For an expert opinion in a lawsuit regarding the suspicion of self-mutilation the physical data of an accident on a circular saw bench have been investigated. The result was as follows: The time required for severing a human forearm of 6 cm diameter is 40 to 80 ms depending on the feeding power. The impulse of 13 to 15 Ns necessary for severing an arm in only 1/10 to 2/10 of the impulse a slipping person exerts when striking the disk. The standard time data for such accidents indicated in the scientific literature are much too high.

Three cases of musculocutaneous nerve injury distal to its innervation of the coracobrachialis muscle are reported. The syndrome typically features painless weakness of the biceps brachii and brachialis muscles, sensory loss in the distal volar forearm, a history of recent vigorous upper extremity resistive exercise, and resolution of the problem on resting the affected extremity. In all three reported cases the dominant arm was involved. The condition should not be confused with C-5 or C-6 radiculopathy, brachial plexopathy, or rupture of the biceps brachii muscle belly or tendon.

We have presented methods for treating extensor tendon injuries from the interphalangeal joint to the wrist and the musculotendinous junction in the forearm. Early and proper splinting in the treatment of extensor tendon injuries is more important than a specific method of surgical repair. We emphasize the need for prolonged splinting, up to eight weeks in distal injuries. CASE: A 62-year-old right-hand-dominant woman presented with 1 year of persistent radiating pain, weakness, and paresthesias in her left forearm and hand. Electromyography findings were significant for ulnar neuropathy distal to the branch innervating the flexor carpi ulnaris (FCU), without superimposed cervical radiculopathy or other focal entrapment neuropathy. During open ulnar nerve neurolysis, an intramuscular lipoma was encountered within the FCU. CASE: A 20-year-old right hand dominant woman presented with an audible and painful snapping wrist occurring with finger flexion. This bothersome mechanical finding and concomitant carpal tunnel syndrome (CTS) did not resolve with conservative measures. Dynamic magnetic resonance imaging revealed anomalous flexor digitorum profundus (FDP) muscle bellies as the cause of the mechanical snapping and irritation of the median nerve.
