Allogeneic red blood cell transfusions: efficacy, risks, alternatives and indications.

Careful assessment of risks and benefits has to precede each decision on allogeneic red blood cell (RBC) transfusion. Currently, a number of key issues in transfusion medicine are highly controversial, most importantly the influence of different transfusion thresholds on clinical outcome. The aim of this article is to review current evidence on blood transfusions, to highlight 'hot topics' with respect to efficacy, outcome and risks, and to provide the reader with transfusion guidelines. In addition, a brief synopsis of transfusion alternatives will be given. Based on up-to-date information of

From static prediction to dynamic cancer trajectories: Virtual Human Twins for breast cancer decision support.

Digital oncology has become increasingly sophisticated at predicting cancer risk, treatment response, and prognosis. Yet many tools still operate as static snapshots, while cancer care unfolds as a trajectory shaped by tumor biology, genetics, treatment, residual disease, toxicity, and patient priorities. We argue that oncology needs dynamic, multiscale Virtual Human Twins (VHTs) that represent the patient and disease as they evolve together, integrating multimodal data to support longitudinal clinical reasoning and decision simulation rather than prediction alone. High-risk triple-negative br

[From medical statistics to medical stochastics].

Stochastics is a science that studies mathematical models trying to predict how a given phenomenon may evolve over time because of chance. It is necessary to abandon the binary thinking of "it works or it doesn't work" and rely on the logic of probability and randomness, which represent the measures of uncertainty people are destined to confront in their life and health choices. In making decisions shared with the patient, the physician must consider the baseline risk, the risk ratio, the variation in absolute risk, the competing risk within the patient, but also the number of patients needed

When 'Sanctity of Life' and 'Self-Determination' clash: Briggs versus Briggs [2016] EWCOP 53 - implications for policy and practice.

In a landmark judgment in the English Court of Protection, the judge (Charles J) found it to be in the best interests of a minimally conscious patient for clinically assisted nutrition and hydration (CANH) to be withdrawn, with the inevitable consequence that the patient would die. In making this judgment, it was accepted that the patient's level of consciousness - if CANH were continued and rehabilitation provided - might improve, and that he might become capable of expressing emotions and making simple choices. The decision to withdraw treatment relied on a best interests decision, which gav

[Guidelines for the sociomedical assessment of performance in patients suffering from breast cancer].

The following guidelines were developed for the medical assessment services of the German Pension Insurance Federal Institution. Starting from day-to-day practice, criteria and attributes to guide decisions for a systemisation of the sociomedical assessment of performance in breast cancer were compiled. The guidelines aim at standardising the sociomedical assessment of performance and help to make the decision-making process more transparent, e. g., for the assessment of applications for decreased earning capacity benefits. The guidelines summarise typical manifestations of breast cancer and d

Surgical decision making for residual limb deformities following traumatic brain injury.

When neurologic recovery has plateaued following traumatic brain injury, careful assessment is needed prior to making surgical decisions to correct residual limb deformities. Sufficient cognition to follow simple commands, cooperate with postoperative therapy, and benefit from improved function is essential. Sensation must also be intact. Motor control of the extremity can be evaluated by clinical examination supplemented by nerve blocks to relieve spasticity in antagonistic muscle groups. Dynamic electromyography is frequently required for surgical decision making. Hand placement may be deter

The Challenges of Prognosticating Persistent Critical Illness: A Qualitative Study.

BackgroundMedical decision-making in the ICU is influenced by a patient's anticipated clinical course. However, intensivists struggle to identify accurately which patients will experience within-ICU events-such as new late-onset shock or acute hypoxic respiratory failure (AHRF)-that contribute to the development of persistent critical illness (PerCI). Given the uniqueness of patients who experience PerCI, understanding how intensivists process and incorporate evolving clinical information into their prognostication remains unknown for this population.Research questionWhat processes do intensiv

A systematic literature review and (network) meta-analysis of the effectiveness of ceftolozane/tazobactam versus aminoglycosides/polymyxins and ceftazidime/avibactam for treating adult patients with multidrug-resistant Pseudomonas aeruginosa infections.

Multidrug-resistantant (MDR) Pseudomonas aeruginosa is a major public health concern necessitating new antimicrobials. There are new antimicrobials available with activity against MDR-P. aeruginosa, but there is a lack of robust evidence synthesis to guide clinical decision-making for patients with infections caused by MDR-P. aeruginosa. This study, which was supported by Merck & Co., Inc., aimed to evaluate the real-world effectiveness of ceftolozane/tazobactam (C/T) compared to other commonly used therapies (aminoglycosides/polymyxins and ceftazidime/avibactam [CZA]) among adults with MDR-P.…

Protocol for the external validation of machine learning models for risk prediction in early-onset preeclampsia: a multicenter prospective cohort study.

