BACKGROUND: We report a case of ocular cat-scratch disease with permanent vision reduction in a patient who was cytoplasmic anti-neutrophil cytoplasmic antibody (C-ANCA) positive.
METHODS: Case report and review of the literature.
RESULTS: While taking steroids and antibiotics, a 52-year-old man with uveitis associated with cat-scratch disease developed retinal vein occlusion and a macular exudate. His final visual acuity was poor because of residual macular degeneration and optic atrophy. Serum C-ANCA increased and decreased in parallel with ocular inflammatory activity.
CONCLUSION: C-ANCA…

Behçet's disease is one of the most difficult forms of uveitis to treat. Variety in disease presentation and severity, as well as regional differences in standard of care, demand a tailor-made approach. Anterior segment inflammation generally responds to topical corticosteroids. However, the onset of posterior segment inflammation usually requires the advancement of treatment to periocular injections and/or oral administration of corticosteroids. Cyclosporin, either alone or in combination with corticosteroids, is considered in refractory patients. Other immunosuppressive drugs, such as azathi…

Antiflammins are synthetic peptides derived from the region of highest local similarity between uteroglobulin and lipocortin. These peptides have shown anti-inflammatory activity on carrageenan-induced rat footpad edema. They are potent inhibitors for phospholipase A2 activation both in vitro and in vivo. Previously, we have demonstrated the effectiveness of topical antiflammins in suppressing acute ocular inflammation and allergic response in rodent endotoxin-induced uveitis and murine allergic conjunctivitis. The mechanisms by which antiflammins protect against inflammation and allergy in th…

There is an increased awareness about the pediatric aspects of Behçet's disease (BD) in the world literature. Recent surveys of patients have outlined peculiar features in this age group such as recurrent attacks of fever and abdominal pain. Considered to be uncommon in childhood, uveitis have a very severe course. The higher frequency of familial cases and the genetic anticipation suggest that the genetic component plays a major role in the early expression of the disease. Pediatric aspects of BD including different categories of patients, are not definitely characterized. Indeed it is now ne…

Ocular involvement, mainly uveitis or retinochoroiditis, is common in various systemic diseases, such as endogenous endophthalmitis, Lyme disease, human T-cell lymphotropic virus type I infection, toxoplasmosis, and toxocariasis. Recent progress, especially in laboratory microbiologic testing, has enabled us to reliably diagnose many formerly idiopathic intraocular inflammatory diseases. A group of systemic infectious diseases, including those discussed here, are implicated as a body of emerging or re-emerging diseases that have appeared in the past two decades and are thought to have a close …

A 14-month-old girl developed chronic stridor and dyspnoea. Four months later she presented arthritis, anterior uveitis and positive ANA. Juvenile chronic arthritis (JCA) was diagnosed. Laryngoscopy demonstrated the presence of cricoarytenoid arthritis (CA). The left vocal cord was adducted and immobile, while the right vocal cord had decreased mobility. Erythema and swelling of the arytenoid cartilage on both sides was seen. Steroid treatment resulted in the resolution of these symptoms and made airway control unnecessary. This case demonstrates that CA may be the first sign of JCA, preceding…

The International Study Group's diagnostic criteria for Behçet's disease are gaining acceptance and may be further refined before the decade … rsuit of a better disease marker. The choice of immunosuppressive therapy in severe disease such as uveitis and arteritis is between alkylating agents and agents that inhibit interleukin-2 production.

To analyze the association between Fuchs' heterochromic iridocyclitis (FHI) and toxoplasmosis, we performed ocular examinations and used various specific laboratory tests to establish a role for Toxoplasma gondii in the pathogenesis of FHI. Results were compared with those for other types of uveitis and healthy controls. Of the 88 patients with FHI, nine (10.2%) had toxoplasmosislike scars, but an association could not be proved by the indirect immunofluorescence antibody test or enzyme-linked immunosorbent assay, or by a test for cellular immunity to Toxoplasma antigen. Analysis of aqueous hu…

In six patients with ocular Lyme borreliosis, bilateral granulomatous iridocyclitis and vitritis were present in five. One of these five also had bilateral optic neuritis. Another patient developed combined trochlear and facial nerve palsies. A syndrome resembling pars planitis with atypical features such as granulomatous keratic precipitates and posterior synechiae should prompt a search for Lyme borreliosis. Topical corticosteroid therapy is necessary to prevent complications of anterior segment inflammation caused by Lyme uveitis, but the benefit of systemic and periocular corticosteroids i…

The clinical response to thalidomide in 30 male patients with Behçet's disease was reviewed. Twenty-six patients with orogenital ulcerations responded (20 completely and 6 partially). Cutaneous lesions including erythema nodosum or necrotic pseudofolliculitis were controlled by treatment in the majority of cases. All patients with arthritis responded (13 completely and 1 partially). Uveitis was improved in 3 patients, stabilizated in 4 and aggravated in 4. Patients with thalidomide purposely discontinued experienced a flare; the reintroduction of therapy controlled the flare. Side effects were…

Ten consecutive patients with juvenile ankylosing spondylitis were investigated in a prospective study. At least one of the parents of each patient had a positive history of probable or definite ankylosing spondylitis, Reiter's syndrome, or acute anterior uveitis. The patients and their parents were examined clinically and were HLA-typed. Nine of the 10 patients exhibited the genetic marker HLA B27, but only one patient was homozygous for HLA B27. This prevalence of homozygosity among HLA B27 positive persons could be expected to occur by chance alone. Based on the limited material studied, we…

Epstein-Barr virus (EBV), a common human herpesvirus, usually causes a self-limited illness in children, manifested by fever, pharyngitis, and lymphadenopathy. However, patients may have atypical presentations including rare ocular manifestations like uveitis, dacryoadenitis, ptosis, or pseudoptosis. In this report, we describe EBV presenting as pseudoptosis in a previously healthy 7-year-old boy. The child was initially worked up as having possible myasthenia gravis with neurologic and ophthalmologic consultations. Ultimately, he was diagnosed with viral dacryoadenitis due to acute EBV infect…
