Stakeholders' perspectives on a digital myopia screening program in children: a qualitative analysis.

PURPOSE: This study was aimed at identifying barriers and opportunities to use a self-administered online refractive eye test by various stakeholders of a pediatric vision screening program. METHODS: This qualitative study performed semi-structured interviews with myopic children and their parents, eye care professionals, and policymakers. Three topic lists were developed, delineating themes to identify gaps, barriers, and opportunities. Interviews were anonymously recorded, transcribed, and coded using thematic analysis. Quantitative data was acquired from a concomitant clinical validation s

Neural Network Prediction of Keratoconus in AIPL1-Linked Leber Congenital Amaurosis: A Proof-of-Concept Pilot Study.

Background/Objectives: Keratoconus (KC) can rapidly erode vision in children with Leber congenital amaurosis (LCA), yet screening usually depends on costly corneal imaging that is often unavailable. We evaluated whether a lightweight, image-free neural network fed only routine clinical and genetic variables can detect KC in patients with AIPL1-related LCA. Methods: This retrospective, proof-of-concept pilot study analyzed chart data for 19 children with biallelic AIPL1 mutations (6 with KC) seen at five tertiary eye centers between January and December 2004. Ten baseline predictors were entere

Accurate machine learning model for human embryo morphokinetic stage detection.

PURPOSE: The ability to detect, monitor, and precisely time the morphokinetic stages of human pre-implantation embryo development plays a critical role in assessing their viability and potential for successful implantation. Therefore, there is a need for accurate and accessible tools to analyse embryos. This work describes a highly accurate, machine learning model designed to predict 17 morphokinetic stages of pre-implantation human development, an improvement on existing models. This model provides a robust tool for researchers and clinicians, enabling the automation of morphokinetic stage pr

Falls in Older Adults: Approach and Prevention.

Falls are a major public health problem, occurring in more than 27% of adults 65 years and older and costing the U.S. health care system tens of billions of dollars each year. The most common risk factors are prior falls, balance disorders, fear of falling, and dementia. Regular physical activity reduces fall risk. Identifying injuries is the first step in evaluating older adults who have fallen. The patient's history may be inaccurate if they have cognitive impairment, and the physical examination can result in false-negative findings. If injury status cannot be determined and suspicion for i

Protocol for a prospective interventional trial to develop a diagnostic index test for stroke as a cause of vertigo, dizziness and imbalance in the emergency room (EMVERT study).

INTRODUCTION: Identifying stroke as a cause of acute vertigo, dizziness and imbalance in the emergency room is still a clinical challenge. Many patients are admitted to stroke units, but only a minority will have strokes. This imposes a heavy financial burden on the healthcare system. The aim of this study is to develop a diagnostic index test to identify patients with a high risk of having a stroke as the cause of acute vertigo and imbalance. METHODS AND ANALYSIS: Patients with acute onset of vertigo, dizziness, postural imbalance or double vision within the last 24 hours lasting for at leas

False amblyopia prediction in strabismic patients by fixation preference testing correlates with contralateral ocular dominance.

BACKGROUND: Binocular fixation preference testing is commonly used to predict strabismic amblyopia in preverbal children. However, false positives can occur and can lead to unnecessary patching treatment. We explored potential causes for this phenomenon. METHODS: In this prospective cohort study (2013-2014) of consecutive verbal strabismic patients without incomitance or decreased vision other than strabismic amblyopia, binocular fixation preference testing was graded from 1 (strong preference) to 4 (free alteration), with grade 1 or 2 considered predictive of amblyopia and the examiner maske

Component perimetry: a fast method to detect visual field defects caused by brain lesions.

PURPOSE: Noise field campimetry, performed according to Aulhorn and Köst, confronts patients with a large field of irregularly flickering dots, and many patients immediately perceive their visual field defects. The original method had a somewhat low specificity and sensitivity, especially for patients with visual field defects caused by cortical lesions. METHODS: The method was improved in two ways. First, the grain of the visual noise was increased toward the periphery of the visual field to accommodate the peripheral decrease in visual acuity. Second, the type of stimulus pattern was varied

Humphrey vs. Compass: A Comparative Study of Two Perimetry Devices.

BACKGROUND: Visual field testing is essential for glaucoma diagnostics; however, patient cooperation can affect the reliability of the results. The COMPASS perimeter (COM; CentreVue-iCare, Padua, Italy) offers an innovative approach to enhance test quality by compensating for fixation losses and automatically adjusting light stimuli. Additionally, it enables simultaneous fundus photography during perimetry. In this prospective study, we compare the COM perimeter with the Humphrey Field Analyzer (HFA) in glaucoma patients and healthy subjects in terms of visual field parameters, reproducibility
