Day Center experience in rehabilitation of craniocerebral injured patients.

The Day Center for head-injured patients specialises in treating patients at later stages following brain trauma. The goal of therapy is social reintegration as measured by the quality of family and social life and by actual occupational status. At this later stage, a year or more since injury, the cognitive and behavioral aspects of brain damage determine the outcome of rehabilitation. The therapy aims at preserving as well as improving patients' overall performance including family life and therefore their families are treated as well. As the patients are not hospitalised at this stage, the

The longitudinal impact of HIV+ parents' drug use on their adolescent children.

The impact of parental substance use on the emotional and behavioral adjustment of their adolescent children was examined over 5 years. A representative sample of 220 parents with HIV (PWH) and 330 adolescent children in New York City were repeatedly assessed. Some parents never used marijuana or hard drugs over the 5 years (nonusers). Among those who were users, substance use varied over time. PWH who used substances during a specific 3-month period were classified as active users and those who abstained from substance use were classified as inactive users. Longitudinal regression analyses we

Potential Mechanisms of Change in Virtual Reality-based Cognitive Behavioural Therapy and Standard Cognitive Behavioural Therapy for Schizophrenia-Related Paranoia: A Mediation Analysis from the FaceYourFears Trial.

BACKGROUND AND HYPOTHESIS: Cognitive models conceptualize paranoia in schizophrenia spectrum disorders as driven and maintained by mechanisms, which can be targeted by cognitive behavioral therapy for paranoia (CBTp). This study examined potential mechanisms as mediators. Safety behaviors, belief inflexibility, and core beliefs about self and others were hypothesized to differentially mediate outcomes in virtual reality-based CBTp (VR-CBTp) and CBTp, with safety behaviors expected to be more influential in VR-CBTp, and belief inflexibility and self-related core beliefs in CBTp. STUDY DESIGN: …

Post-Dialysis Syndrome: A Narrative Review.

Use of the term post-dialysis syndrome (PDS) was recently recommended to refer to the exacerbation of a debilitating, under-recognized clust … erventions for PDS include adjustment of dialysis prescription, exercise and physical activity, and cognitive behavioral therapy. Studies examining these interventions have focused on a narrow range of post-dialysis symptoms, however, and given the lack of PDS management guidelines, patients strongly advocate for further research into treatments. In the interim, we recommend …

Predictors of Parental Accommodation and Response Treatment in Young Children With Obsessive-Compulsive Disorder.

Background: Cognitive-behavioral family-based treatment (CBFT) is the treatment standard in very young children with obsessive-compulsive disorder (OCD), which includes the same core components of cognitive-behavioral therapy (CBT) with significant family involvement. Although the latter reports high rates of remission, some children do not improve with treatments. Therefore, it is necessary to identify possible moderating variables such as comorbidity, severity of disorder, years of onset, parental anxiety, and parental accommodation. This study has two main aims: (1) to propose a predictive

Integration of Clozapine-associated Harm Obsessions into Cognitive Behavioral Conceptualization and Treatment Planning for Thought Broadcasting: A Case Study.

BACKGROUND AND OBJECTIVES: As many as 30% of individuals with a schizophrenia spectrum disorder experience obsessive-compulsive symptoms (OCS). Clozapine has demonstrated superior efficacy for the treatment of medication-resistant schizophrenia but it is also associated with an increased risk for OCS. Because pharmacologic management of clozapine-related OCS can be particularly challenging, cognitive behavioral therapy (CBT) should be considered. Nevertheless, there are few detailed accounts of CBT for OCS and schizophrenia. METHODS: The authors describe the interdisciplinary outpatient care …

Cognitive psychophysiological treatment of bodily-focused repetitive behaviors in adults: An open trial.

BACKGROUND: Body-focused repetitive behaviors (BFRBs), such as hair pulling, skin picking, and nail biting, are repetitive, destructive, and nonfunctional habits that cause significant distress. Separate BFRBs form a cohesive group and could be assessed as part of the Tourette/tic spectrum or obsessive-compulsive spectrum of disorders. The treatment of choice is either antidepressant or behavioral treatment, both of which have shown effectiveness. The cognitive psychophysiological (CoPs) model focuses on the tension and emotional build up that triggers habits by addressing cognitive-behavioral

Does a community-based stress management intervention affect psychological adaptation among underserved black breast cancer survivors?

BACKGROUND: In this randomized trial, Project CARE, we examined whether participation in a cognitive-behavioral stress management and breast cancer wellness and education program improved psychological outcomes among a sample of underserved black breast cancer survivors. METHODS: Both complementary medicine interventions were 10-sessions, manualized, group-based, and were culturally adapted for black women in the community from evidence-based interventions. Participants were 114 black women (mean age = 51.1, 27-77 years) who had completed breast cancer treatment 0-12 months before enrollment

Qualitative investigation of the speech-language therapy experiences of individuals who covertly stutter.

