Dihydroxy bile acids increase mucosal permeability and bacterial uptake in human colon biopsies.

OBJECTIVE: Bile acids in mM concentrations are known to increase chloride secretion and alter mucosal permeability in animal colon. Increased mucosal permeability is believed to play an important role in the development of intestinal inflammation. The aim of this study was to investigate the influence of microM concentrations of dihydroxy bile acids on permeability and bacterial uptake in the normal human colon. MATERIAL AND METHODS: Endoscopic biopsies from the sigmoid colon of 18 subjects with normal colonic histology were mounted in modified Ussing chambers. Chenodeoxycholic acid (CDCA) an

Clinical outcome of the laparoscopic surgery for stage II and III colorectal cancer.

BACKGROUND: Laparoscopic colorectal cancer surgery has become widely accepted recently. However, the oncological validity of this surgery has not yet been well analyzed, especially for advanced cancer. The aim of this study is to assess the clinical outcome of laparoscopic surgery for stage II/III colorectal cancer in our hospital. PATIENTS AND METHODS: Between June 1999 and August 2006, 321 patients underwent laparoscopic colorectal cancer surgery in our hospital; of those 121 cases whose pathological findings revealed stage II/III were included in this study. Among these cases, we assessed

Long-term risk of colorectal cancer after excision of rectosigmoid adenomas.

BACKGROUND AND METHODS: Surveillance by repeated colonoscopy is currently recommended for patients with colorectal adenomas. We assessed the long-term risk of colorectal cancer after rigid-instrument sigmoidoscopy and polypectomy in 1618 patients with rectosigmoid adenomas (tumor of the rectum or distal sigmoid colon) who did not undergo surveillance. A total of 22,462 person-years of observation were accrued (mean, 14 years per patient). RESULTS: The incidence of subsequent rectal cancer in these patients was similar to that in the general population (standardized incidence ratio, 1.2; 95 pe

Elective Treatment of Large-Bowel Obstruction in Asymptomatic Sigmoid Volvulus.

BACKGROUND: Sigmoid volvulus is an uncommon cause of intestinal obstruction representing the 5% of all Western cases, associated with old age and a history of neurological and psychiatric condition. Generally, its diagnosis is established by clinical and radiologic findings. It often represents an emergency and it is commonly associated with pain, vomit and abdominal tenderness. CASE PRESENTATION: We present a case of a 59 years old man, admitted to our emergency department, showing an abdominal X-Ray reporting a distention of large bowel, which was required due to presence of multiple diarrhe

Accuracy of ultrasound in prediction of rectosigmoid infiltration in epithelial ovarian cancer.

OBJECTIVE: To examine prospectively the accuracy of ultrasound in predicting rectosigmoid tumor infiltration in patients with epithelial ovarian cancer. METHODS: Patients referred for a suspicious pelvic mass between 2012 and 2014 were examined by ultrasound following the standard protocol for assessment of tumor infiltration. Of the 245 patients examined, 191 had proven ovarian cancer and underwent primary surgery and were included in the analysis. Patients with apparently benign or inoperable disease were excluded. Rectosigmoid infiltration was evaluated by histopathology or according to pe

Feasibility of single-stage resection and primary anastomosis in patients with acute noncomplicated sigmoid volvulus.

BACKGROUND: The management of sigmoid volvulus remains controversial. The aim of this study was to evaluate the feasibility of treating sigmoid volvulus by using a single-stage resection and anastomosis versus a 2-stage approach. METHODS: A total of 136 patients with noncomplicated sigmoid volvulus who had undergone emergency surgery within the last 15 years were evaluated retrospectively. Sigmoid resection plus Hartmann colostomy was performed in 45 patients, and sigmoid resection plus primary anastomosis was performed in 91 patients. In 40 of the patients who underwent a Hartmann procedure,

Skip segment Hirschsprung's disease: a systematic review.

PURPOSE: Hirschsprung's disease is characterised by the congenital absence of ganglion cells beginning in the distal rectum and extending proximally for varying distances. 'Zonal aganglionosis' is a phenomenon involving a zone of aganglionosis occurring within normally innervated intestine. 'Skip segment' Hirschsprung's disease (SSHD) involves a 'skip area' of normally ganglionated intestine, surrounded proximally and distally by aganglionosis. While Hirschsprung's disease is believed to be the result of incomplete craniocaudal migration of neural crest-derived cells, the occurrence of SSHD ha

Frequency and clinical impact of Dolichocolon in women submitted to surgery for rectosigmoid endometriosis.

INTRODUCTION: Rectosigmoid endometriosis and Dolichocolon can both present with a triad of chronic abdominal pain, constipation and bloating. The relationship between these two pathologies is unknown. The present study aims to determine the frequency of DC in women with rectosigmoid endometriosis and its possible impact on pre- and post-operative symptoms. MATERIAL AND METHODS: We conducted a retrospective cohort study on 113 consecutive patients submitted to magnetic resonance imaging enema and subsequent complete surgical removal for symptomatic rectosigmoid endometriosis between June 2015

Hartmann's procedure versus sigmoidectomy with primary anastomosis for perforated diverticulitis with purulent or faecal peritonitis (LADIES): a multicentre, parallel-group, randomised, open-label, superiority trial.

