Anti-neutrophil cytoplasmic antibodies (ANCA) in autoimmune liver disease.

ANCA were detected in 67% of sera from patients with primary sclerosing cholangitis (n = 15), in 83% of sera from patients with autoimmune chronic active hepatitis (n = 18) and in 35% of sera from patients with primary biliary cirrhosis (n = 20). ANCA were not detected in non-autoimmune chronic liver diseases. Characterization studies of the antigens involved showed that ANCA in autoimmune liver disease are not directed against myeloperoxidase (mpo), proteinase 3 (pr3) or elastase (hle), but the ANCA in autoimmune liver disease show reactivity with lactoferrin, a 67/66 kD protein combination o…

Chronic liver disease. The scope of causes and treatments.

Evaluation of chronic liver disease begins with a carefully taken history, thorough physical examination, and standard laboratory tests. Often, however, other studies are required, such as a viral hepatitis panel, serologic tests for autoimmune markers, tests for antimitochondrial antibodies, measurement of serum iron and ceruloplasmin levels, liver biopsy, and imaging studies of the extra-hepatic bile ducts. Medical treatment of chronic active hepatitis, primary biliary cirrhosis, and primary sclerosing cholangitis remains unsatisfactory. Early treatment of hemochromatosis and Wilson's diseas…

Death of an African elephant from probable toxemia attributed to chronic pulpitis.

A 31-year-old captive male African elephant (Loxodonta africana) of 5,000-kg body weight died suddenly in ventral recumbency. Lesions seen at necropsy were bilateral purulent pulpitis and periodontitis of both tusks, serous atrophy of coronary groove fat, Grammocephalus cholangitis, myocardial and skeletal lipofuscinosis, and scattered segmental necrosis in the pectoral muscles. Nonhemolytic streptococci, Corynebacterium sp, Peptostreptococcus anaerobius, Fusobacterium nucleatum, and Bacteroides sp, were recovered from the exudate around one or both tusks. We postulated that the elephant died …

Hepatocellular carcinoma and liver pathology in tonga.

All liver histology records from Tonga during 1970-79 were reviewed, including 42 biopsy and one necropsy specimens. Primary hepatocellular carcinoma was the histological diagnosis in 13 (30 percent). The age standardised incidence rate for confirmed cases are: males 2.9 and females 1.5 per 100 000. These are unremarkable compared to some reported rates, but many of these areas have included a high percentage of unconfirmed cases. Other diagnosis included cirrhosis (14), cholangiocarcinoma (2), metastatic carcinoma (2), cholangitis (2), hepatic fibrosis (2), and actinomycosis (1). The difficul…

Fascioliasis presenting as non-resolving liver abscess: Case series from eastern India.

Fascioliasis is a foodborne trematode infection caused by Fasciola hepatica and Fasciola gigantica. It is caused by ingestion of undercooked aquatic vegetation containing infective stage metacercariae. Symptoms in the acute stage include fever, abdominal pain and eosinophilia and in the chronic stage symptoms of biliary obstruction or cholangitis. Very few fascioliasis cases have been reported from India. In the last 1 year, we diagnosed six cases of fascioliasis who presented with acute stage symptoms. They were diagnosed by characteristic dietary history, liver abscess on imaging and eggs of…

Biliary Colonic Fistula Formation in the Setting of Obstructive Cholelithiasis.

Biliary enteric fistulas are rare form of internal biliary fistula encountered in <1% of patients with biliary disease while cholecystocolic … equired for diagnosis, given the presentation with nonspecific symptoms. Given the risk of imminent cholangitis, sepsis, and liver parenchymal damage, early treatment with cholecystectomy with the closure of the fistula is recommended.

Is a giant incisional hernia a contraindication for laparoscopic cholecystectomy?

