Early treatment of scoliosis with growing rods in children with severe spinal muscular atrophy: a preliminary report.

BACKGROUND: Spinal muscle atrophy (SMA) is a progressive neuromuscular disease predominantly presenting in infancy and early childhood. Scoliosis is the most common spinal deformity in these patients and treatment in SMA patients is controversial. Treatment is usually definitive fusion. The purpose of this study is to evaluate a novel growing rod technique used to treat more involved children with SMA types I and II with scoliosis at an earlier age. METHODS: An Institutional Review Board approved retrospective medical record review was performed of children with SMA who were treated for scoli

Association between plate location and plate removal following facial fracture repair.

BACKGROUND: Titanium-based plates used to repair facial fractures are sometimes removed despite their high biocompatibility. Local discomfort can lead to plate removal surgery. Local discomfort may differ according to patient characteristics, tissue properties and plate thickness; however, little is known about the relationship between these conditions and plate removal. METHODS: We performed a hospital-based, retrospective cohort study of patients who underwent internal fixation for facial or frontal bone fracture. To identify factors associated with plate removal, we used multivariate logis

Surgical management of severe deformity resulting from posterior tibial tendon dysfunction.

A retrospective case study format was used to evaluate the results of surgical treatment of severe deformities due to long-standing tibialis posterior tendon dysfunction. To be included in the study, the peri-talar (talonavicular) subluxation on a weight-bearing AP X-ray should have been at least 75%. This criterion limited the field to patients with long-standing, rigid, severe deformities. Surgical correction was obtained through a two-incision approach, dorsomedial and dorsolateral. Significant shortening of the medial column and extensive removal of adaptive bone formation was necessary to

Treatment of talar neck fractures: clinical results of 50 patients.

Fractures of the neck and body of the talus present as one of the most challenging and rare injuries. These fractures are often associated with other ankle, foot, and skeletal injuries, which complicate their treatment. The clinical course of 50 patients with a mean age of 29 years with a severe talus fracture between 1992 and 1997 is presented. According to the Hawkins classification there were 16 (32%) of type I, 14 (28%) of type II, 9 (18%) of type III, and 11 (22%) of type IV. Forty-three patients (86%) underwent operative treatment: 27 (63%) by open reduction and internal fixation with sc

Extraction and implant restoration of an inverted impacted anterior tooth with severe crown-root dilaceration: a 4-year follow-up case report.

BACKGROUND: Inverted impaction of anterior teeth with crown-root dilaceration is a rare and clinically challenging developmental anomaly. When orthodontic traction or autotransplantation fails to achieve satisfactory outcomes, extraction followed by implant restoration becomes the preferred treatment option. This approach, however, involves multiple challenges, including the extraction of the inverted impacted tooth, bone defect repair, and aesthetic reconstruction of the anterior dentition. CASE PRESENTATION: This single-case report details the treatment process of a 17-year-old patient with

CLINICAL MANAGEMENT OF IMMEDIATE IMPLANT PLACEMENT AND LOADING IN THE ESTHETIC ZONE WITH FINAL PROSTHETIC RESTORATION.

Immediate implant placement with immediate loading in the esthetic zone is a predictable treatment option when strict surgical and prosthetic protocols are followed. This case report describes the clinical management of a 50-year-old systemically healthy, non-smoking male treated with two immediately placed implants in the maxillary central incisor region (teeth 11 and 21). Following atraumatic flapless extraction due to extensive root caries (tooth 11) and a chronic periapical granulomatous lesion with insufficient residual root length (tooth 21), two tapered implants (3.5×13 mm; Hiossen) wer

Percutaneous treatment of insufficiency fractures : principles, technique and review of literature.

Insufficiency fractures of the pelvis, sacrum, spine, and long bones are painful, debilitating, and are common consequences of osteoporosis. Conventional treatment for these fractures varies from conservative therapy to surgery with plate and screw fixation. The former fails to address the underlying problem of fracture and frequently does not alleviate symptoms, while the latter is invasive and not always possible in older populations with low bone density and numerous co-morbidities. Osseous augmentation with polymethylmethacrylate (PMMA) has been used for over two decades to treat fractures

Lessons learned from volar plate fixation of scaphoid fracture nonunions.