BACKGROUND: Previous studies have suggested that sigmoidectomy with primary anastomosis is superior to Hartmann's procedure. The likelihood of stoma reversal after primary anastomosis has been reported to be higher and reversal seems to be associated with lower morbidity and mortality. Although promising, results from these previous studies remain uncertain because of potential selection bias. Therefore, this study aimed to assess outcomes after Hartmann's procedure versus sigmoidectomy with primary anastomosis, with or without defunctioning ileostomy, for perforated diverticulitis with purule

[A rare cause of hepatic coma].

CASE REPORT: A 65-year-old female patient was admitted to the hospital in somnolent state. During physical examination, she had an increase in blood pressure and a positive bilateral Babinski's sign. Laboratory findings showed elevated liver enzymes, metabolic alkalosis and slightly elevated kidney values. 3 h hours later she lapsed into a hepatic coma (ammonia > 400 micrograms/dl). Subsequently, indirect anamnesis revealed the following affections: ureterosigmoidostomy in 1942, cystectomy in 1943, and right-sided nephrectomy in 1950. DISCUSSION: This case presents with an uncommon origin of

The anatomic distribution of colorectal cancer in a New York City Puerto Rican group.

Studies have attempted to define the anatomic distribution of colorectal cancer in some black and white groups of the U.S. However, little, if any research has looked at the regional distribution of colorectal cancer in an American Hispanic, especially Puerto Rican, group. This study attempts to provide some insight into the subsegmental distribution of colorectal cancer in this group of the American population which has a heavy concentration of people in many major U.S. cities. We retrospectively reviewed the charts of Puerto Rican patients who had colorectal adenocarcinoma and were on the fi

[Intraoperative colonic irrigation in the emergency treatment of occlusive lesions of the left colon].

This study reports the use of an intraoperative antegrade colonic irrigation in the management of left-sided large bowel obstruction requiring emergency surgery. 35 consecutive patients had primary bowel resection with immediate anastomosis (without colostomy) after intraoperative antegrade colonic irrigation. The cause of the obstruction was large bowel carcinoma in 26, diverticulitis in seven and volvulus in two cases. There were two post-operative deaths (5.7%). No digestive fistula was observed. The results of this study suggest that intraoperative colonic irrigation is an effective method

[Sixty-one cases of acute obstructions of the colon caused by cancer. Indications for emergency surgery].

Sixty-one cases of acute malignant colonic obstruction were operated upon within 24 hours after the patient's admission, between 1979 and 1989: 10 for a right colon carcinoma, 51 for a left-sided lesion. Indications, for emergency operation were: complete clinical obstruction, iliac tenderness, radiologic ileal fluid levels and arrest on barium enema. The initial procedures consisted of 8 right hemicolectomies, and for the left-sided obstructions, 32 colostomies, 7 colectomies with immediate anastomosis, and 12 colectomies without anastomosis. Overall mortality was nil for the right side and 4

The treatment of perforated diverticulitis by one-stage intracolonic bypass procedure.

The one-stage intracolonic bypass procedure prevents gastrointestinal secretions and fecal content from coming in contact with an anastomotic site without interrupting the intraluminal continuity of the fecal flow from proximal to distal colon. This can be achieved by the intraluminal implantation of a soft, pliable tube above the anastomotic site. Previous clinical and experimental data have indicated that the intracolonic bypass procedure can protect an anastomosis in the presence of maximal colonic loading, gross dehiscences, or fecal peritonitis. This report presents 28 patients with perfo

Giant sigmoidal diverticulum: report of a case and review of the literature.

A case report of giant sigmoidal diverticulum and a review of the relevant medical literature are presented. Only 20 cases have previously been reported. Diagnosis is dependent on a gradually enlarging mass, usually found on physicial examination. Radiologic study usually confirms the diagnosis and differentiates this condition from others, such as intestinal duplication, emphysematous cholecystitis, cholecysto-enteric fistulas, etc. The treatment is prophylactic. It is important to undertake early surgical intervention before the condition progresses to perforation or other complications.

Mesenteric panniculitis: review of the leterature and presentation of cases.

Sixty-two cases of mesenteric panniculitis collected from the literature and six cases treated by the authors are reviewed. Symptomatology, pathology, treatment, and outcome of this disorder are discussed. It seems that mesenteric panniculitis is a well established clinical entity of unknown etiology, with a benign course and favourable outcome. Radical surgical treatment is not advised. The sole surgical intervention advocated would be deviation of the gastrointestinal tract in patients with symptoms of obstruction.
