[Case of Creutzfeldt-Jakob syndrome with involvement of the neostriatum].

The authors describe a case of CJD in a man aged 45 years. The disease began with sluggishness of movements and speech difficulties, followed by development of pyramidal system damage and dementia. EEG findings were normal throughout the whole duration of the disease that is 3.5 years. Neuropathological examination disclosed major neuronal loss, spongy degeneration and astrogliosis in the striatum bilaterally. The cerebral and cerebellar cortex and brain stem showed similar but much less intense changes. This particular topography of the lesions was probably the cause of absent EEG changes.

The origins of the subcortical component of the median nerve somatosensory evoked potentials.

Short latency median nerve somatosensory evoked potentials (SEPs) were studied in two patients presenting with well localized unilateral pontine lesions producing hemisensory deficits. Both the early subcortical (N11-N13) and the later cortical component (N19-P22) of the SEP were abnormal in the patient whose lesion involved the medial lemniscus, while they remained unaffected in a second case of lateral inferior pontine infarction in which the medial lemniscus was spread. These results suggest that the N11-N13 component of the scalp recorded SEP emanates from the brain stem medial lemniscal p…

Abnormalities of the vestibulo-ocular response in congenital esotropia.

In 32 infants with congenital esotropia, I measured the vestibuloocular response. The eight patients with nystagmus compensation syndrome exhibited brisk responses. Fourteen of the 24 who had estropia without nystagmus had severely diminished responses and the other ten had moderate responses. Vestibulo-ocular testing is useful in detecting subtle forms of nystagmus compensation syndrome, especially in young infants, in whom thorough examination of ductions and versions is difficult. Dysfunction of the vestibular system is not necessarily a primary cause of congenital estropia, but brain-stem

[Clinical and biological symptoms of T. gambiense tryponosomiasis in the meningo-encephalitic period (author's transl)].

Main clinical symptoms are reviewed in the first part of the article. They consist in mental alertness disorder, motorial confusion and quite constantly in reflexes and abnormal movements due to brain stem lesion. The author reports on 22 cases. No correlation appears between prognosis and epileptiform seizures. Bertran's prognostical score has given contradictory indications. Neujan's criteria have not been very reliable to select treatment schedule. No correlation has been observed between biological data on the one hand and clinical symptoms as well as lethal evolution on the other hand.

Current evidence for the organic etiology of spastic dysphonia.

For over 100 years it has been universally assumed in the literature that spastic dysphonia is a functional or psychoneurotic voice disorder. In the last few years, new data have accumulated that support the concept that spastic dysphonia is caused by an organic, rather than a functional, abnormality. Histologic examination of segments of the recurrent laryngeal nerve removed from patients with spastic dysphonia has revealed myelin abnormalities in 30% of the nerves examined. Neurologic examination indicated brain stem or basal ganglia disturbances in some patients who had no apparent nerve di…

Hindbrain stroke in children caused by extracranial vertebral artery trauma.

Hindbrain transient ischemic attacks (TIAs) culminating in posterior circulation stroke are described in five children. Atlanto-axial subluxation and angiographical documentation of C1 to C2 level arterial pathology are documented in one patient. Four additional patients with nearly identical clinical presentations, posterior fossa TIAs, stroke and basilar angiographical pathology are reviewed. A mechanical traumatic etiology is suggested. Unexplained transient repeated brain stem and/or cerebellar sympotomatology may be due to extracranial vetebral artery stenosis or occlusion by atlanto-axia…

Courts, rights and the critically brain-injured patient.

The reality of current clinical practice in the UK is that where a patient's family refuses to agree to testing for brain stem death (BD), such cases will ultimately end up in court. This situation is true of both adults and children and reinforced by recent legal cases. While recourse to the courts might be regrettable in such tragic cases, if public trust in the medical diagnosis of BD is to be maintained all aspects of the process must be conducted in a way that is transparent and open to scrutiny. This is not an 'ineffective expenditure' of resources, but an essential element of a human ri…

[Approaches to the clinical classification of the functional status in patients with stroke].

The article presents the results of original investigation that included a detailed clinical analysis of 203 patients with stroke undergoing rehabilitation. The significance of pathogenic type of stroke and localization of the lesion in the brain are demonstrated. <<Rehabilitation subgroups>> are distinguished among the patients with ischemic stroke of hemispheric and stem localization, and hemorrhagic hemispheric stroke. For each ((rehabilitation subgroup) a detailed description of the key indicators of the neurological status, results of cognitive and psychological tests based on general and

Pharmacological protein targets in polyglutamine diseases: mutant polypeptides and their interactors.

Polyglutamine diseases are a group of pathologies affecting different parts of the brain and causing dysfunction and atrophy of certain neural cell populations. These diseases stem from mutations in various cellular genes that result in the synthesis of proteins with extended polyglutamine tracts. In particular, this concerns huntingtin, ataxins, and androgen receptor. These mutant proteins can form oligomers, aggregates, and, finally, aggresomes with distinct functions and different degrees of cytotoxicity. In this review, we analyze the effects of different forms of polyQ proteins on other p

Optimal treatment of a recurrent giant craniopharyngioma: lessons from a case.