UNLABELLED: Treating scaphoid nonunions presents difficulties particularly when there is bone loss, significant humpback deformity or avascular necrosis. We describe a new type of fixation with a volar scaphoid plate that adds to the methods of internal fixation that are available for the treatment of recalcitrant scaphoid nonunions. We will also discuss 'lessons learned' from a cases series. The case series includes 20 consecutive patients treated with volar buttress plating and a pedicled vascularized bone graft from the ipsilateral volar distal radius. There was clinical and radiographic ev

Histomorphometric and biomechanical analyses of the osseointegration of loaded orthodontic microscrews inserted at different cortical bone thickness sites.

OBJECTIVES: The aim of this study was to evaluate the osseointegration of loaded orthodontic microscrews at different cortical bone thickness (CBT) sites. STUDY DESIGN: Forty-eight microscrews were inserted bilaterally in the tibias of 12 beagles, and divided into thick and thin CBT group. After 1, 3, 5, and 7 weeks, the dogs were killed and bone-screw specimens prepared for polyfluorochrome sequential labeling, microscopic computerized tomography (μCT) analysis, and biomechanical pull-out testing. RESULTS: All μCT parameters were increased in the thin CBT group, and decreased in the thick C

Open interlocked nailing without a targeting device or X-ray guidance for non-union of the femur: a case series.

From October 2005 to August 2007, we operated on six patients who had femoral non-unions and performed interlocked intramedullary nailing without X-ray guidance or a targeting device. There were three fractures of the distal femur, two fractures of the mid-shaft and one of the proximal femur. Fatigue failure of a non-interlocked Kuntscher nail and one nail migration were the presenting features in two patients. The presence of sclerosis of the bone ends in four cases and a need for cancellous bone grafts at the site of non-union in all patients made wide dissection and open reduction unavoidab

Return to Play in National Football League Players After Operative Jones Fracture Treatment.

BACKGROUND: Jones fractures commonly occur in professional athletes and operative treatment remains the standard of care in this patient population. In our clinical experience, an aggressive postoperative rehabilitation protocol for National Football League (NFL) players with an average return to play (RTP) between 8 and 10 weeks can have successful outcomes with few complications. The purpose of this study was to quantify RTP and rate of complications, including nonunion, refracture, and reoperation among a cohort of NFL players with operatively treated Jones fractures. METHODS: Between 2004

[Midcarpal arthrodesis with cortical bolting chip for treatment of grade II/III scaphoid non-union and scapholunate advanced collapse].

INTRODUCTION: These case reports describe a mediocarpal arthrodesis with excision of the scaphoid using a cortical bolting chip and screw fixation instead of the use of Kirschner wires or Spider plates. METHODS: Four patients with a symptomatic SLAC/SLAC-wrist >or= grade II were treated. The evaluation occurred six months later. For the evaluation the Cooney and Bussey as well as DASH scores were used (pain, strength, range of motion, functional status, possible limitations of quality of life, subjective perception of the patient). The radiological evaluation was performed by conventional wri

Operative Fixation of Chondral Loose Bodies in Osteochondritis Dissecans in the Knee: A Report of 5 Cases.

BACKGROUND: Osteochondritis dissecans (OCD) can progress to loose body formation, with or without subchondral bone attachment to the lesion. The efficacy of internal fixation of chondral loose bodies has not been determined. HYPOTHESIS: Operative fixation of cartilaginous loose bodies would result in (1) healed OCD at second-look arthroscopy, (2) restored cartilage appearance on magnetic resonance imaging (MRI), and (3) nearly normal knee function, as determined by patient-reported outcome scores. STUDY DESIGN: Retrospective case series; Level of evidence, 4. METHODS: Five patients who unde
