Use of antimicrobial agents in United States neonatal and pediatric intensive care patients.

OBJECTIVE: Antimicrobial use contributes to the development of emergence and dissemination of antimicrobial-resistant bacteria among intensive care unit (ICU) patients. There are few published data on antimicrobial use in neonatal (NICU) and pediatric ICU (PICU) patients. METHODS: Personnel at 31 Pediatric Prevention Network hospitals participated in point prevalence surveys on August 4, 1999 (summer) and February 8, 2000 (winter). Data collected for all NICU and PICU inpatients included demographics, antimicrobials and indications for use and therapeutic interventions. RESULTS: Data were re

Rupichrysenoids A-H, phytocannabinoid-like meroterpenoids from Rhododendron rupicola var. chryseum: Isolation, structure, and bioactivity.

The first phytochemical investigation on the aerial parts of Rhododendron rupicola var. Chryseum led to the discovery of eight previously undescribed phytocannabinoid-like meroterpenoids (PLMs), named rupichrysenoids A-H (1-8), along with two previously reported ones (9 and 10). The structural establishment of PLMs 1-8 was achieved by means of spectroscopic data (NMR and HRMS) analysis, ECD calculations, and single crystal X-ray diffraction, as well as NMR calculations and DP4+ probability analyses. PLMs 2 and 10 showed significant α-glucosidase inhibitory effects with IC50 values of 16.73 ± 1

Argyrin B exhibits potent therapeutic efficacy in a Clostridioides difficile-infection mouse model while preserving microbiota functionality.

Clostridioides difficile infection (CDI) remains a leading cause of antibiotic-associated diarrhoea, with high recurrence rates and limited treatment options that preserve gut microbiota. Current treatments, including vancomycin, further disrupt the already compromised gut microbiota, often prolonging dysbiosis and increasing the risk of colonization by resistant pathogens. This study demonstrates that argyrin B exhibits potent activity against C. difficile in vitro and significantly reduces bacterial burden in a mouse model of infection. Argyrin B displayed a narrow antimicrobial spectrum, su

Novel β-Carboline Derivative ZLWH-67 Exerts Potent Anti-MRSA Activity through Multiple Mechanisms.

Infections induced by methicillin-resistant Staphylococcus aureus (MRSA) are serious, highlighting the urgent need for exploring new antibacterial agents. Here, we report a new β-carboline derivative, termed ZLWH-67, that displays potent antibacterial activity against MRSA. ZLWH-67 exhibited bactericidal properties, low cytotoxicity and hemolytic toxicity, and good safety in vivo and was not susceptible to resistance. The potential antibacterial mechanisms of ZLWH-67 were studied by RNA-seq analysis and verified by RT-PCR. The results indicated that ZLWH-67 might exert its effects through mult

Multiple Cold Abscesses of a Chest Wall with Ribs Destruction in a Hemodialysis Patient.

Cold abscess of the chest wall is an uncommon disease that is mainly caused by tuberculous infection. Staphylococcal cold abscesses of the chest wall are extremely rare and usually clinically occult, frequently leading to a delay in diagnosis and consequently severe infectious complications. We report an 88-year-old woman with end-stage renal disease, who presented with an exit site infection of a cuffed tunneled dialysis catheter caused by methicillin-resistant Staphylococcus aureus (MRSA). Despite the removal of the catheter and administration of antibiotics, she developed refractory MRSA ba

Acute Focal Bacterial Nephritis in an Infant Referred with Apnea Caused by Mixed Infection with Enterococcus raffinosus and Escherichia coli.

A 38-day-old infant was referred to our hospital for evaluation of apnea, fever, and pyuria. Invasive bacterial infection, including meningitis, was suspected because of the presence of apnea. A contrast-enhanced CT scan revealed acute localized bacterial nephritis, and meningitis was ruled out. Gram-positive cocci and Gram-negative rods, ie, Enterococcus raffinosus and Escherichia coli, were isolated from a urine culture at the referring hospital. This case report describes the youngest case of E. raffinosus infection. Apnea was the main complaint, but the origin of fever was infant acute foc

The bacteriocin Angicin interferes with bacterial membrane integrity through interaction with the mannose phosphotransferase system.