A case of a 33-year-old man presented with symptoms of dramatic deterioration of the level of consciousness because of a recurrence of a previously aspirated and irradiated craniopharyngioma is described. The tumor had grown enormously in dimensions and was extending in the region of hypothalamus, third ventricle and brain stem, with signs of local compression and obstructive hydrocephalus. Radical surgical excision, despite the size and the location of the lesion, was the therapy of choice. The surgical technique is described. We conclude that in expert hands, microsurgery aiming at total rem…

Surgical perspectives of endoscopy of the cerebellopontine angle.

Owing to distal light and panoramic view, the endoscope expands the view inside the cerebellopontine angle without touching the cerebellum or the brain stem and allows clear identification of the vascular and nervous components crossing the cerebellopontine angle. Using a retrosigmoid approach with a combined surgical and endoscopic procedure, the surgeon gets more accurate information to visualize the adjacent structures to a small acoustic neuroma, to check the lateral part of the internal auditory canal, and to locate a vasculonervous cross-conflict. In the future, an endoscopic vestibular …

Symptomatic subependymoma--a case report.

Subependymoma is a rare, slow-growing, benign noninvasive tumor of the central nervous system that may be located in the fourth ventricle, the septum pellucidum, the third and the lateral ventricles, the aqueduct, and the proximal spinal cord. Symptoms, if any, usually result either from direct compression of the brain stem or from acute hydrocephalus due to occlusion of the foramen of Monro or aqueduct of Sylvius. In this report, we describe a case of subependymoma of the lateral ventricle with headache in a young female patient. This is the first reported case subependymoma in Korea that was…

The 'herald hemiparesis' of basilar artery occlusion.

In thrombosis of the basilar artery, not infrequently a hemiparesis is present at an early stage, when brain-stem signs may be absent or few, slight or equivocal. It is natural to suspect that such a hemiparesis signifies localization in a cerebral hemisphere, but unexpectedly in a few hours bilateral hemiplegia appears associated with coma or a locked-in syndrome, indicating basilar artery occlusion. I would term this disarming hemiparesis the "herald hemiparesis" of basilar artery occlusion. If the true nature of the condition is recognized early, the use of heparin and other measures may fo

[Migraine accompagnée in childhood and adolescence].

The term migraine accompagnée or complicated migraine is applied to those cases of migraine in whom a neurological deficit, which is transient in most instances, occurs. The three main varieties are hemiplegic (unilateral sensory loss, hemiplegia, aphasia), basilar (brain stem dysfunction) and ophthalmoplegic migraine. The clinical symptoms of migraine accompagnée in children and adolescents are discussed on the basis of the observations in 26 patients (11 boys, 15 girls) and the results of EEG-, CT- and angiographic investigations are referred. A special case of "migraine without headache" an…

[Neurotoxic syndrome in infectious diseases in children].

On the material of 72 children with infectious toxicoses the authors describe the encephalic syndrome as a manifestation of the neurotoxic syndrome. The neurotoxic syndrome was studied clinically and morphologically. A marked syndrome was characterized by a considerable edema of the brain and its stem and not infrequently by the vedging of the cerebellum into the great occipital orifice. Hemispheric focal symptomatology was typically associated with intracranial hemorrhage which was considered as a manifestation of the DIC syndrome. The authors believe that the neurotoxic syndrome in infectiou

[Whipple disease of the CNS].

By the example of two cases that could only be cleared up by a postmortem examination the affection of the central nervous system by Whipple's disease is dealt with. The granulomatous inflammatory process which is almost exclusively localised in the grisea and shows a particular concentration in the diencephalon, in the cortex of the frontal, temporal and islet regions as well as in the deeper brain stem, had resulted in a variety of neurological-psychiatric symptoms, in most cases a systemic psychosyndrome, myoclonism and paresis of ocular movement. In both cases the diagnoses was verified by…

Multiple meningiomas in separate neuroaxial compartments in a child.

A case of a child with multiple meningiomas in separated neuroaxial compartments is reported. The first tumor was in the frontal region and was removed surgically when first diagnosed 8 years prior to the appearance of the other tumor. The second tumor was located in the foramen magnum region, and reached considerable dimensions before becoming symptomatic and giving signs of brain stem compression as evidenced by progressive spastic tetraparesis and respiratory disturbances, without hydrocephalus. The diagnosis was confirmed by a low CT scan section, the tumor was successfully removed with th…

Surgical treatment of arteriovenous malformations of the posterior fossa.

Nine cases of arteriovenous malformations of the posterior fossa were operated upon, using microsurgical techniques. The excision was radical in eight patients. Seven of them, as well as the sole patient who had a partial removal of the malformation, returned to their previous occupations without neurological signs. In one case the preoperative deficit was unchanged. Some lesions that appear to penetrate the brain stem actually lie on its surface and can be dissected through an extrapial plane of cleavage. Extension of the malformation into the cerebellar peduncles requires dissection of the t…

[A case of a child who developed Cushing reflex after a clinical diagnosis of brain death].

The author presents a case of brain death caused by salmonella encephalopathy in a 1-year-old girl. The patient suffered from fever, convulsive status, and coma. On the 5th day, her respiration was arrested. At that time, we confirmed the absence of brainstem reflexes. An electroencephalogram (EEG) showed a flat pattern. Only wave I was recordable in auditory brainstem responses (ABR). On the 6th day, we obtained a flat EEG and a disappearance of all components of ABR. A simultaneous elevation of her heart rate and blood pressure was observed 14 to 18 hours after the respiratory arrest. This p