Laparoscopic cholecystectomy (LC) is one of the most commonly performed surgical procedures worldwide. A previous abdominal operation is not considered a significant risk factor for conversion to open cholecystectomy. We describe the case of an 80-year-old woman with a surgical history of a giant uncomplicated incisional midline hernia presenting at our department with choledocholithiasis and acute cholangitis. After an ERCP with extraction of common bile duct stones, a LC was planned. The first trocar was inserted in the right midclavicular line, using an open technique and a careful inspecti…

Liver Transplantation for Hilar Cholangiocarcinoma.

Hilar cholangiocarcinoma (hCCA) is an infiltrative disease that often presents with locally advanced and/or metastatic disease, with a minority of patients eligible for surgical resection. Select patients with unresectable hCCA, or patients with hCCA in the setting of primary sclerosing cholangitis, with tumors less than 3 cm and no evidence of extrahepatic disease, can be effectively treated with neoadjuvant chemoradiation followed by liver transplantation. Staging laparotomy documenting lack of occult metastatic disease, including a portal lymphadenectomy documenting no nodal metastases, is …

Bending the distal tip of a guidewire aids in cannulation of an occluded uncovered biliary metal stent.

Biliary metal stent implantation is an effective treatment for malignant obstructive jaundice. But it's well known that stents put in for a long time can become occluded and cause jaundice and cholangitis. At this time, endoscopic intervention is usually required to replace the stent or re-insert the stent. Re-cannulation for metal stent occlusion is challenging because the guide wire may pass through the side holes of the uncovered metal stents, resulting in prolonged surgical time and exposure to radiation. Here we present a small tip that may help endoscopists complete the re-cannulation of…

Immunoglobulin G4-related Disease Accompanied by Peripheral Neuropathy: A Report of Two Cases.

Due to its rarity and the limited literature, the clinicopathological characteristics of peripheral nerve involvement in immunoglobulin G4 (IgG4)-related disease are unknown. We present two cases of IgG4-related disease, accompanied by peripheral neuropathy, presenting as unilateral ptosis (case 1) and sclerosing cholangitis (case 2), respectively. In both cases, sural nerve biopsy indicated vasculitis as the underlying pathophysiology; the peripheral neuropathy was refractory to corticosteroid therapy. In contrast to the previously proposed pathomechanism of IgG4-related neuropathy (direct ly…

Intravascular lipoma of the inferior vena cava and left renal vein.

Intravenous lipoma of the inferior vena cava is an incidental finding on contrast-enhanced computed tomography in 0.5% of individuals. We report a case of multiple intravenous lipomas discovered during diagnosis of cholangitis in a 39-year-old woman. Imaging revealed three fatty masses that appeared connected by cordlike structures: one in the left renal vein with wide mural attachment and two in the inferior vena cava, the higher of which was mobile. We hypothesize that these originated as a single lipoma that subsequently divided into three distinct masses. Because mobile masses may cause pu…

Double Whammy: Rare Case of Infected Chronic Seroma Due to Bacterial Translocation From Biliary Sepsis.

We present a case of infected chronic seroma post ventral hernia repair using the Rives-Stoppa technique likely from bacterial translocation from ascending cholangitis. After definitive treatment with endoscopic retrograde cholangiopancreatography (ERCP) and drainage of obstructed gallstones, she continued to show signs of sepsis. Percutaneous drainage of seroma was diagnostic for infection, where Escherichia coli (E. coli) was cultured and coupled with IV antibiotics, her infection was treated. To the best of our knowledge, this is the first case of seroma infection from biliary sepsis, and t…

[Antinuclear antibodies in the diagnostics of rheumatic diseases].

Testing for antinuclear antibodies (ANA) on human epithelial cell lines (HEp-2) using indirect immunofluorescence (IIF) is central for ruling out or for diagnosing connective tissue diseases and other diseases, such as primary biliary cholangitis and autoimmune hepatitis as well as drug-induced ANA. The comprehensive description of 29 different ANA-IIF patterns by the international consensus of ANA patterns (ICAP) facilitates the harmonization of ANA-IIF diagnostics. Positive ANA tests are frequently observed in healthy individuals and a reason for referral to rheumatologists. In these cases, …

Fungal and Parasitic Infections of the Liver.

