Perceptions of quality of life among Ugandan patients living with HIV: a qualitative study.

BACKGROUND: Ugandans have endured the HIV epidemic for three decades. Now, with the availability of antiretroviral therapy (ART) and early diagnosis, those living with HIV can live longer and can enjoy the same life expectancy as the rest of the Ugandan population. This emerging trend necessitates the assessment of quality of life, alongside other patient outcomes, of those undergoing therapy, alongside other patient outcomes. While major strides have been made in developing measures of quality of life in the developed world, there remains a paucity of evidence from resource-limited settings. …

Impaired T-cell proliferation among HAART-treated adults with suboptimal CD4 recovery in an African cohort.

BACKGROUND: Most HIV-infected subjects exhibit a progressive rise in CD4 T-cell counts after initiation of highly active antiretroviral therapy (HAART). However, a subset of individuals exhibit very poor CD4 T-cell recovery despite effective control of HIV-RNA viraemia. We evaluated CD4 T-cell proliferation among suboptimal responders and its correlation with CD4 T-cell activation. METHODS: The magnitude of CD4 increase (difference between absolute CD4 counts at baseline and absolute CD4 counts at 4 years of ART) was grouped into 4 quartiles for the 211 patients with sustained HIV-RNA viral s

Determinants of the accuracy of rapid diagnostic tests in malaria case management: evidence from low and moderate transmission settings in the East African highlands.

BACKGROUND: The accuracy of malaria diagnosis has received renewed interest in recent years due to changes in treatment policies in favour o … using HRP2-based RDTs over a period of approximately four years in four highland sites in Kenya and Uganda representing hypoendemic to mesoendemic settings. In addition, the utility of the tests was evaluated in comparison with expert microscopy for disease management in 2,241 subjects in two sites with different endemicity levels over four months. Genome scan of M. tuberculosis infection and disease in Ugandans.

Tuberculosis (TB), caused by Mycobacterium tuberculosis (Mtb), is an enduring public health problem globally, particularly in sub-Saharan Africa. Several studies have suggested a role for host genetic susceptibility in increased risk for TB but results across studies have been equivocal. As part of a household contact study of Mtb infection and disease in Kampala, Uganda, we have taken a unique approach to the study of genetic susceptibility to TB, by studying three phenotypes. First, we analyzed culture confirmed TB disease compared to latent Mtb infection (LTBI) or lack of Mtb infection. Sec…

The long-term effects of protein energy malnutrition in early childhood on bone age, bone cortical thickness and height.

Three groups of Ugandan children, 18 in each group, and one comparison group of 18 children were examined at 11-17 years of age. The three groups had previously been admitted for treatment of protein energy malnutrition between the ages of 8 to 15, 16 to 21 and 22 to 27 months respectively. The comparison group had not been clinically malnourished throughout the period up to 27 months of age. The children came from one tribe and from similar socio-economic background, and were individually matched on age and sex. The bone age was estimated by hand wrist radiography scored for maturity by the T…

HIV Transmission in a Declining African Epidemic.

BACKGROUND: Novel HIV prevention interventions such as long-acting pre-exposure prophylaxis (PrEP) could substantially reduce HIV transmission in Africa. However, efficient implementation in high-prevalence settings where incidence has declined requires an understanding of the contemporary dynamics driving new infections. METHODS: We identified incident HIV cases from a longitudinal, population-based cohort in Uganda. We individually matched cases to HIV-negative controls; traced and enrolled reported sexual partners; and enrolled female sex workers (FSWs) from reported venues. Conditional lo…

Recurrence Rate and Risk Factors for Apical Prolapse After Sacrospinous Ligament Fixation: A Prospective Cohort Study.

OBJECTIVE: We aimed to determine the one-year recurrence rate of apical prolapse following sacrospinous ligament fixation and to identify clinical and surgical risk factors associated with recurrence in a low-resource setting. METHODS: This was a prospective cohort study of women who underwent sacrospinous ligament fixation as part of surgery for apical prolapse at the urogynecology unit of Mbarara Regional Referral Hospital (MRRH) in Uganda. The surgeries performed included vaginal hysterectomy with sacrospinous ligament vault fixation, sacrospinous ligament hysteropexy, and sacrospinous lig…

Digital health implementation research across selected African countries: a bibliometric analysis of maternal health and infectious diseases with observations on precision medicine representation (2015-2025).

BACKGROUND: Digital health technologies are increasingly recognised as important tools for strengthening healthcare delivery across African health systems. Despite rapid growth in digital health innovation, the implementation-oriented research landscape remains fragmented, with limited synthesis of the field's thematic structure, collaboration patterns, and evolving research priorities. This study maps the intellectual structure and thematic evolution of implementation-focused digital health research across selected African countries (South Africa, Kenya, Uganda, Nigeria, Ethiopia, Ghana, Tanz…

Diagnostic accuracy of tongue swab testing in persons with sputum Xpert Ultra Trace results.

BACKGROUND: Molecular amplification of tongue swab samples is a non-sputum-based investigational approach to diagnose pulmonary tuberculosis (pTB). Performance characteristics in persons with low-positive results on sputum molecular tests are unknown.