PURPOSE: Individuals who covertly stutter have a unique experience of stuttering that involves concealing the core behaviors of stuttering (e.g., repetitions, prolongations, and blocks). From the listener's perspective, covert stuttering results in minimum typical, overt stuttering behaviors. However, from the speaker's perspective, covert stuttering often increases the cognitive and emotional impact of stuttering. This study explores the speech-language therapy experiences of individuals who covertly stutter in order to improve treatment recommendations and best practice. METHODS: This inves

Introduction to a theoretical model based on embodied cognition: a pilot study of the diagnosis and treatment of hemiphobia.

PURPOSE: Neurorehabilitation programs have traditionally followed the mind-body dichotomy proposed by Descartes, which resulted in the creation of divergent theoretical frameworks. The purpose of this pilot study is to present a transdisciplinary proposal that integrates human experience via the embodied cognition concept, that is, to explore how the individual relates from their experiential totality. We illustrate this model with a case report: a woman with a physical imbalance caused by stroke, which we diagnose as an experiential disorder and we treat using a transdisciplinary approach. M

Essential statistical principles of clinical trials of pain treatments.

This article presents an overview of fundamental statistical principles of clinical trials of pain treatments. Statistical considerations relevant to phase 2 proof of concept and phase 3 confirmatory randomized trials investigating efficacy and safety are discussed, including (1) research design; (2) endpoints and analyses; (3) sample size determination and statistical power; (4) missing data and trial estimands; (5) data monitoring and interim analyses; and (6) interpretation of results. Although clinical trials of pharmacologic treatments are emphasized, the key issues raised by these trials

Large-Scale Validation of the Paddling Pool Task in the Clockmaze for Studying Hippocampus-Based Spatial Cognition in Mice.

Rationally designed behavioral tests are important tools to assess the function of specific brain regions. The hippocampus is a crucial neural substrate for spatial cognition, and many studies have linked hippocampal dysfunction with defects on spatial learning and memory in neurological conditions ranging from Alzheimer's disease to autoimmune syndromes, such as neuropsychiatric lupus. While our understanding of hippocampal function, from the molecular to the system levels, has increased dramatically over the last decades, this effort has not yet translated into efficacious therapies for cogn

Cognitive performance in patients recovering from depression.

Investigations into the development of cognitive impairment in 30 hospitalized depressive patients have suggested that such disturbances encompass two dichotomous entities: a core entity of long-persisting, therapy-resistant impairment and an entity of reversible impairment with a prompt onset of improvement. The course of improvement turned out to evolve largely independently with respect to psychopathology: about one half of patients displayed severe cognitive dysfunctions which remained virtually unchanged until hospital discharge, while 80% significantly improved within the first 12 days w

Diminishing loss sensitivity during risky decision-making among male individuals with gambling disorder.

BACKGROUND AND AIMS: Gambling disorder (GD) poses severe impacts on both individuals and society. Impairment in risky decision-making is a key behavioral characteristic of GD, but the underlying cognitive processes of these deficits remain unclear. METHODS: A total of 100 male participants with GD and 59 healthy controls were recruited to complete psychological assessments and the Balloon Analog Risk Task. Since GD involved abnormal loss evaluation, we developed a novel cognitive model incorporating diminishing loss sensitivity and revealed the processes underlying the risk-taking behaviors w

Electroconvulsive therapy in erotomanic delusion: a path to belief updating.

OBJECTIVE: Erotomania is a rare delusional disorder marked by the persistent, unfounded belief that another person is in love with the affected individual. While antipsychotic medications may alleviate the intensity of delusions, the core belief often persists, leading to significant emotional distress and potential behavioral risks. The aim of this study was to illustrate the potential therapeutic role of electroconvulsive therapy (ECT) in the treatment of chronic, treatment-resistant erotomania. MATERIALS AND METHODS: We present a case report of a patient with a lifelong erotomanic delusion

Executive Dysfunction in Klinefelter Syndrome: Associations With Brain Activation and Testicular Failure.

CONTEXT: Executive dysfunction is a well-recognized component of the cognitive phenotype of Klinefelter syndrome (KS), yet the neural basis of KS-associated cognitive weaknesses, and their association with testicular failure is unknown. OBJECTIVE: We investigated executive function, brain activation, and pubertal development in adolescents with and without KS. METHODS: Forty-three adolescents with KS (mean age 12.3 ± 2.3 years) and 41 typically developing boys (mean age 11.9 ± 1.8 years) underwent pubertal evaluation, behavioral assessment, and completed functional magnetic resonance imaging

Cognitive remediation and occupational outcome in schizophrenia spectrum disorders: A 2year follow-up study.

Neurocognitive impairment is prominent in schizophrenia and a significant predictor of poor occupational outcomes. Vocational rehabilitation (VR) is frequently implemented to counteract high unemployment rates. Individuals with schizophrenia however face numerous challenges such as neurocognitive impairments and psychotic symptoms. Hence, augmenting VR to address illness-related factors may optimize occupational outcomes. The aim of this study was to examine the effects of Cognitive Remediation (CR) combined with VR(CR+VR) compared to techniques from Cognitive Behavioral Therapy (CBT) combined…
