, 4%), but 32 of 84 patients (38%) had significant findings that could not be classified. These uncla

Reconstruction of a Proximal Phalangeal Base Volar Fracture-Dislocation Using Distal Tibial Allograft: A Case Report.

CASE: A 65-year-old man presented with an index finger dorsal combined saw injury. Examination showed exposed articular defect of the index finger proximal phalangeal base and extensor lag. The proximal phalangeal base articular defect was reconstructed using distal tibial osteochondral allograft and the tendon injury reconstructed with a turn-down interposition graft. The patient attained a painless functional arc of motion at 17 months postoperatively. CONCLUSION: Proximal phalangeal base fracture-dislocations with partial-articular bone loss pose reconstructive challenges. We present a rar

Mini-Open Achilles Repair With a Flat Braided Suture in a Low-Profile Configuration.

Achilles repair has evolved over the past 30 years, from large open procedures with high complication rates to shorter, less-invasive procedures with better outcomes. Percutaneous repair has comparable failure rates with open repairs, fewer complications, and faster recovery. However, percutaneous Achilles repairs risk sural nerve injury. A mini-open repair fuses the gap between percutaneous and open procedures, and this approach has the potential to mitigate nerve injury while maintaining the increased efficiency in procedure time and patient recovery. The purpose of this Technical Note and a

Severe heterotopic ossification after arthroscopic acromioplasty: a case report.

Heterotopic ossification is a well-recognized complication of spinal cord injury, closed head injury, total hip arthroplasty, burns, and other trauma and has been observed in various tissues such as muscles, tendons, ligaments, and menisci. Complications of arthroscopic acromioplasty are relatively uncommon and include hematoma, traction neuropathy, infection, acromial fracture, reflex sympathetic dystrophy, and instrument breakage. However, little has been reported on heterotopic ossification of the shoulder, particularly after arthroscopic surgery. Recurrent rotator cuff impingement symptoms

Investigation of acute and chronic injury mechanisms of the deltoid ligament: insights from finite element analysis and animal study.

BACKGROUND: The deltoid ligament (DL) is the primary stabilizer of the medial ankle, but its injury mechanisms remain poorly understood. This study aimed to investigate the injury risk and mechanisms of individual DL bundles under both acute and chronic conditions to inform prevention and treatment strategies. METHODS: A validated finite element model of the human foot was used to examine peak stresses in DL bundles under four acute loading scenarios. Chronic loading was simulated by applying gait loads after transecting the lateral ligaments, and the resulting DL stresses were compared with

Surgery to restore upper extremity function in tetraplegia-Preferences for early and frequent access to information.

INTRODUCTION: People with cervical spinal cord injury (SCI) identify improving upper extremity (UE) function as a top priority. In addition to comprehensive rehabilitation, UE surgeries, including nerve and tendon transfers, enhance function. However, barriers exist to disseminating information about surgical options to enhance UE function. OBJECTIVE: To assess the experiences and preferences of people with cervical SCI and their caregivers in accessing information about surgery to enhance UE function. DESIGN: Prospective cohort study. Participants were followed up for 24 months and complete

Surgically Treated Pediatric Hand and Fingers Palmar Wounds Caused by Metal Fences: Analysis Over a 5-Year Period.

BACKGROUND: Hand wounds are frequent in children (31% of hand injuries) and one possible mechanism responsible for profound laceration is metal fences while climbing. These injuries usually require a surgical look to explore soft-tissue damages, but the literature is poor regarding surgical findings and outcomes after this kind of traumatism. The aim of the study was therefore to report a consecutive series of hand and finger wounds caused by metal fences in children, focusing on surgical findings and potential complications. METHODS: All consecutive hand or finger deep wounds caused by metal

Direct repair for managing acute and chronic lateral ulnar collateral ligament disruptions.

PURPOSE: Acute elbow injuries that disrupt the lateral ulnar collateral ligament and result in posterolateral rotatory instability usually require surgical treatment. The 2 technical options reported, direct repair and use of a palmaris longus tendon graft, have usually favored the use of the graft. To balance this emphasis, we report our experience with direct repair of the humeral origin in cases of trauma, whether acute, delayed, or recurrent. It was our hypothesis that because the humeral origin is the point of failure and separation, restoration of this attachment is sufficient to restore

Femoral cross-pin safety in anterior cruciate ligament reconstruction as a function of femoral tunnel position and insertion angle.