Hepatosplenic candidiasis and other fungal infections of the liver are uncommon in healthy individuals; however, high index of suspicion is essential in immunocompromised patients with prolonged fever. Parasitic infections are protozoan or helminthic; their distribution and epidemiology are variable among different world regions. Clonorchiasis, opisthorchiasis, fascioliasis, and ascariasis are helminthic infections that commonly involve the biliary systems. Signs and symptoms of cholangitis require prompt management to relieve biliary obstruction; addition of antihelminthic agents is essential…

[Do we overtreat patients? Heavily increased risk for ERCP pancreatitis after treatment for asymptomatic common bile duct stones].

Common bile duct stones (CBDS) as well as ERCP treatment may cause jaundice, pancreatitis and cholangitis. More than half of the patients with asymptomatic CBDS found on intraoperative cholangiography pass all of their stones spontaneously. The ERCP treatment of asymptomatic common bile duct stones seems to carry a fivefold increased risk for complications, especially pancreatitis. It is unclear what carries the highest complication risk, untreated asymptomatic CBDS or ERCP treatment. The high risk for life-threatening complications combined with high rate of spontaneous stone passage raises t…

Ursodeoxycholic acid in pregnancy?

The case of a 34-year-old woman with primary biliary cholangitis (PBC) before, during and after pregnancy is described. The use of ursodeoxycholic acid (UDCA) during and after pregnancy is discussed. UDCA has not been approved by the drug regulatory authorities as a pregnancy-safe drug; therefore, the reluctance of clinicians to prescribe UDCA during pregnancy is understandable. This Grand Round aims to provide a detailed analysis of the current evidence, safety data and clinical experience with UDCA (and alternative drugs) during pregnancy and lactation. Based on this analysis, advice for cli…

[Hepatic dysfunction in patients with cardiogenic shock].

Cardiogenic shock is a life-threatening condition that is frequently associated with acute hepatic dysfunction. Due to low cardiac output resulting in end-organ hypoperfusion and hypoxia, different types of liver dysfunction can develop, such as hypoxic hepatitis or acute liver failure. A very serious and late sequela is the secondary sclerosing cholangitis in critically patients. Clinical management of acute hepatic dysfunction involves the stabilization of cardiac output to improve hepatic perfusion and the optimization of liver oxygenation. However, despite maximum efforts in supportive tre…

A Review of Extraintestinal Manifestations and Complications of Inflammatory Bowel Disease.

Extraintestinal manifestations (EIMs) are common in inflammatory bowel disease (IBD), in both Crohn's disease and ulcerative colitis. Almost any organ system can be affected, including the musculoskeletal, dermatologic, renal, hepatopancreatobiliary, pulmonary and ocular systems. However, the musculoskeletal and dermatologic systems are the most commonly involved sites of manifestations. While some manifestations such as peripheral arthritis and erythema nodosum have an association with IBD activity, others such as axial arthropathy, pyoderma gangrenosum and primary sclerosing cholangitis have…

Individualizing Care: Management Beyond Medical Therapy.

The evolving research landscape, with advances in the omics technologies, availability of large-scale patient cohorts, and forthcoming availability of novel drugs in primary biliary cholangitis (PBC), is creating a unique opportunity for developing a precision medicine (PM) program. PM has potential to change the paradigm of management. Diagnostic work-up of PBC patients may include information on genetic variants and molecular signature to define a particular subtype of disease and provide an estimate of treatment response and survival. To reach this point, specific interventions, such as seq…

A novel case of Raoultella planticola osteomyelitis and epidural abscess.

A spinal epidural abscess is the collection of pus in the epidural space, and is a potentially life-threatening condition that requires early detection and prompt management. Almost two-thirds of cases are caused by Staphylococcus aureus, followed by Gram-negative bacilli which account for approximately 16% of cases. Raoultella planticola is an emerging pathogen, and is an extremely rare cause of invasive infection in humans. It has been reported to cause urinary tract infections, pneumonia, bacteraemia, cholangitis, cholecystitis, conjunctivitis and soft tissue infections. We report the first…
