Complications and Reoperations Following Meniscal Repair and Transplantation.

PURPOSE OF REVIEW: Meniscal preservation through repair and transplantation is fundamental to modern orthopaedic surgery, with the goal of restoring knee biomechanics and delaying degenerative joint disease. Despite generally favorable outcomes, these procedures are associated with a range of complications and reoperations. This review aims to comprehensively evaluate the current literature on complications and reoperations following meniscal repair and meniscal transplantation, with emphasis on failure mechanisms, risk factors, and management strategies. RECENT FINDINGS: Recent literature rep

To be, or not to be, on insulin for simultaneous kidney-pancreas transplant: Is that the question?

The prevalence of type 2 diabetes mellitus (T2DM) among patients with kidney failure is nearly 40%. The therapeutic paradigm for the management of T2DM in the setting of chronic kidney disease (CKD) has undergone a dramatic evolution in recent years. Progressive CKD decreases the need for exogenous insulin, allowing noninsulin treatments to be more effective in controlling serum glucose levels. Although diet, exercise, weight loss, and behavioral therapy are important factors in diabetes management, several noninsulin alternatives have risen to the forefront, including glucagon-like peptide 1

Immunosuppressant and Prophylactic Medications in Patients with a Kidney Transplant.

Kidney transplantation is one of several treatment options available for patients with end stage kidney disease. Management of patients following kidney transplantation is a complex, multi-faceted endeavor that requires the involvement of all members of the multidisciplinary team. Medications used to prevent transplant rejection come with a myriad of side effects and monitoring considerations. In addition to immunosuppression, these patients are also more susceptible to infections and require antimicrobial prophylaxis in the early post-transplant period. Understanding the common medications an

One transplanted heart: A bridge across faiths.

This reflective narrative traces a heart transplant in Kolkata, India, between a 21-year-old Hindu donor and a 27-year-old Muslim recipient, a surgery that was as much a spiritual milestone as a clinical success. Drawing on Indian philosophical and spiritual traditions that revere the heart as more than a biological organ, this account reflects on how such stories of organ transplantation can transcend religious boundaries and inspire social unity. Ultimately, the piece calls for greater awareness around organ donation and emphasises the need to reframe religious discourse around shared human

Apixaban in Abdominal Transplantation: An Antithrombotic Stewardship Assessment.

BACKGROUND: Apixaban is a direct oral anticoagulant (DOAC) that is widely used in the general population; however, its use in solid organ transplant (SOT) recipients has not been extensively studied. Although there is interest in DOACs as the preferred oral anticoagulants in SOT recipients due to their favorable pharmacokinetic properties, there remain concerns for the optimal dose in the setting of organ dysfunction, at the time of engraftment, and for use for off-label indications such as graft thrombosis (GT). OBJECTIVE: To assess "real-world" apixaban utilization in abdominal SOT recipien

Transplant Indications in Polycystic Liver and Kidney Disease.

Polycystic liver and kidney disease encompasses a spectrum of inherited disorders ,primarily autosomal dominant polycystic kidney disease (ADPKD) and autosomal dominant polycystic liver disease (ADPLD). Although many patients remain asymptomatic for decades, a subset develops debilitating complications, including massive hepatomegaly, recurrent cyst infections, malnutrition, and progressive renal dysfunction. Diagnosis relies on imaging criteria and genetic testing. ADPKD is most commonly associated with PKD1 and PKD2 mutations, whereas ADPLD is associated with PRKCSH and SEC63 mutations. Magn

Malignancy and Kidney Transplant: Core Curriculum 2026.

Kidney transplant recipients are at least twice as likely to develop cancer compared with immunocompetent people. Cancer is now the second leading cause of death in kidney transplant recipients. Candidates and recipients are living longer with chronic conditions and immunosuppression, which increases the risk of cancers, especially skin and kidney cancers, lymphoma, and plasma cell dyscrasias. Given the complexities associated with the care of transplant patients with cancer, along with the advent of novel cancer therapies that include targeted and immunotherapies (ie, immune checkpoint inhibi

Study of ureteric jet in kidney transplant recipient.

INTRODUCTION: An intact vescio-ureteric junction (VUJ) complex is necessary to maintain its normal physiological function. A ureteric jet is produced when urine is vigorously propelled from the VUJ into the urinary bladder. The transplanted ureter lacks a normal VUJ but maintains its intrinsic peristaltic activity. this study was conducted to compare prospectively between the ureteric jet parameters in voluntary kidney donors by Doppler ultrasound of the same with patients who underwent ureteric re-implantation during a kidney transplantation procedure. METHODS: There were a total of 44 parti

Combined Liver-Small Bowel Transplant in Children.