PURPOSE: To compare femoral cross-pin guidewire insertion at differing angles to identify "safe zones" relative to saphenous nerve, popliteus tendon, fibular collateral ligament, peroneal nerve, and femoral artery/vein locations between transtibial and medial-portal femoral tunnel drilling methods. METHODS: Five paired cadaveric knees were randomly assigned to a transtibial or medial-portal femoral (anatomic) tunnel drilling group. Guidewires were inserted at differing frontal plane angles (+10°, 0°, -10°, and -20°). Distances between the guidewire and the anatomic structure of interest were

Dispelling the Myth of Work-Related de Quervain's Tenosynovitis.

De Quervain's Tenosynovitis (DQT) is a common condition characterized by pain about the dorsal-radial aspect of the wrist, just proximal to the radial styloid. The condition is precipitated by a thickened first dorsal compartment and its tendons. The impression that DQT is caused from work-related injury is misdirected, as no study has established an association between hand usage at work or trauma with DQT. Physicians should exercise caution when discussing the causes and natural history of DQT with symptomatic patients.

The mangled foot and ankle: soft tissue salvage techniques.

The mangled foot and ankle can be defined as a severe injury to multiple organ systems of this appendage that can include bone, muscle, tendon, and neurovascular structures. The magnitude and often rate-limiting parameter most likely to affect the decision to attempt foot salvage is the degree of soft tissue destruction. If irreplaceable or if function cannot be restored, amputation should be considered. If reconstruction is attempted, all nonviable tissues regardless of intrinsic function must first be removed by meticulous débridement.

MR arthrography of the second and third metatarsophalangeal joints for the detection of tears of the plantar plate and joint capsule.

OBJECTIVE: Tears of the plantar plate and joint capsule of the metatarsophalangeal joint are underrecognized causes of lesser metatarsalgia. This study evaluated MR arthrography for evaluation of this condition in 45 metatarsophalangeal joints in 41 patients. CONCLUSION: Most metatarsophalangeal joint capsular tears occur at the distal lateral border of the plantar plate, often with injury of the adjacent interosseous tendon. Plantar plate tears are less common. MR arthrography of the metatarsophalangeal joint delineates capsular tears and associated abnormalities.

Posterior deltoid-to-triceps tendon transfer to restore active elbow extension in patients with tetraplegia.

Tetraplegia, paralysis of all 4 limbs, is usually the result of cervical spinal cord injury. Recovery of independence is the primary goal of people with tetraplegia; this requires reconstruction of upper extremity function. Active elbow extension, necessary for a wide range of activities of daily living, is usually surgically reconstructible. The posterior deltoid-to-triceps tendon transfer was developed to restore active elbow extension; good functional results and high levels of patient satisfaction have been reported for this operation.

Poor posture subjects a worker's body to muscle imbalance, nerve compression.

Forward head and shoulder postures and associated muscle imbalances are prevalent among today's workers, according to ongoing research in the physical therapy field. Information on proper posture should be used proactively to educate employees as part of injury prevention training. Maintaining muscle strength in the desirable posture will require ongoing exercise and attention from the worker. The self-direct approach will not eliminate all cases of tendonitis, epicondylitis and other disorders, but it will have a significant positive impact.

Outcomes of Zone II Flexor Tendon Repair Under General Versus Wide Awake Local Anesthesia: A Randomized Controlled Trial.

PURPOSE: Wide awake local anesthesia no tourniquet (WALANT) is gaining popularity with flexor tendon repair. We hypothesized that results of zone II flexor tendon repair performed under WALANT would be superior to those performed under general anesthesia (GA). METHODS: A randomized controlled trial was conducted to compare results of repair of zone II flexor tendon lacerations under WALANT versus GA. Following sample size estimation, 86 digits were included and randomized into two groups. All surgeries were performed by a single surgeon using a six-stranded core stitch and running epitenon su

Advancements in percutaneous fixation for foot and ankle trauma.

Operative fixation of foot and ankle trauma can be challenging. Often times, the soft tissue envelope can have extensive damage as a result of the fracture. In these cases, percutaneous fixation may be used. Percutaneous fixation can benefit both soft tissue and osseous healing when used correctly. Many techniques have been described in the literature that may help to preserve blood supply, minimize soft tissue dissection, and restore a functional limb. This article reviews general guidelines for fracture and soft tissue management, osseous healing of fractures, and how certain techniques infl

[Histological and electron microscopic study of rotator cuff tear].

OBJECTIVE: To understand the changes of rotator cuff during tear and observe the pathologic and ultramicroscopic structure of rotator cuff. METHOD: Histological and ultra thin slides of samples of 36 cases of rotator cuff were made. RESULT: The degeneration of rotator cuff happened in synovium side, and then extended to the middle and deep sides of tendon. The deep side of cuff tendon could degenerate without synovium. The repair of tendon could be achieved with two types of healing. CONCLUSION: It would be difficult to achieve full repair when complete tear and deep side tear occurred to t