BACKGROUND: Early-onset preeclampsia (eoPE), defined as preeclampsia occurring before 34 weeks of gestation, remains one of the leading causes of severe maternal morbidity, iatrogenic prematurity, and maternal mortality worldwide. Its heterogeneous and rapidly evolving clinical course makes the prediction of adverse outcomes particularly challenging. Although machine learning (ML)-based models have shown promise in stratifying eoPE risk and supporting clinical decision-making, robust prospective external validation is still lacking. The present study aims to address this gap through a multicen…

The role of environmental impact assessments in hospital care: Healthcare professionals' views on research and implementation priorities.

BACKGROUND: The crossing of planetary boundaries, such as climate change and biosphere integrity, threatens human health, while healthcare systems paradoxically contribute substantially to these environmental challenges. Although research on the environmental impact of care activities and pathways is expanding, it remains unclear how this information is used in clinical practice. This study explores healthcare professionals' views on the environmental impact of hospital care, the role of environmental impact data and research and implementation priorities to support sustainability in clinical

Modelling midline shift and ventricle collapse in cerebral oedema following acute ischaemic stroke.

In ischaemic stroke, a large reduction in blood supply can lead to the breakdown of the blood-brain barrier and to cerebral oedema after reperfusion therapy. The resulting fluid accumulation in the brain may contribute to a significant rise in intracranial pressure (ICP) and tissue deformation. Changes in the level of ICP are essential for clinical decision-making and therapeutic strategies. However, the measurement of ICP is constrained by clinical techniques and obtaining the exact values of the ICP has proven challenging. In this study, we propose the first computational model for the simul…

Involved-site Radiotherapy Delineation Dilemmas in the Treatment of Adult Hodgkin Lymphoma: Turkish Society for Radiation Oncology Hematological Oncology, Pediatric Oncology and TBI Study Group Case-based Questionnaire Review (TROD 03-005).

AIMS: The International Lymphoma Radiation Oncology Group (ILROG) defined involved-site radiotherapy (ISRT) guidelines. These rules offer a certain variability that allows for autonomous decision-making in diverse clinical settings. However, this flexibility also gives rise to conflicts about the selection of treatment fields in the daily decision-making process. The aim of this study was to show the extent of interobserver variability when ILROG-ISRT recommendations were used in different clinical scenarios. MATERIALS AND METHODS: The 10-question survey used in our study consisted of two par

Clinical determinants impacting overall survival of patients with operable brain metastases from non-small cell lung cancer.

Non-small cell lung cancer (NSCLC) is currently the leading cause of cancer-related death worldwide, and the incidence of brain metastases (BM) in NSCLC patients is continuously increasing. The recent improvements of systemic treatment in NSCLC necessitate continuous updates on prognostic subgroups and factors determining overall survival (OS). In order to improve clinical decision-making in tumor boards, we investigated the clinical determinants affecting survival in patients with resectable NSCLC BM. A retrospective analysis was conducted of NSCLC patients with surgically resectable BM treat…

Altruism, Scepticism, and collective decision-making in foreign-born U.S. residents in a tuberculosis vaccine trial.

BACKGROUND: The current vaccine against tuberculosis, BCG, is effective when given in most TB-endemic countries at birth but has diminished efficacy against pulmonary TB after 15-20 years. As a result, new booster vaccines for adolescents and adults are being developed to realize the World Health Organization target of global elimination of TB by 2035. Multiple TB candidates thus are in active clinical development. METHODS: One of these, DAR-901, is advancing in human clinical trials. These clinical trials are conducted in BCG immunized adults with and without HIV infection in order to assess

Machine learning to predict survival in patients treated for hepatocellular carcinoma: a systematic review.

BACKGROUND: Given the interplay of cirrhosis with malignancy, management of hepatocellular carcinoma (HCC) relies on multidisciplinary clinician judgement to guide treatment. Accurate survival prediction would facilitate decision-making and resource allocation while maintaining patient-centred care with optimized survival outcomes while avoiding futility, particularly regarding liver transplant. Machine learning may offer innovative support in clinical decision making. This systematic review is the first to our knowledge to summarize existing machine learning algorithms predicting survival in