Combined liver-small bowel transplantation is a lifesaving option for children with intestinal failure and end-stage liver disease. Careful patient selection, guided by residual bowel length, enteral tolerance, and liver fibrosis, is critical. Advances in surgical techniques, immunosuppression, and perioperative care have improved survival rates. Long-term outcomes rely on multidisciplinary management, growth and neurodevelopmental monitoring, and vigilant surveillance for infection and rejection. Emerging strategies, including bioengineered grafts and optimized perfusion, hold promise for fur

Molecular Rejection Signatures in Pancreas Transplant Biopsies.

Pancreas allograft rejection remains a key contributor to graft failure, yet its diagnosis relies on histological criteria. In this study, we applied the Molecular Microscope® Diagnostic System (MMDx) kidney-derived algorithm developed for organ transplant rejection to pancreas transplant biopsies to evaluate its potential diagnostic performance and clinical utility. We analyzed 124 pancreas allograft biopsies obtained between April 2019 and May 2025. Molecular and histologic classifications were compared and related to pancreas graft survival. MMDx classified 93/124 biopsies (75%) as no-rejec

Menopausal Experience in Solid-Organ Transplant Recipients.

IntroductionAdvancements in transplant care have led to an increasing number of women with solid organ transplantation (SOT) living through peri- and postmenopause. Menopause can significantly impact quality of life, and the added burden of SOT may exacerbate symptoms. Few studies have evaluated the menopausal experience in this population, and existing data rarely use validated questionnaires.Research QuestionThis study aimed to characterize menopausal symptoms and menopausal hormone therapy (MHT) use in peri- and postmenopausal SOT recipients using validated measures.DesignA cross-sectional

Persistent hyperparathyroidism in the 1st year postrenal transplantation.

BACKGROUND: Hyperparathyroidism (HPT) is a frequent complication in chronic kidney disease and may persist in 20%-50% of cases 1 year after kidney transplantation (KT). HPT contributes to long-term allograft dysfunction and increased risk of fractures. There is scant data on the prevalence of HPT in kidney transplant recipients (KTRs) in Saudi Arabia. The aim of this study is to evaluate the prevalence of this complication and to identify its risk factors in a single center in Saudi Arabia. SUBJECTS AND METHODS: In a retrospective study, data of 204 KTRs who underwent KT between January 2017

A Review of Hemophagocytic Lymphohistiocytosis in Transplant-Focusing on the Heart.

Hemophagocytic lymphohistiocytosis (HLH) is a complex and often difficult-to-diagnose disease that can be easily missed and is associated with high mortality. Transplantation serves as a predisposing factor, and the challenges in diagnosis and treatment are compounded in transplant patients. Although there are only a few documented cases of HLH in heart transplant (HT) recipients, the increasing prevalence of HT globally suggest that more cases are likely to emerge. In this review, we focus on HLH specifically in the context of HT. We present a unique and complex case report of HLH in a HT pat

Complications of paediatric kidney transplantation: a paediatrician's review.

Kidney transplantation is the preferred treatment for children with end-stage kidney disease, offering notable improvements in long-term survival and overall quality of life. Nonetheless, paediatric transplant recipients are susceptible to a spectrum of complications that compromise allograft function and overall health. This review highlights key post-transplant complications to aid general paediatricians in recognising and managing these challenges in concert with nephrologists. Early post-transplant complications include surgical issues and electrolyte abnormalities. Infectious complication

Time to rethink Blastocystis in faecal microbiota transplantation.

Blastocystis commonly colonises the human gut, but its presence has traditionally led to the exclusion of faecal microbiota transplantation donors. Emerging evidence links it to greater microbial diversity and favourable metabolic profiles, while no harm is seen in immunocompetent recipients. Routine screening may be unnecessary, except for subtype-specific testing in immunocompromised patients.

Live vaccines in solid organ transplant recipients.

PURPOSE OF REVIEW: Vaccine preventable infections (VPIs) contribute substantially to morbidity and mortality in solid organ transplant (SOT) recipients. Recent outbreaks and declining herd immunity have brought attention to optimizing vaccination pre and posttransplant, as many patients remain incompletely immunized at the time of transplant. This review summarizes modern evidence and evolving recommendations regarding live vaccination in SOT candidates and recipients. RECENT FINDINGS: Vaccination coverage at the time of transplant remains suboptimal, although targeted interventions have show
