Arthrodesis of the metatarsophalangeal joint of the great toe.

Since 1975 the usual treatment of hallux valgus and hallux rigidus at the Medical High School, Lübeck, has been arthrodesis of the metatarophalangeal joint of the great toe. We report 48 operations performed between 1975 and 1977 on 35 patients with a follow-up of 3-32 months. The operative technique, using the "dynamic compression plate" is described in detail. The patient's assessment and the clinical and radiological situations were recorded. There were only three poor results. Thus, arthrodesis of the metatarsophalangeal joint is considered to be a reliable therapeutic method in hallux val

The use of bone staples to correct valgus and varus knee deformities in Caribbean children: A 30-year review.

Paediatric coronal-plane angular knee deformities are very common and should be corrected before skeletal maturity to prevent significant morbidity in adulthood. Hemi-epiphysiodesis using bone staples was traditionally used for correction, but this is now most often performed using tension band plating. However, these implants are costly and therefore we believe that there is still a role for bone staples in low- and middle-income countries (LMICs). Results of a 30-year retrospective, single-centre study of bone stapling performed for angular knee deformity correction in Trinidad showed that o

Residual Medial Ankle Pain After the Delayed Union of a Lateral Malleolus Fracture: A Case Report.

A 17-year-old girl sprained her left ankle and was diagnosed with a lateral malleolar fracture. She was treated conservatively for six months but had medial ankle pain with activity. Imaging revealed an oblique lateral malleolar fracture, with posterolateral displacement and partial fusion of the bone fragments, and bone marrow edema on the medial articular surface of the talus and medial malleolus. We diagnosed ankle instability due to delayed union with a displacement of the lateral malleolus, which caused an osteochondral lesion. We performed arthroscopic and open surgery eight months after

A rare presentation of osteochondroma at the spinous process of the cervical spine: a case report.

Osteochondroma is a benign cartilaginous tumor that usually arises from the growth plate of the long bones. Its presentation in flat bones is uncommon. Spinal osteochondroma is a rare case, with only a 4% incidence compared to all spinal neoplasms. We report an unusual case of osteochondroma at the spinous process of the C4 spine of a 15-year-old patient with the chief complaint of neck discomfort on movement. The patient has previously been diagnosed and treated for multiple hereditary exostoses of other bones. The patient underwent surgery for the current lesion with complete excision to the

Unilateral facet fixation as a salvage procedure: Report of two cases with cervical primary bone tumors with extensive bone destruction.

We describe cases of two patients with primary cervical bone tumors that resulted in extensive destruction of bones of the region. In the first patient, the tumor and its growth resulted in the destruction of C3-C5 vertebral bodies and the unilateral destruction of facets and pedicles. In the second case, there was focal destruction of the body and odontoid process and unilateral pedicle and facet of C2. Tumor resection and spinal stabilization were the aims of the surgery. In both cases, unilateral fixation of facets was done. In the first patient, multi-segmental transarticular fixation by C

Nutritional Approaches as a Treatment for Impaired Bone Growth and Quality Following the Consumption of Ultra-Processed Food.

The severe impairment of bone development and quality was recently described as a new target for unbalanced ultra-processed food (UPF). Here, we describe nutritional approaches to repair this skeletal impairment in rats: supplementation with micro-nutrients and a rescue approach and switching the UPF to balanced nutrition during the growth period. The positive effect of supplementation with multi-vitamins and minerals on bone growth and quality was followed by the formation of mineral deposits on the rats' kidneys and modifications in the expression of genes involved in inflammation and vitami

Treatment of Femoral Shaft Pseudarthrosis, Case Series and Medico-Legal Implications.

Pseudarthrosis (PSA) is a possible complication of femoral shaft fracture treatment. It is often associated with reduced bone quality and can, therefore, adversely affect quality of life. Its treatment poses a major challenge for orthopaedic surgeons. Several authors have set forth different surgical approaches for the treatment of pseudarthrosis, such as internal fixation with plate and screws, replacement of an intramedullary nail or prosthetic replacement. In cases associated with bone loss, osteopenia, or comminution of fracture fragments, autologous or homologous bone grafts may also be u

Efficacy of a tip of the big toe remodeling in the distal nail embedding with bone overgrowth of the distal phalanx.

INTRODUCTION: Distal nail embedding due to hyponychium hypertrophy can be caused by traumatic or surgical avulsion of the nail. As a consequence of these changes, the nail plate is blocked through the deformed tip of the toe. Changes that occur at the tip of the big toe are due to bone growth on the dorsal surface of the distal end of the distal phalanx. This study aimed to present a surgical technique for the treatment of hypertrophy of the tip of the toe and evaluate its effectiveness. MATERIAL AND METHODS: The surgical technique involved remodeling of the tip of the big toe, with removal o

A review of terminology for equine juvenile osteochondral conditions (JOCC) based on anatomical and functional considerations.

This manuscript describes a new classification of the various joint-related lesions that can be seen in the young, growing horse based on their anatomical and functional aetiopathogenesis. Juvenile osteochondral conditions (JOCC) is a term that brings together specific disorders according to their location in the joint and their biomechanical origin. When a biomechanical insult affects the process of endochondral ossification different types of osteochondrosis (OC) lesions may occur, including osteochondral fragmentation of the articular surface or of the periarticular margins, or the formatio

Lessons learnt from the management of 1500 complex facial fractures.

The lessons learnt from the management of 1500 consecutive patients with complex facial fractures have been analysed. This analysis and the major changes in treatment principles have resulted in dramatic improvement in results. The application of craniofacial surgical principles, extended exposure of the craniofacial skeleton, accurate fracture reduction with rigid internal bony fixation and primary bone graft reconstruction has revolutionised the care of these patients. The reattachment of the soft tissue to the reconstructed skeleton provides the final link in this comprehensive one stage re

Does rigid instrumentation increase the fusion rate in one-level anterior cervical discectomy and fusion?

