Distal humerus nonunion in adults: Management strategies and contemporary outcomes.

Distal humerus nonunion (DHN) in adults is an uncommon but highly disabling condition that poses significant reconstructive challenges. The complexity arises from a combination of poor local biology, limited metaphyseal bone stock, and the biomechanical demands of the elbow joint, often resulting in pain, instability, stiffness, and functional loss. We aim to emphasize current evidence on the pathoanatomy, diagnostic approach, and contemporary management strategies for distal humerus nonunion, with a focus on decision-making between osteosynthesis (including revision fixation of previously ope…

Effect of a 6-Week Weighted Baseball Throwing Program on Pitch Velocity, Pitching Arm Biomechanics, Passive Range of Motion, and Injury Rates.

BACKGROUND: Emphasis on enhancing baseball pitch velocity has become popular, especially through weighted-ball throwing. However, little is known about the physical effects or safety of these programs. The purpose of this study was to examine the effects of training with weighted baseballs on pitch velocity, passive range of motion (PROM), muscle strength, elbow torque, and injury rates. HYPOTHESIS: A 6-week weighted ball training program would result in a change in pitching biomechanical and physical characteristics. STUDY DESIGN: Randomized controlled trial. LEVEL OF EVIDENCE: Level 1. M…

Radiological, clinical and functional evaluation using the Quick Disabilities of the Arm, Shoulder and Hand questionnaire of children with medial epicondyle fractures treated surgically.

PURPOSE: The main goal of this study was to retrospectively evaluate the clinical, functional and radiographic outcomes of displaced medial epicondyle fractures with or without associated elbow dislocation in children undergoing surgical treatment and to evaluate upper-extremity function with the Quick Disbilities of the Arm, Shoulder and Hand (Quick DASH) questionnaire. The hypothesis was that patients with associated elbow dislocation would have similar functional outcome to patients with displaced fractures with or without intra-articular entrapment of the medial epicondyle. METHODS: A rev…

Stabilisation of articular fractures of the distal humerus: early experience with a novel perpendicular plate system.

INTRODUCTION: The treatment of fractures of the distal humerus is difficult and involves the risk of bad functional results, particularly with articular fractures or impaired bone quality. Anatomical reconstruction and stable fixation allowing early mobilisation of the elbow are the basic prerequisites for good clinical outcomes. The aim of our study was to evaluate whether the Distal Humerus Plate (DHP, Synthes), a novel, perpendicular, fixed-angle plate system, fulfils the requirements for this treatment goal. METHODS: From 2004 to 2007, 46 consecutive patients (19 men and 21 women; mean ag…

Atypical joint involvement in neuropathic arthropathy secondary to syringomyelia: A case report.

Neuropathic arthropathy (NA) is a chronic, progressive, destructive, aseptic inflammatory joint disorder resulting from sensory neurological deficit. We report a new case of NA with simultaneous involvement of the shoulder, elbow, and hands secondary to syringomyelia. Due to the rarity of this condition and its unusual location. The patient presented with painless deformation of both hands. Clinical examination revealed amyotrophy in both hands associated with hypoesthesia and bilateral areflexia of the upper limbs. The plain radiographs showed bone destruction and reconstruction in the metaca…

Posterior Interosseous Nerve Injury From Chronic Monteggia Fracture Dislocation Requiring Nerve Reconstruction.

UNLABELLED: Monteggia fracture-dislocations are rare elbow injuries in the pediatric population. Multiple case reports have described posterior interosseous nerve (PIN) palsies in the setting of Monteggia fracture-dislocations, usually due to stretch neuropraxia. In this report of a Bado Type III Monteggia fracture-dislocations with delayed diagnosis of an entrapped PIN, we highlight the importance of timely diagnosis. Surgical reconstruction using a sural nerve autograft was required when the PIN was found to be entrapped in the radiocapitellar joint and encircling the radial neck. KEY CONCE…

Engagement of the Radial Head in Posterior Capitellum Fracture: Defining the Critical Point.

INTRODUCTION: Elbow dislocation is usually treated conservatively with a good outcome. When there is associated injury to the capitellum, "Osborne-Cotterill Lesion," then the clinical course is less predictable and controversial. In this article, we attempt to define the critical point on the capitellum surface that, when breached, forebodes potential engagement of the radial head in the defect and imminent dislocation. CASE REPORT: We present two cases of fracture and fracture dislocation of the elbow with "Osborne-Cotterill lesion." They were treated with fracture reduction and fixation. In…

Electrodiagnostic Testing of Entrapment Neuropathies: A Review of Existing Guidelines.

Entrapment neuropathies cover a wide range of isolated nerve injuries along the course of the upper and lower extremity nerves. Electrodiagnostic (EDX) testing is usually an essential part of the evaluation of entrapment neuropathies, and examinations for the most common entrapment neuropathies, carpal tunnel syndrome and ulnar neuropathy at the elbow, constitute a significant part of the daily work in EDX laboratories. Despite this, guidelines for EDX testing are generally sparse or do not exist for entrapment neuropathies, whereas a wide variety of different techniques are available to the c…

Modified Jobe Approach With Docking Technique for Ulnar Collateral Ligament Reconstruction.

The ulnar collateral ligament (UCL) of the elbow acts as the primary restraint to valgus force experienced in the late cocking and early acceleration phases of overhead throwing. If the UCL or dynamic flexor-pronator musculature is incompetent, elbow extension and valgus torque, as seen in throwing, can result in posteromedial impingement with subsequent chondromalacia and osteophyte formation. Before the first UCL reconstruction, performed by Frank Jobe in 1974, this injury was considered career ending in overhead athletes. Since the index procedure, further techniques have been developed to …

Isolated Avulsion of Extensor Carpi Radialis Longus and Brachioradialis Origins: A Case Report and Surgical Repair Technique.

The mobile wad of the elbow provides a tremendous mechanical advantage with respect to elbow flexion and wrist extension. Injury to these structures causes significant upper extremity dysfunction. In this article, we report the case of a 31-year-old right hand-dominant man who sustained an isolated avulsion of the extensor carpi radialis longus and brachioradialis origins from the lateral epicondyle and lateral supracondylar ridge. We describe our diagnostic workup and present our surgical repair technique. The literature includes only 2 case reports of bony avulsion fracture of the origin of …