In a natural environment, bacteria are members of multispecies communities. To compete with rival species, bacteria produce antimicrobial peptides (AMPs), called bacteriocins. Bacteriocins are small, cationic, ribosomally synthesized peptides, which normally inhibit closely related species of the producing organism. Bacteriocin production is best studied in lactic bacteria (LAB). Streptococcus anginosus, belonging to LAB, produces the potent bacteriocin Angicin, which shows inhibitory activity against other streptococci, Listeria monocytogenes and vancomycin resistant Enterococcus faecium (VRE

Thrombin-Derived Peptides Potentiate the Activity of Gram-Positive-Specific Antibiotics against Gram-Negative Bacteria.

The continued rise of antibiotic resistance threatens to undermine the utility of the world's current antibiotic arsenal. This problem is particularly troubling when it comes to Gram-negative pathogens for which there are inherently fewer antibiotics available. To address this challenge, recent attention has been focused on finding compounds capable of disrupting the Gram-negative outer membrane as a means of potentiating otherwise Gram-positive-specific antibiotics. In this regard, agents capable of binding to the lipopolysaccharide (LPS) present in the Gram-negative outer membrane are of par

Enterococcus spp. as a Producer and Target of Bacteriocins: A Double-Edged Sword in the Antimicrobial Resistance Crisis Context.

Enterococcus spp. are one of the most frequent producers of bacteriocins (enterocins), which provides them with an advantage to compete in their natural environment, which is the gut of humans and many animals. The enterocins' activity against microorganisms from different phylogenetic groups has raised interest in Enterococcus spp. in different contexts throughout the last decades, especially in the food industry. Nevertheless, some species can also cause opportunistic life-threatening infections and are frequently multidrug-resistant (MDR). Vancomycin-resistant Enterococcus (VRE), in particu

Understanding the Mechanism of Action of NAI-112, a Lanthipeptide with Potent Antinociceptive Activity.

NAI-112, a glycosylated, labionine-containing lanthipeptide with weak antibacterial activity, has demonstrated analgesic activity in relevant mouse models of nociceptive and neuropathic pain. However, the mechanism(s) through which NAI-112 exerts its analgesic and antibacterial activities is not known. In this study, we analyzed changes in the spinal cord lipidome resulting from treatment with NAI-112 of naive and in-pain mice. Notably, NAI-112 led to an increase in phosphatidic acid levels in both no-pain and pain models and to a decrease in lysophosphatidic acid levels in the pain model only

Characterization of the Interaction of Daptomycin With Site II on Human Serum Albumin.

Daptomycin, a cyclic lipopeptide antibiotic, is clinically used for the treatment of infections caused by Gram-positive bacteria, including the methicillin-resistant Staphylococcus aureus and the vancomycin-resistant Enterococci. While daptomycin shows high plasma protein binding (90-93%), our knowledge of the binding process is not extensive. To address this issue in more detail, we characterized the binding of daptomycin to plasma proteins and the findings indicate that the association constant for the binding of daptomycin to human serum albumin (HSA) is much higher than that for α1-acid gl

Impact of enterococcal urinary tract infections in immunocompromised - neoplastic patients.

Infections in immunocompromised-neoplastic patients represent a severe complication. Among bacteria, Enterococcus species constitute a common causative pathogen of urinary tract infections (UTIs), especially among hospitalized patients with or without urinary tract carcinoma, related commonly to urinary tract abnormalities, urinary catheters or prolonged antibiotic treatment. Although enterococci have been considered more commonly as colonization bacteria in the intestine than virulent agents, they are frequently implicated in UTIs. The high incidence of enterococcal UTIs is associated with se

[Persistent and recurrent skin infection with small-colony variant methicillin-resistant Staphylococcus aureus].

S. aureus is one of the problematic bacteria, capable to develop resistance mechanisms to all antibiotics that the bacteria are naturally susceptible. A particular phenotypic mechanism, especially against the antibiotics that repressed the synthesis of the cellular wall and aminoglycosides, was evidenced in subpopulations that grows in small-colonies and represents auxotrophic mutants for hemin, menadione or thymidine. This type of strains has been isolated most frequently from patients with osteomyelitis, septic arthritis or pulmonary infections after a long period of antibiotic treatment. Th