BACKGROUND CONTEXT: Although plate fixation enhances the fusion rate in multilevel anterior cervical discectomy and fusion (ACDF), debate exists regarding the efficacy of nonplating to rigid plate fixation in one-level ACDF. PURPOSE: To determine the efficacy of nonplating to rigid plate fixation in regards to fusion rate and clinical outcome in patients undergoing one-level ACDF with autograft. STUDY DESIGN: A review of 69 consecutive patients who underwent one-level ACDF with autograft and with or without rigid anterior cervical plate fixation. PATIENT SAMPLE: Sixty-nine patients who unde

Effects of maternal consumption of morphine on rat skeletal system development.

BACKGROUND: Opioid abuse is among the most ubiquitous issues world-wide, and when it happens in mothers, it puts them at risk of diseases that can be transferred to the next generation. Previous studies have indicated that morphine addiction during pregnancy could inhibit development in rat embryos and infants. The present study focused on the effects of maternal consumption of morphine on rat skeletal system development and also investigate the molecular pathway of chondrogenesis and osteogenesis of infants from control and addicted rat groups. METHODS: Thirty-two female rats were randomly a

Effect of screw placement on fixation in the humeral head.

The objectives of this study were (1) to determine the most advantageous screw locations within the humeral head when plate and screw fixation is to be used and (2) to determine the effect of positioning the screw tip abutting the subchondral bone. Ten paired humeral heads were harvested with a monoplanar cut through the anatomic neck. Through use of a standardized template, 7 holes were drilled and tapped in each specimen for insertion of 6.5-mm fully threaded cancellous screws perpendicular to the plane of the cut. Paired specimens were randomized into 2 groups, one with the screw purchase i

Titanium vs. carbon fiber-reinforced intramedullary nailing for humeral bone tumors.

BACKGROUND: Multiple techniques have been described to treat humeral diaphyseal bone tumors requiring curettage or excision. Recent studies have suggested that carbon fiber-reinforced polyetheretherketone (CFR-PEEK) intramedullary nails (IMNs) may be preferable to titanium IMNs for patients with musculoskeletal tumors due to CFR-PEEK's high tensile strength, radiolucency, a modulus of elasticity closer to native bone, and improved postoperative surveillance/radiation dosing. In this study, we describe the rate of fixation failure for both CFR-PEEK and titanium humeral IMNs when used for humera

Targeted expression of SHH affects chondrocyte differentiation, growth plate organization, and Sox9 expression.

UNLABELLED: The role of Hedgehogs (Hh) in murine skeletal development was studied by overexpressing human Sonic Hedgehog (SHH) in chondrocytes of transgenic mice using the collagen II promoter/enhancer. Overexpression caused a lethal craniorachischisis with major alterations in long bones because of defects in chondrocyte differentiation. INTRODUCTION: Hedgehogs (Hhs) are a family of secreted polypeptides that play important roles in vertebrate development, controlling many critical steps of cell differentiation and patterning. Skeletal development is affected in many different ways by Hhs. G

Dose-dependent effects of estrogen-enriched bone cement on membrane quality in the Masquelet technique: experimental rat model.

The Masquelet technique is a 2-stage surgical method used for the reconstruction of critically sized bone defects due to various etiologies. Estrogen is known to have anabolic effects on bone metabolism. In this study, based on literature data, the aim is to systematically evaluate the histological and immunohistochemical effects of adding different doses of estrogen to polymethylmethacrylate cement on induced membrane tissue.  Methods: Twenty-eight male rats were included in the study and divided into 4 groups. Three experimental groups received different doses of estradiol, a subtype of estr

A Rare Case of Co-existent Superior Semicircular Dehiscence and Sigmoid Sinus Plate Dehiscence Contributing to Pulsatile Tinnitus.

Pulsatile tinnitus (PT) is an uncommon but often debilitating symptom, frequently associated with vascular etiologies, tumors, or structural abnormalities of the temporal bone. We present the case of a 40-year-old woman diagnosed with concurrent unilateral superior semicircular canal dehiscence (SSCD) and sigmoid sinus plate dehiscence (SSPD), both contributing to her symptoms. This case highlights the critical role of thorough clinical and radiological assessment in detecting multiple coexisting etiologies in patients with PT.

Fibroblast growth factor receptor 3 and the human chondrodysplasias.

Heterozygous mutations of the gene encoding the fibroblast growth factor receptor 3 (FGFR3) have been found in persons with achondroplasia, thanatophoric dysplasia, and hypochondroplasia. They exhibit considerable genetic homogeneity, and specific mutations strongly correlate with the clinical severity of disease. The mutations activate the FGFR3 by promoting dimerization, by stimulating intrinsic tyrosine kinase activity, and perhaps by altering ligand and dimerization specificity. The downstream signals regulate events in the growth plate, ultimately inhibiting linear bone growth.

Giant Cell Tumor of the Olecranon With a Displaced Pathological Fracture Successfully Treated by Intralesional Curettage and Plate Fixation: A Case Report.

Giant cell tumor of bone (GCTB) is a locally aggressive intermediate bone tumor that typically arises at the epiphysis of long bones. GCTB occurring in the olecranon is extremely rare, and the optimal surgical strategy when complicated by pathological fractures with marked displacement involving the articular surface remains unclear. In this case, the fracture site was significantly displaced. Through the gap created by the displacement, we performed thorough curettage of the lesion, anatomical reduction, synthetic bone grafting, and plate fixation. Histopathological examination confirmed GCTB
